SUMMARYA functional analysis of four different types of volume‐controlled ventilators has been made with special emphasis onforceandpoweras developed in the force generators and in a simulated patient circuit. The power, which has the dimensions of pressure, time and gas‐flow, has been measured by automatic multiplication. The ventilators have been classified according to the available force and two main groups are distinguished: (1)Constant force generatorswith (a) non‐adjustable flow and (b) adjustable flow; (2)increasing force generatorswith (a) direct action and (b) indirect action on the patient circuit. Measurements of force and power were made at the output of the force generators as well as in the patient circuit during different degrees of flow resistance at equal tidal volume and respiratory frequency. The constant force generator with non‐adjustable flow had a maximum power of 0.7 kpm/sec., which decreases to 0.25 kpm/sec. in the patient circuit. The pressure‐flow‐time diagram shows that pressure and flow can never increase simultaneously, as an increase of pressure results in a decrease in flow. Theconstant force generatorwithadjustable flowhas a maximum power of 0.1 kpm/sec. at a flow rate of 0.38 litre/sec. The “alveolar”‐pressure curve for different degrees of airway resistance is deduced, and it can be shown that a negative phase during expiration with this ventilator only had a minimal effect of lowering the alveolar pressure when a high airway resistance was present.Theincreasing force generatorscan develop high power,i.e.0.85 kpm/sec. or more. Thegenerator with direct actionon the patient circuit has a gas flow which is proportional to the speed of the bellows. It is therefore relatively inflexible and has slight possibilities of adjusting the developed power to various time constants in the patient circuit. Pressure and flow are synchronised with each other with a maximum at the end of the expiration. The generator withindirect actionon the patient circuit could develop a power of 1 kpm/sec. or more. It could adjust its power pattern in a way that corresponded to various time constants of the patient's lungs. The power and time characteristics of a negative expiratory phase for the ventilators were analysed with respect to intrapulmonary pressures developed at the end of expiration and to the possibilities of synchronisation of spontaneous breathing within the frequency of the ventilator.The physiological aspects of the various ventilators are discussed on the basis of power demands in patients with severe cardiopulmonary failure. It has been shown that such patients may require power outputs as high as 0.8 to 1.0 kpm/sec. of the ventilators. This is of importance when ventilators of different design are used clinically. The physiological effects of the gas distribution achieved with different force‐power patterns are discussed, and it is believed that an increasing force generator with an increasing flow and a power maximum at the end of the inspiratory phase is better than a decreasing‐flow, low‐power‐force generator.ZUSAMMENFASSUNGEs wurde eine Funktionsanalyse von vier verschiedenen Typen volumengesteuerter Ventilatoren durchgeführt mit besonderer Berücksichtigung von Druck (→force←) und Kraft (→power←), wie sie in den Druckgeneratoren und in einem künstlich nachgeahmten Patienten‐Kreissystem entstehen. Die Kraft, welche die Dimensionen von Druck, Zeit und Gasstrom beinhaltet, wurde durch automatische Multiplikation gemessen. Die Ventilatoren wurden entsprechend dem zur Verfügung stehenden Druck klassifiziert und dabei zwei Hauptgruppen unterschieden: 1) Generatoren, die einen konstanten Druck liefern, mit a) unverstellbarem Strom und b) verstellbarem Strom; 2) Generatoren, die einen ansteigenden Druck liefern, mit a) direkter Wirkung und b) indirekter Wirkung auf das Patienten‐Kreissystem. Messungen von Druck und Kraft wurden sowohl beim Auslass des Druckgenerators als auch im Patientenkreis bei verschieden starken Strömungswiderständen jedoch bei gleichbleibendem Atemzeitvolumen und gleichbleibender Atemfrequenz vorge‐nommen. Der Konstantdruck‐Generator mit unverstellbarem Gasstrom lieferte eine Maximalkraft von 0,7 kpm/sec, die sich im Patientenkreis auf 0,25 kpm/sec reduzierte. Das Druck‐Strom‐Zeit‐Diagramm zeigt, dass Druck und Strom nie gleichzeitig ansteigen können, da ein Druckanstieg einen Strömungsabfall zur Folge hat. Der Konstantdruck‐Generator mit verstellbarem Gasstrom lieferte eine Maximalkraft von 0,1 kpm/sec bei einem Gasstrom von 0,38 1/sec. Es wurde nun die →Alveolar←‐Druckkurve für verschieden hohe Atemwegswider‐stände abgeleitet, wobei gezeigt werden konnte, dass eine negative Phase während der Exspiration mit diesen Ventilatoren nur einen ganz geringen Einfluss auf eine alveolare Druckminderung hatte, wenn ein hoher Atemwiderstand vorlag.Die druckerhöhenden Generatoren konnten eine hohe Kraft entwickeln, nämlich 0,85 kpm/sec und mehr. Der Generator mit direkter Wirkung auf den Patientenkreis hat einen Gasstrom, der sich proportional zur Geschwin‐digkeit der Faltenbälge verhält. Er ist daher relativ starr und bietet nur geringe Möglichkeiten die entwickelte Kraft den verschiedenen Zeitkonstanten im Patientenkreissystem anzupassen. Der Generator mit indirekter Wirkung auf den Patientenkreis konnte eine Kraft von 1 kpm/sec oder mehr entwickeln. Er konnte sein Kraftmuster in einer Weise verändern, dass es mit den verschiedenen Zeitkonstanten der Patientenlunge korrespondierte. Die Kraft‐ und Zeitcharakteristika einer negativen Phase wurden für den Ventilator analysiert in Hinblick auf die intrapulmonalen Drucke, die am Ende der Exspiration entstanden und auf die Möglichkeiten einer Synchronisation der Spontanatmung mit der Frequenz des Ventilators.Die physiologischen Aspekte der verschiedenen Ventilatoren werden auf Grund der Krafterfordernis für Patienten mit schwerem cardiopulmonalen Versagen diskutiert. Es war nachgewiesen worden, dass solche Patienten einen Kraftausstoss der Ventilatoren von 0,8–1,0 kpm/sec benötigten. Dies ist von Bedeutung, wenn Ventilatoren verschiedener Bauweise klinisch angewendet werden. Die physiologischen Auswirkungen der Gasverteilung, die mit verschiedenen Druck‐Kraft‐Mustern erreicht werden, werden besprochen. Dabei wird die Meinung vertreten, dass ein drucksteigernder Generator mit ansteigendem Gasstrom und einem Kraftmaximum am Ende der Inspirat
