SUMMARYFive young healthy subjects whose alveolar CO2tensions were maintained constant at 45 mm Hg by adjustment of CO2admixture to the inhaled air were given pentazocine or pethidine in three doses of 15 mg/70 kg b.w. and 50 mg/70 kg b.w., respectively, at 20‐minute intervals. SUMMARYChanges in the renal blood flow, with special reference to the regional blood flow, were studied in normovolaemic and hypovolaemic dogs. Heated thermocouples, as described by GIBBS, were used. The effects of adrenaline, noradrenaline, methoxamine, metaraminol, normetaoxedrin, Arg8‐vasopressin and angiotensin II were studied. Noradrenaline in continuous intravenous injections of less than 0.6 μg/kg.min. increased the blood flow in the renal medulla and the urinary output increased, but the cortical blood flow decreased. Other pressor substances produced decreased blood flow in the renal cortex and medulla. Injection of papaverine resulted in decreased arterial blood pressure through general vasodilatation and the renal blood flow decreased. Small doses of bradykinin in the renal artery increased the blood flow in all regions of the kidney, without a decrease in blood pressure. The effects of apnoeic diffusion oxygenation and ventilation with carbon dioxide in oxygen were studied and compared. On bleeding the dogs, there was a marked decrease in the blood flow to all regions of the kidney and the urine production stopped. Retransfusion or infusion of plasma expander increased the blood flow in all regions of the kidney and the anuria ceased.ZUSAMMENFASSUNGBei normo‐ und hypovolämischen Hunden wurden die Veränderungen der Nierendurchblutung ‐ insbesondere der regionalen Durchblutung ‐ untersucht. Dabei kamen von Gibbs beschriebene Wärmeleitsonden zur Anwendung. Es wurden die Wirkungen von Adrenalin, Noradrenalin, Methoxamin, Metaraminol, Normetaoxedrin, Arg8‐Vasopressin und Angiotensin II studiert. Bei kontinuierlicher i.v. Zufuhr von Noradrenalin in Dosen bis zu 0.6 μg/kg.min. nahmen die Durchblutung des Nierenmarkes und die Harnproduktion zu, während die Durchblutung der Nierenrinde abnahm. Die anderen Pressorsubstanzen verursachten eine Durchblutungsverminderung sowohl der Nierenrinde als auch des Markes. Die Injektion von Papaverin führte zu arterieller Blutdrucksenkung zufolge allgemeiner Vasodilatation. Dementsprechend war auch die Nierendurchblutung vermindert. Die Injektion kleiner Dosen von Bradykinin in die Nierenarterie erhöhte die Durchblutung aller Nierenregionen ohne gleichzeitige Blutdrucksenkung. Des weiteren wurden die Wirkungen der apnoischen Sauerstoff‐Diffusionsatmung im Vergleich zur Beatmung mit Kohlensäure in Sauerstoff untersucht. Bei der Ausblutung der Hunde kam es zu ausgeprägter Durchblutungsverminderung in alien Nierenregionen und die Harnproduktion hörte auf. SUMMARYContinuous peridural analgesia using bupivacaine 0.5% with epinephrine 1:200,000 in 22 unselected patients undergoing surgery of the hip is described. That the dose of bupivacaine is related to the age of the patient is confirmed by the results of this study. The latency of onset of analgesia was 6.1 minutes, the mean duration of the initial injection was 475 minutes, while that for subsequent injections was 255 minutes. Optimal conditions for the surgery described were obtained using this method, while excellent postoperative analgesia was provided. A minor degree of tachyphylaxis was apparent during the postoperative period, although this was not substantiated by statistical analysis. Continuous peridural analgesia is recommended as the anaesthetic of choice for operations on the hip.ZUSAMMENFASSUNGEs wird die kontinuierliche Periduralanaesthesie mit 0,5 % Bupivacain und Epinephrin im Verhältnis 1:200.000 an 22 nicht ausgewählten Patienten, die einer Hüftoperation unterzogen wurden, beschrieben. Dass die Dosis Bupivacain mit den Alter des Patienten in Beziehung steht, wird von den Ergebnissen