SUMMARYThe molecular level chemistry of the acetylcholine‐acetylcholinesterase system has been examined in some detail. The mechanisms of normal substrate hydrolysis and anticholinesterase activity were stressed.It is quite possible that similar forces and mechanisms are inherent in other alterations in physiology for which the anesthesiologist is responsible. The acetylcholine‐acetylcholinesterase system is but one example of this relationship between small and large molecules.The paucity of knowledge concerning the molecular level aspects of what we do can only lead to the conclusion that much progress must be made in these areas if we are to understand the extraordinary complex changes induced in the many biologic systems of the body by anesthesia and anesthetics.ZUSAMMENFASSUNGDie Chemie des Azetylcholin‐Cholinesterasesystems wurde auf Molekular‐niveau im Detail untersucht. Dabei wurden die Mechanismen der normalen Substrathydrolyse und der Anticholinesteraseaktivität herausgestell.Es ist absolut möglich, dass ähnliche Kräfte und Mechanismen auch anderen physiologischen Veränderungen innewohnen, für die der Anaesthesist verant‐wortlich ist. Das Azetylcholin‐Cholinesterasesystem ist aber ein Beispiel dieser Beziehung zwischen kleinen und grossen Molekülen.Die geringen Erfahrungen betreffend die Aspekte auf molkekularem Niveau von den Dingen, die wir machen, kann nur zu dem Schluss führen, dass auf diesen Gebieten noch viel Fortschritte gemacht werden müssten, um die ausserordentlich komplexen Veränderungen zu verstehen, die in den vielen biologischen Systemen des Körpers durch die Narkose und Anaesthetik

SUMMARYLAC‐43 is a new local anaesthetic which differs from mepivacaine in that the methyl group has been replaced by a butyl group.The duration of a block of the little finger with LAC‐43 was studied in a group of 20 subjects, all of the same sex (female) and the same age group. The trial was conducted at random by a double‐blind method. The durations of the anaesthesias were assessed both by the pin‐prick method and by means of the so‐called ninhydrin method.With both methods, it was found that LAC‐43 0.25% with a vasoconstrictor has twice as long a statistically reliable duration as Carbocaine with adrenaline, with toxically equivalent doses, and that the test preparation has four times as long a duration as Carbocaine in the same concentration but without adrenaline.LAC‐43 0.25% without vasoconstrictor has a low frequency of complete blocks, viz. 50%.It was not possible to determine any statistically significant difference in the results between the pin‐prick method and the ninhydrin method, nor was any reliable difference found in the duration between the left and the right little finger.ZUSAMMENFASSUNGBei LAC‐43 handelt es sich urn ein neues Lokalanästheticum, welches sich dadurch von Mepivacain (Carbocain/Scandicain) unterscheidet, dass es an Stelle der Methyl‐eine Butylgruppe trägt.Die Wirkungsdauer eines Nervblocks mit LAC‐43 am kleinen Finger wurde an 20 gleichgeschlechtlichen (weiblichen) Versuchspersonen etwa gleichen Alters in einem Doppelblindversuch mit zufälliger Zuordnung geprüfl. Die Anaesthesiedauer wurde sowohl mit dem Nadelstichverfahren wie mittels der sogenannten Ninhydrin‐Methode getestet.An Hand beider Prüfverfahren liess sich zeigen, dass 0,25% LAC‐43 unter Zusatz eines Vasokonstriktors doppelt so lange wirkt, wie Carbocain mit Adrenalin in einer Konzentration gleicher Toxicität; ein Vergleich mit Carbocain ohne Adrenalin ergab eine 4 fach längere Wirkung. Die Unterschiede sind statistisch hoch gesichert. SUMMARYDuring 1954–1963, three‐types of respirators were used at this unit, viz. the Engström, Gothia and Lundia respirators. These were used in 54 postoperative and 15 medical cases. An attempt was made to relate the rates of ventilatory deaths to pre‐treatment ventilatory capacity and to the type of respirator. For the surgical cases with obstructive‐type ventilatory impairment, the ventilatory death rate was lower with the Engström respirator (0/8) than with the other respirators (4/6).