Effects of halothane (1.5–4% in O2/N2) on aortic baroreceptors and aortic diameter close to the receptor area were studied in three cats and six rabbits. Receptor activity was quantitated by integration of activity in the whole aortic nerve, and aortic diameter was measured by ultrasonic technique inside the undisturbed thorax. Basic anesthesia was produced with chloralose and urethane or with pentobarbital. Following control observations at various blood pressures, aortic nerve activity and aortic diameter were followed during halothane administration for up to 33 min. Halothane produced bradycardia and hypotension, but had no consistent effects on the relationships between aortic nerve activity and arterial pressure, or aortic diameter and pressure.ZUSAMMENFASSUNGDie Wirkung von Halothan (1.5‐4 % in O2/N2) auf die aortalen Barorezeptoren und den Aortendurchmesser nahe dem Rezeptorgebiet wurde an 3 Katzen und 6 Kaninchen untersucht. Die Rezeptoraktivität wurde durch Integration der Aktivität im ganzen Aortennervensystem quantitativ erfasst und der Aortendurchmesser mittels Ultraschalltechnik im uneröffneten Thorax gemessen. Basisanaesthesie wurde mit Chloralhydrat und Urethan oder mit Pentobarbital erzielt. Nach Feststellung der Kontrollwerte bei verschieden hohen Blutdruckniveaus wurde die Aktivität des Aortennervensystems und der Aortendurchmesser bei Halothanverabreichung bis zu 33 Minuten gemessen. Halothan führte zu Bradykardie und Hypotonie, hatte aber keine sichere Wirkung auf das Verhältnis zwischen Aktivität des Aortennervensystems und dem arteriellen Druck, oder dem Aortendurchmesser

With a Xenon133radiospirometric technique the regional lung function during anesthesia and operation in the lateral decubitus position has been evaluated during spontaneous as well as controlled ventilation.A drastically changed distribution of ventilation during the period of controlled respiration giving a detrimental reduction of ventilation per unit lung volume, and of ventilation/perfusion ratio in the dependent lung, was the principal finding. This reduced ventilation of the dependent lung was postoperatively still partly remaining, even when the patient had regained an adequate spontaneous respiration.This is considered proof that controlled ventilation is of importance for the development of a state predisposed to atelectasis in the dependent lung, in the lateral decubitus position.ZUSAMMENFASSUNGEs wurde die radiospirometrische Technik mit Xenon133in Bezug auf die regionalen Lungenfunktionen während Spontanatmung und kontrollierter Beatmung geprüft.Eine drastische Änderung der Verteilung der Ventilation während kontrollierter Beatmung, welche eine nachteilige Verringerung der Ventilation pro Einheit Lungenvolumen bedingt, und des Ventilations‐Perfusionsverhältnisses in der abhängigen Lunge war das wesentlichste Resultat. Diese verringerte Ventilation der abhängigen Lunge blieb teilweise noch postoperativ bestehen, sogar dann, wenn der Patient schon wieder ausreichend spontan atmete.Diese Tatsache kann als Beweis dafür angesehen werden, daß die kontrollierte Beatmung das entscheidende Moment für die Entwicklung eines Bildes ist, das zur Atelektase in der abhängigen Lunge in seitlicher Position

The influence on gas distribution of different respirator settings was studied in a two compartment lung model. A volume‐limited respirator with an accelerating gas flow pattern was used. An increase in respiratory frequency resulted in a less uniform ventilation even after redistribution of gas had occurred. Increased minute volume caused a more uneven ventilation due to reduced possibilities of redistribution. The gas acceleration and the flow maximum were increased by raising the pressure in the driving system of the ventilator. Only slight and indecisive effects on gas distribution were observed in so doing. No correlation between maximum inspiratory flow rate and gas distribution was found. Impedance, which is frequency dependent, is considered to cause the