SUMMARYMethods are described for studies of the pulmonary ventilation, mechanics of breathing and alveolar CO2concentration during artificial respirator ventilation during surgical procedures.The characteristic feature of the present measuring system for recording flow and volume was that only the insufflation phase was measured; the gas compressed in the respirator was separated by a valve system from the expired gas, and the zero reset control of the volume integrator was triggered by the respirator after each recorded breath.Calibrations, sources of error and overall accuracy of the measurements are discussed.ZUSAMMENFASSUNGMethoden zum Studium der Lungenventilation, der Atemmechanik und der alveolären C02Konzentration während künstlischcr Respiratorbeatmung bei chirurgischen Eingriffen werden beschrieben.Das charakteristische Merkmal des mitgeteilten Messsystems zur Registrierung von Strömurig und Volumen ist die alleinige Messung der Insufflationsphase, wobei das im Respirator komprimierte Gas durch ein Ventilsystem vom Ausatmungsgas getrennt wird, und die Nullstellung der Volumenregistrierung automatisch vom Respirator nach jedem registrierten Atemzug erfolgt.Die Kalibrierung, die Fehlergrenzen und die totale Messgenauigkeit werden diskuti

SUMMARYTracheal pressure, gas flow and tidal volume were recorded in 23 children aged from 1 day to 23 months during surgery and respiratory ventilation in order to clarify changes in pulmonary mechanical relationships. Of the surgical procedures, six were intrathoracic, nine abdominal and eight minor external procedures. Through administration of varying tidal volumes it was shown that total compliance remained unchanged with an insufflation pressure of less than 25 cm H2O.The value for pre‐operatively recorded total compliance revealed linear correlation with body weight.Large instantaneous variations of total compliance were demonstrated during surgical procedures. In three cases, a reduction of total compliance amounting to more than 75% of the control value was recorded. Reduction of total compliance could be related to mechanical restriction of lung expansion caused by surgical activity. Large momentary reductions in total compliance were produced experimentally by epigastric pressure prior to surgery.Following operation, as well as after release of the epigastric pressure, a certain reduction in compliance persisted. This could be almost completely abolished by hyperinflation.The significance of the results for measurement and maintenance of adequate ventilation during surgery is discussed.ZUSAMMENFASSUNGBei 23 Kindern im Alter von einemTag bis 23 Monaten wurde derTracheal‐druck, der Gasaustausch und das Atemzugvolumen während der Operation bei Beatmung mit dem Respirator registriert, urn eventuelle Veränderungen der atemmechanischen Verhältnisse beurteilen zu können. Bei den Operationen handelte es sich um 6 intrathorakale, 9 abdominelle und 8 kleinere bezw. äussere Eingriffe.Indem verschiedene Atemzugvolumina eingestellt wurden, konnte nach‐gewiesen werden, dass sich der Wert für die Total‐Compliance (Volumen‐Druck‐Koeffizient) bei einem Insufflationsdruck unter 25 cm H2O konstant verhielt.Der vor der Operation registrierte Wert für die Total‐Compliance zeigte eine gradlinige Korrelation zum Körpergewicht.Während des chirurgischen Eingriffes konnten grosse, schnelle Veränderungen der Total‐Compliance nachgewiesen werden. In 3 Fällen wurde eine Erniedrigung derselben mit iiber 3/4 des Ausgangswertes registriert. Diese Erniedrigung konnte mit der mechanischen Behinderung der Lungenausdehnung als Folge der operativen Tätigkeit in Zusammenhang gebracht werden.Bei experimentellem Druck auf das Epigastrium vor der Operation konnte ebenfalls eine grosse, plotzliche Erniedrigung der Total‐Compliance hervor‐gerufen werden.Nach der Operation sowie nach Entfernung des experimentellen Druckes auf das Epigastrium verblieb die Total‐Compliance etwas erniedrigt, doch konnte diese Erniedrigung durch Hyperinflation normalisiert werden.Die Bedeutung der gewonnenen Ergebnisse für die Einstellung und Aufrechterhaltung einer adäquaten Ventilation w

SUMMARYTwo methods for calculation of the administered pulmonary ventilatory volume during artificial ventilation with the Engström respirator have been investigated in a case material consisting of 17 infants aged from 1 day to 23 months.The effect of the ventilation during the influence of surgical procedures was assessed through measurement of carbon dioxide tension in arterialized capillary blood and infra‐red analysis of the carbon dioxide content of “alveolar” gas.A very good agreement between the calculated and measured pulmonary ventilation could be demonstrated.The carbon dioxide analyses revealed that during ventilation based on Radford's standards a mild hyperventilation is produced.ZUSAMMENFASSUNGBei 17 Säuglingen und Kleinkindern im Alter von 1 Tag bis 23 Monaten wurden 2 Methoden zur Berechnung der Lungenventilation bei künstlicher Beatmung mit dem Engström Respirator untersucht.Der Effekt der Beatmung wurde durch Messung der Kohlendioxyd‐Konzen‐tration im arterialisierten Kapillarblut und durch Infrarotanalyse der Kohlen‐dioxyd‐Fraktion im “Alveolargas” beurteilt.Zwischen den berechneten und den gemessenen Werten fur die Lungenventilation fand sich eine sehr gute Übereinstimmung.Die Kohlendioxyd‐Analysen liessen erkennen, dass bei Berechnung der Beatmung nach dem Radford'schen Standard eine leichte

SUMMARYThe intracellular pH of human erythrocytes in nine subjects was calculated by the DMO method and compared with direct measurements on the hemo‐lysate. The standard deviation of differences between means of duplicate observations of the two methods was ± 0.111 pH unit. The amount of DMO‐like substance in normal human blood was found to vary from one subject to another and must be taken into account in the calculations. It is concluded that the DMO method is of limited accuracy in estimating intracellular pH of human erythrocytes, but it may be useful in evaluating the extent of any changes.ZUSAMMENFASSUNGDas nach der DMO‐Methode bei 9 Versuchspersonen errechnete intracelluläre pH menschlicher Erythrozyten wurde mit direkten Messungen im Haemolysat verglichen. Die Standardabweichung der Unterschiede zwischen den Mittelwerten der beiden Methoden, die jeweils aus doppelten Beobach‐tungen stammten, betrug ± 0,111 pH‐Einheiten.