dieser Untersuchung bestätigt. Die Zeit bis zum Einsetzen der Analgesie betrug 6,5 Minuten, die durchschnittliche Dauer der Anfangsinjektion lag bei 475 Minuten, während jede nachfolgende Injektion 255 Minuten anhielt. Mit dieser Methode wurden optimale Bedingungen für die beschriebene Operation geschaffen und auch eine ausgezeichnete postoperative Analgesie erzielt. Geringe Anzeichen von Tachyphylaxie traten postoperativ auf, obwohl dies durch statistische Analyse nicht erhärtet werden konnte. SUMMARYA new technique for blind nasotracheal intubation by means of a guiding catheter blindly inserted into the trachea has been devised for cases in which intubation of the conscious patient under local analgesia is indicated when standard procedures for blind intubation have failed.The techniques used for local analgesia of the upper airway and the various devices and techniques employed in cases in which intubation is difficult are reviewed. Cases which should be considered for “awake” intubation are discussed.The risk of aspiration of gastric contents into the lungs under local analgesia of the upper airway is emphasized.Finally, it is described how the new intubation technique—with a slight modification—may be used for blind orotracheal intubation and for the change of nasotracheal and orotracheal tubes.The applicability of the method is illustrated in 20 case histories.ZUSAMMENFASSUNGEine neue Methode der blinden nasotrachealen Intubation mit Hilfe eines blind in die Trachea eingeführten Führungskatheters wurde für jene Fälle erdacht, bei denen einerseits die Intubation des Patienten in Lokalanaesthesie angezeigt ist, andererseits denen die üblichen Vorgänge zur blinden Intubation versagt haben.Es wird ein Überblick gegeben über die angewendeten Methoden für die Lokalanaesthesie der oberen Luftwege und für die verschiedenen Hilfsmittel und Techniken, die bei schwierigen Intubationen angewendet werden. SUMMARYA method for the determination of oxygen consumption during respirator ventilation with the Engström respirator in the newborn and young infants is described. The volumetric principle is based on the assessment and recording of a volume deficit in a closed system supplied with a carbon dioxide absorber. The method has been tested in animal experiments and has proved useful in measuring uptake values of the order of 0.1 ml/min.ZUSAMMENFASSUNGEs wird eine Methode zur Bestimmung des Sauerstoffverbrauches bei der Beatmung von Neugeborenen und Kleinkindern mit dem Engström‐Respirator beschrieben. SUMMARYBecause of a fairly wide range of duration of intercostal nerve blocks postoperatively in patients an objective method was used to study this problem. Infrared thermography was applied to study intercostal nerve blocks produced by bupivacaine 0.5% with and without adrenaline. Consistent results were obtained in volunteers when thermography and pin‐prick methods were compared. Added adrenaline does not seem to prolong the intercostal nerve block.When patients were studied only bupivacaine 0.5% with adrenaline was used. The range of duration was wide (7 to 20 hours), and the variation between pin‐prick and thermography results less consistent.ZUSAMMENFASSUNGWegen der ziemlich weitgestreuten Wirkungsdauer postoperativer Intercostalnervenblockaden wurde eine objektive Mediode zum Studium dieses Problems angewandt. Wir bedienten uns der Infrarot‐Thermographie zur Untersuchung von Intercostalnervenblockaden durch Bupivacain 0,5%, mit und ohne Adrenalin. Beim Vergleich der Thermographic mit der Nadelstich‐Testmediode an freiwilligen Versuchspersonen ergab sich gute Übereinstimmung. Adrenalinzusatz scheint die Nervbloskdauer nicht zu verlängern.Bei den Untersuchungen an Patienten wurde nur 0,5%iges Bupivacain mit Adrenalin verwendet. Die Wirkungsdauer war weit gestreut (zwischen 7 und 20 Stunden), die Übereinstimmung zwischen Thermographic und Nadel‐stichtest war weniger