variations in gas distribution, whereas the flow characteristics seem to have less influence on the uniformity of ventilation.ZUSAMMENFASSUNGDer Einfluß verschiedener Respiratoreinstellungen auf die Gasverteilung wurde an einem Zwei‐Compartment‐Lungenmodell untersucht. Ein volumsgesteuerter Respirator mit akzelerierendem Gas‐Flow wurde dazu benützt. Erhöhung der Atemfrequens erzielte eine ungleichmäßigere Verteilung, sogar nach Redistribution. Erhöhtes Minutenvolumen verursachte eine ungleichmäßigere Ventilation, entsprechend der verringerten Redistributionsmöglichkeit. Die Gasakzeleration und der maximale Flow wurden durch Erhöhung des Druckes im Antriebssystem des Respirators beschleunigt. Nur geringer und nicht eindeutiger Effekt auf die Gasverteilung konnte dadurch beobachtet werden. Zwischen maximaler inspiratorischer Flowgeschwindigkeit und Gasverteilung wurde keine Korrelation gefunden. Twelve patients operated on for gall bladder disease under methoxyflurane (Penthrane®) anaesthesia underwent renal function studies pre‐ and postoperatively.In seven of the patients the concentration of the anaesthetic agent was low, and in five a higher concentration was used.No effect on the glomerular filtration rate (endogenous creatinine clearance) and free water clearance was established. Fractional excretion of sodium decreased highly significantly in the low concentration group, but to a lesser extent in the higher concentration group. This might indicate that there was some effect on the sodium reabsorption in the tubules at methoxyflurane concentrations that were high but still within the normal range.Patients anaesthetized with halothane had a decrease of fractional excretion of sodium of the same order as those patients who received methoxyflurane in high dosage.ZUSAMMENFASSUNG12 Patienten, die wegen Gallenblasenerkrankung unter Methoxyfluran (Penthran®) operiert wurden, wurden prä‐ und postoperativ Nierenfunktionsprüfungen unterzogen.Bei 7 der Patienten war die Konzentration des Narkoseanges niedrig, bei 5 wurde eine höhere Konzentration verabreicht.Es zeigte sich keine Wirkung auf die glomuläre Filtrationsrate (endogene Kreatinin‐Clearance) und freie Wasser‐Clearance. Die anteilige Natrium‐Ausscheidung verminderte sich hochsignifikant in der Gruppe mit der niedrigen Konzentration, jedoch in geringerem Ausmaß in der Gruppe der höheren Konzentration. The material consisted of 27 pregnant full‐term or nearly full‐term patients on whom caesarean section was performed under general anaesthesia for obstetric reasons.Group A (11 patients) consisted of healthy women, and Group B (10 patients) of mothers who had a history suggesting uteroplacental insufficiency. Group C (6 patients) was a control material with no history of uteroplacental insufficiency.General anaesthesia was performed with thiopentone, suxamethonium and nitrous oxide‐oxygen and controlled ventilation. In Groups A and B thepatients started to breath an oxygen (90 %) +CO2 (10 %) mixture from 5 min before induction of anaesthesia up until the moment of delivery (15 min later). In Group C the patients did not inhale any CO2mixture.The maternal and foetal acid‐base parameters (pH and BD) were controlled in maternal arterial and foetal capillary blood (Saling1962) before starting CO2inhalation, and also at delivery.The maternal and foetal Paco2increased significantly in Groups A and B. Both mother and infant developed a secondary metabolic acidosis which led to a clear fall in foetal umbilical arterial pH in Group B. In Group A the umbilical arterial pH values stayed within the physiological range.The maternal‐foetal pH gradient that normally occurs at delivery diminished clearly during CO2inhalation in Group A and disappeared completely in Group B, whereas in control Group C the pH gradient was well established. The possible mechanism of elimination of this gradient and its effects on uteroplacental gas exchange are discussed.ZUSAMMENFASSUNGDas Untersuchungsmaterial bestand aus 27 Schwangeren, welche