SUMMARYA continuous uniform post‐tracheotomy care adopted in a hospital with more than 200 tracheotomies annually is described. No noteworthy complications were seen in 25 consecutive cases of children under 10 years of age. The results are compared with earlier statistics, and prophylactic steps are discussed.Heat and moisture exchangers for connection with the tracheal cannula have been constructed and tested both experimentally and clinically. They are able to treble the absolute humidity of inspired room air, and good clinical results were obtained, even in cases where ordinary steam therapy proved ineffective. The dead spaces and air‐flow resistances of the apparatuses are made equal to that of the upper respiratory tract, which seems to be an additional improvement, facilitating normal respiration after decannulation.ZUSAMMENFASSUNGEine einheitliche Art der Behandlung und Pflege tracheotomierter Patienten die an einem Krankenhause mit über 200 Tracheotomien jährlich angewandt wird, wird beschrieben. Bei 25 aufeinanderfolgenden Tracheotomien, die an Kindern unter 10 Jahren ausgeführt wurden, traten keine nennenswerten Komplikationen auf. Die Resultate werden mit früheren Statistiken verglichen und prophylaktische Massnahmen werden diskutiert.Zur Befeuchtung und Erwärmung der Einatmungsluft wurden an die Trachealkanülen gekoppelte Feuchtigkeits‐ und Wärmespeichereinrichtungen konstruiert und sowohl klinisch als auch experimentell erprobt. Es ergab sich, dass diese kleinen Feuchtigkeits‐ und Wärmespeicher die absolute Feuchtigkeit der Zimmerluft verdreifachen konnten, und es wurden auch bei Fällen, in denen sich gewöhnliche Dampftherapie als unzureichend erwies, gute klinische Resultate erzielt. Der Totraum und der Luftwiderstand wurden den Verhältnissen in den oberen Luftwegen angeglichen, hierdurch lässt sich offenbar eine Erleichterung im Übergang zur Normalatmung nach Entfernung

SUMMARYThe toxicological and pharmacological properties of Carbocaine, lidocaine and procaine were tested in simultaneous experiments on animals and man. Tests methods recommended by other investigators were used; the experiments were performed by the “blind” or “double‐blind” method, and the results were subjected to statistical analysis. The results obtained are compared with those published by other investigators.It appeared that procaine has the lowest toxicity of the three local anaesthetics studied. The conclusion of the toxicity tests performed was that Carbocaine is somewhat less toxic than lidocaine, in particular, when low—i. e. sublethal, therapeutic—doses are administered.In surface anaesthesia of the rabbit cornea, in infiltration anaesthesia in guinea pigs and man, and in conduction anaesthesias of various types, Carbocaine proved to be the most effective drug, followed by lidocaine and procaine in that order. In tests with adrenaline‐containing solutions of the three local anaesthetics, the difference between the anaesthetic effects of Carbocaine and lidocaine was not always significant.The clinical value of a local anaesthetic is determined by its toxicological and anaesthetic properties. As lidocaine has proved to be an excellent local anaesthetic although it is more toxic than procaine, the results obtained suggest that Carbocaine may in time be recognised as a still better drug.Whether it is permissible from the results obtained to conclude that Carbocaine represents a new conquest in the field of local anaesthesia can obviously be decided only on the basis of the experience gained when it has been used under variable clinical conditions for several years.ZUSAMMENFASSUNGDie toxikologischen und pharmakologischen Eigenschaften von Carbocain, Lidocain und Procain wurden durch Experimente an Menschen und Tieren erforscht. Von anderen Untersuchern empfohlene Testmethoden wurden angewandt und mit Hilfe der Blind‐ oder Doppelblindmethode durchgeführt. Die Ergebnisse wurden statistischer Analyse unterworfen und mit denen anderer Untersucher verglichen.Es zeigte sich, dass Procain von den untersuchten Lokalanästhetica die niedrigste Toxicität aufwies. Im Endergebnis der ausgeführten Toxicitätsuntersuchungen zeigte es sich, dass Carbocain etwas weniger toxisch als Lidocain war, besonders, wenn niedrige, also sublethale oder therapeutische Dosen zur Anwendung kamen.Bei Oberflächenanästhesien der Cornea von Kaninchen, Infiltrationsanästhesien an Meerschweinchen und Menschen und bei Leitungsanästhesien verschiedener Typen, erwies Carbocain sich als das Präparat mit der grössten Wirkung, gefolgt von Lidocain und Procain in der angeführten Reihenfolge. Bei Untersuchungen von adrenalinhaltigen Lösungen der drei Anästhetica, war der Unterschied in der Wirkung des Carbocains und Lidocains nicht immer significant.Der klinische Wert eines Lokalanästheticums wird durch seine toxischen und anasthetischen Eigenschaften bestimmt. Zwar hat sich Lidocain als ein ausgezeichnetes Lokalanästheticum erwiesen, obwohl es mehr toxisch als Prokain ist, die hier gefundenen Ergebnisse zeigen jedoch, dass Carbocain zur Zeit als ein noch besseres Präparat angesehen werden muss.Ob es zulässig ist an Hand der vorliegenden Ergebnisse zu schliessen, dass Carbocain einen Neulandgewinn auf dem Gebiete der Lokalanästhesie darstellt, kann offenbar nur entschieden werden, nachdem man es unter variierenden klinischen Beding