entweder am Termin oder knapp davor standen, bei denen aus geburtshilflichen Gründen eine Sectio in Allgemeinnarkose gemacht wurde.Gruppe A (11 Patientinnen) bestand aus gesunden Frauen und Gruppe B (10 Patientinnen) aus Müttern, deren Anamnese auf eine uteroplazentare Insuffizienz hindeutete. Gruppe C (6 Patientinnen) dienten als Vergleichsgruppe ohne Anhaltspunkte für uteroplazentare Insuffizienz.Allgemeinnarkose wurde mit Pentothal, Suxamethonium und Lachgas‐Sauerstoff‐anaesthesie durchgeführt und kontrollierte Beatmung wurde dazu benutzt. In den Gruppen A und B begannen die Patientinnen 90 % Sauerstoff und 10 % CO2zu atmen, und zwar 5 Minuten vor der Narkose‐einleitung bis zum Augenblick der Geburt (15 Minuten später). In Gruppe C bekamen die Patientinnen kein CO2‐hältiges Gemisch.Die miitterlichen und kindlichen Säure‐Basen‐Parameter (pH und BD) wurden im mütterlichen arteriellen und fotalen kapillären Blut (Saling1962) bestimmt und zwar vor Beginn der CO2‐Atmung und bei der Geburt.Das mütterliche und fötale Pac02nahm in den Gruppen A und B signifikant zu. Mutter und Kind entwickelten eine sekundäre metabolische Azidose, welche in Gruppe B zu einem klaren Abfall des fötalen Nabelarterien‐pH führte. In Gruppe A blieb das Nabelarterien‐pH innerhalb des physiologischen Bereiches.Der miitterlich‐fötale pH‐Gradient, der normalerweise bei der Geburt besteht, nahm in Gruppe A während der CO2‐Atmung deutlich ab und verschwand vollständig in Gruppe B, während in

The muscular fasciculations caused by suxamethonium (SCh) and the potential hazards connected with this can be prevented by pre‐injection of a small amount of d‐tubocurarine. This study was performed to determine the optimal time‐interval and dose‐ratio between these drugs which could prevent muscular fasciculations without compromising the conditions of intubation or without lengthening the neuromuscular block.A small dose (0.05 mg/kg) of d‐tubocurarine was given to 197 patients followed by suxamethonium (1 mg/kg). The drugs were given at varying intervals of 1, 2, 3 or 5 min. An additional 104 patients were injected with SCh, 1.5 or 2.0 mg/kg, after the above‐mentioned dose of d‐tubocurarine. Fifty‐two patients received only SCh, 1 mg/kg, and served as controls.The vigorous fasciculations decreased from the control value of 69 % to the minimum of 0 % when SCh, 1 mg/kg, was used 3 min after pre‐injection of d‐tubocurarine. With this mode of administration, reactions on intubation increased from 15 % to 48 %. Increasing the dose of SCh to 1.5 or 2.0 mg/kg caused vigorous fasciculations in 6 and 8 %, respectively, but gave excellent conditions for intubation in both groups. In the group given SCh, 2.0 mg/kg, the duration of neuromuscular block was significantly prolonged.ZUSAMMENFASSUNGEs wird ein neues Narkoseventil vorgestellt, das die Beseitigung überschüssigen Gases in der Umgebung des Personals erlaubt und welches gute Kontrolle der Ventilation ohne häufige Änderungen gestattet. Die Grundprinzipien sind vollkommen verschieden von denen, welche dem konventionellen Heidbrink‐Ventil zugrunde liegen.Die Beatmung kann durch die Parameter von Druck oder Volumen eingestellt werden, wobei letzteres welches das Atemzugvolumen des Patienten bestimmt, so wie erwünscht ist. Überschüssiges Gas verläßt das System am Ende der Exspirationsphase und der Druck im System kann 1,5 cm Wasser nicht übersteigen. Die Atmung kann kontrolliert werden oder spontan sein bei gleichbleibender Ventileinstellung.Wenn Druckbeatmung erforderlich ist wird das Ventil auf den erwünschten endinspiratorischen Druck eingestellt und das überschüssige Gas verläßt das System am Ende der Inspirationsphase. Cardiac output was determined and arterial and pulmonary artery pressures were recorded during a control period, after spinal anaesthesia (Th I—III) and after administration of 1 mg dihydroergotomine (DHE) i. v. in 10 healthy males.After spinal anaesthesia, arterial and pulmonary artery pressures decreased, whereas heart rate and cardiac output remained unchanged. After administration of DHE, arterial pressure increased to control level, while pulmonary artery pressure increased significantly above that level. Cardiac output and heart rate remained mainly unchanged.Since stroke volume did not increase after DHE administration, the restitution of arterial pressure must be due to an effect on resistance vessels, although it has been shown that DHE has more pronounced effect on capacity than resistance vessels.However, the increase in pulmonary artery pressure might indicate that in this study also, the effect of DHE was more pronounced on capacity vessels, but in a situation with normal filling pressures this does not affect the cardiac output to a significant degree.ZUSAMMENFASSUNGCardiac Output, Arterien‐und Pulmonalarterien‐druck wurden während einer Kontrollperiode nach Spinalanaesthesie (Th I‐III) und nach Verabrei‐chung von 1 mg Dihydroergotamin (DHE) i. v. bei 10 gesunden Männern registriert.Nach Spinalanaesthesie waren Arterien‐und Pulmonalarteriendruck vermindert, während Herz‐frequenz und Cardiac Output unverändert blieben. Nach Verabreichung von DHE stieg der Arterien‐druck auf das Kontrollniveau an, während der Pulmonalarteriendruck dieses Niveau signifikant überstieg. Cardiac Output und Herzfrequenz blieben im wesentlichen unverändert.Da das Schlagvolumen nach Verabreichung von DHE nicht ansteigt, muß die Wiederherstellung des Arterienausgangsdruckes durch die »Resistancegefäße« bedingt sein, obwohl gezeigt wurde, daß DHE mehr Wirkung auf die Kapazität als auf die »Resistancegefäße« hat.Jedenfalls könnte die Erhöhung des Pulmonalar‐teriendruckes bei dieser Unte suchung darauf hinweisen, daß die Wirkung von DHE auf die Kapazitätsgefäße zwar stärker war, aber daß dies bei normalem Fülldruck den

AB Two series of patients were anaesthetised with 0.5% halothane, 50% N2O in oxygen with a non‐rebreathing system. Series 1 consisted of seven patients anaesthetised twice with an interval of 2 weeks or more. On one occasion the patients received pancuronium 0.05 mg/kg for curarisation, and on the other, gallamine 1.0 mg/kg. Pulse rate and blood pressure were measured and recorded every 5 min. It was found that heart rate was unaffected by pancuronium, whereas there was a slight, but statistically insignificant, increase after gallamine. After both drugs there was a moderate but equal drop in blood pressure.Series 2 consisted of seven pairs of patients. The procedures were the same as in Series 1 except for the dosage of gallamine which was adjusted to provide, as nearly as possible, the same degree of muscle relaxation as that of pancuronium 0.05 mg/kg on the basis of measurements of hand grip strength after stimulation of the median nerve. From this it appears that pancuronium is 38 times more potent than gallamine under halothane anaesthesia.As far as heart rate and blood pressure are concerned, the changes were similar to those observed in Series 1, with the exception of a statistically significant increase in heart rate following the higher dosage of gallamine.ZUSAMMENFASSUNGZwei Gruppen von Patienten wurden mit 0,5 % Halothan, 50 % N2O in Sauerstoff mit einem Nichtrückatmungssystem narkotisiert. Gruppe 1 be‐stand aus 7 Patienten, welche zweimal in einem Abstand von 2 Wochen oder mehr narkotisiert wurden. In einem Fall erhielten die Patienten 0,05 mg/kg Pancuronium zur Relaxation und im anderen Fall Gallamin 1,0 mg/kg. Pulsfrequenz und Blutdruck wurden alle 5 Minuten geprüft. Es ergab sich, daß die Herzfrequenz durch das Pancuronium nicht verändert wurde, während aber ein leichter, statistisch nicht signifikanter, Anstieg nach Gallamin erfolgte. Nach beiden Mitteln kam es zu einem mäßigen, aber in beiden Gruppen gleichen Blutdruckabfall.Die zweite Gruppe bestand aus 7 Patientenpaaren. Die Vorgangsweise war dieselbe wie in Gruppe 1, mit der Ausnahme, daß die Dosis von Gallamin, welche so gewählt war, daß sie, wie genan als möglich, denselben Grad von Relaxation wie die von Pancuronium 0,05 mg/kg, überprüft durch die Messung der Hand‐ »grip strength« nach Stimulation des Medianus, ergab. Daraus geht hervor, daß Pancuronium unter Halothannarkose 38 mal stärker als Gallamin ist.Was Herzfrequenz und Blutdruck betriff, waren die Veränderungen ähnlich denen, die bei Gruppe 1 beobachtet wurden, ausgenommen die statistisch‐signifikante Erhöhung der Herzfrequenz nac

Demonstration of cardiopulmonary resuscitation was performed by 215 medical students on anaesthetized patients. Mouth‐to‐mouth or bag‐mask ventilation and orotracheal intubation were tested and any mistakes were noted. Only 71% of the students were able to open the airway adequately and diagnose the adequacy of ventilation. Only 54% performed mouth‐to‐mouth ventilation correctly, although in 94% of the cases the students succeeded in inflating the lungs. The most frequent mistakes were inadequate backward head‐tilt or oral seal. Bag‐mask ventilation was performed correctly in only 25% of the attempts. Insufficient compression of the mask and an obstructed airway, as a result of the inadequate backward tilt of the head, were the most common mistakes. The orotracheal intubation was performed correctly in 42% of the cases, the average intubation time being 35 s in the successful group. The theoretical knowledge of the students in acute or follow‐up treatment was graded in cardiac arrest, drowning and different types of intoxication. Only about one third were graded as good and 60–70% as satisfactory. The efficacy of the training programme is discussed.ZUSAMMENFASSUNG250 Medizinstudenten demonstrierten an narkotisierten Patienten kardiopulmonale Wiederbelebung. Mund‐zu‐Mund oder Beutel‐Maske‐Beatmung, sowie orotracheale Intubation wurden geübt und alle Fehler vermerkt. Nur 71 % der Studenten waren imstande, die Luftwege entsprechend zu öffnen und den Effekt der Beatmung zu beurteilen. Nur 54 % führten die Mund‐zu‐Beatmung rightig durch, obwhol 94 % der Studenten die Lungen aufzublasen imstande waren. Die häufigsten Fehler waren ungenügendes Rückwärtsbeugen des Kopfes und Mundverschluß. Beutel‐Maske‐Beatmung wurde nur in 25 % der Versuche korrekt durchgeführt. Ungenügendes Andrücken der Maske und verschlossene Luftwege, bedingt durch nicht ausreichendes Rückwärtsbeugen des Kopfes, waren hier die häfigsten Fehler. in 42 % der Fälle wurde die orotracheale Intubation exakt durchgeführt, die durchschnittliche Zeitdauer hifür betrug 35 Sekunden in der erfolgreichen Gruppe. Das theoretische Wissen der Studenten in akutbehandlung, sowie nachfolgender Versorgung wurde in den Fächern Herzstillstand, Ertrinken und verschiedene Arten von Vergiftungen überprüft. A new anaesthesia valve is presented which allows removal of excess gas from the vicinity of personnel, and gives fine control of ventilation without requiring frequent adjustment. The basic principles are very different from those applied in the conventional Heidbrink valve.Ventilation may be controlled by the parameters of either pressure or volume, the latter ensuring absolutely that the patient's tidal volume is as desired. Excess gas leaves the system at the end of the expiratory phase, and pressure within the system cannot exceed 1.5 cm H2O. Respiration can be controlled or can be spontaneous at the same valve setting.When pressure ventilation is required the valve is set to the required end‐inspiratory pressure, and excess gas then leaves the system at the end of the inspiratory phase. Adjustment of the valve is only necessary when there is a change in the patient's compliance.There is a built‐in safety system which prevents closure of the valve during spontaneous respiration.Specially designed exhaust equipment can be attached to the exhaust port of the valve in order to remove the excess gas by a vacuum system.ZUSAMMENFASSUNGEs wird ein neues Narkoseventil vorgestellt, das die Beseitigung überschüssigen Gases in der Umgebung des Personals erlaubt und welches gute Kontrolle der Ventilation ohne häufige Änderungen gestattet.