—As regards effect on blood gases, the respirators differed only in the obstructive cases; here, too, the Engström respirator was superior.Respiratory depressant drugs and/or muscle relaxants were given in larger amounts to patients treated with the Gothia and Lundia respirators than to those treated with the Engström respirator. This difference was very great in the obstructive group of patients. In patients without airway obstruction, the three respirators were equivalent with regard to effect on ventilatory death rate and blood gases, and nearly equivalent in the ease of management and requirement of respiratory depressant drugs. In the obstructive patients, the Engström respirator was superior in all these respects.ZUSAMMENFASSUNGWährend der Jahre 1954 bis 1963 wurden an der Abteilung drei verschiedene Typen von Respiratoren angewandt, nämlich der Engström, der Lundia und der Gothiarespirator. Die Apparate wurden bei 54 postoperativen und 15 medizinischen Fällen eingesetzt. Es wurde der Versuch gemacht, die Anzahl der Ventilationstodesfälle in Relation zur Vorbehandlung, zur Atemkapazität und zur Respiratortype zu setzen. Die Anzahl der Ventilationstodesfälle bei chirurgischen Patienten mit Ventilationshindernissen obstruktiver Art war bei Anwendung des Engströmrespirators niedriger (0/8) als bei anderen Respiratoren (4/6). Was die Wirkung auf die Blutgaswerte betrifft, unterschieden sich die Respiratoren nur in den Fällen mit Obstruktionen. Auch hier war der Engström respirator überlegen.Sowohl respirationsdepremierende Medicamente als auch Muskelrelaxantia wurden in grösseren Mengen an diejenigen Patienten verabreicht, die mit oder Gothiarespiratoren als an solche, die mit dem Engströmrespirator behandelt wurden.Der Unterschied war bei der obstruktiven Gruppe sehr erheblich. Bei Patienten ohne Luftwegsobstruktionen waren die Respiratoren gleichwertig in Bezug auf die Anzahl der Ventilationstodesfälle und die Blutgaswerte und nahezu gleichwertig in Bezug auf die Einfachheit der Bedienung und die Notwendigkeit der Verabreichung respirationsdeprimierender Stoffe. SUMMARYReserpine pre‐treatment increased the LD50of procaine, tetracaine and mepivacaine especially when the local anaesthetics were given intravenously. This effect was only partially due to hypothermia produced by reserpine. Monoamine oxidase inhibitor pheniprazine decreased the LD50of procaine, particularly when given subcutaneously, but it did not clearly influence the toxicity of mepivacaine.ZUSAMMENFASSUNGReserpinvorbehandlung erhöht die LD50von Procain, Tetracain und Mepivacain, besonders wenn die Lokalanaesthetika intravenös verabreicht wurden. SUMMARYThe influence of reserpine on the sleeping (recovery) time after halothane anaesthesia was studied in mice. Reserpine was given intraperitoneally, usually in a dose of 5 mg/kg, and 1.5 % halothane mixed with air was administered for 10 minutes.A single dose of reserpine increased the sleeping time from a control value of about 69 sec. to about 964 sec. in 6 hours. Some of the reserpine effect was still present 96 hours later. The increase of sleeping time was parallel to the depletion of brain noradrenaline and 5‐hydroxytryptamine.When given repeatedly once a day, 0.1 mg/kg of reserpine had no effect on the brain amines and the halothane anaesthesia. A dose of 1.0 mg/kg produced a pronounced effect after one or two injections.Atropine, pheniprazine, pyrocatechol and increased ambient temperature shortened the sleeping time in reserpine‐treated mice. These treatments also had an inhibitory effect on the reserpine‐induced hypothermia. Prolonged respiration of halothane caused more deaths in the reserpine‐treated animals than in the controls. Oxygen exerted a slight protective action in both groups.ZUSAMMENFASSUNGDer Einfluss von Reserpin auf die Schlaf‐ (Erholungs‐)zeit nach Halo‐thannarkose wurde bei Mäusen geprüft. Reserpin wurde intraperitoneal, meist in einer Dosis von 5 mg/kg verabreicht, Halothan als 1,5%iges Gemisch mit Luft durch 10 min gegeben.Eine Einzeldosis von Reserpin erhöhte die Schlafdauer nach 6 Stunden auf etwa 964 sec gegenüber einem Kontrollwert von ca. 69 sec. Eine gewisse Reserpinwirkung blieb durch 96 Stunden nachweisbar. Die Zunahme der Schlafdauer ging der Verarmung des Gehirns an Noradrenalin und 5‐Hydroxy‐tryptamin parallel.Mehrfach verabreichte Tagesdosen von 0,1 mg/kg Reserpin blieben ohne Einfluss auf den Amingehalt des Gehirns und den Ablauf der Halothan‐anaesthesie. Eine Dosis von 1,0 mg/kg bedingte nach 1 oder 2 Injektionen einen deutlichen Effekt.Atropin, Pheniprazin, Pyrocatechol und erhöhte Aussentemperaturen ver‐kürzten die Schlafzeit reserpinvorbehandelter Mäuse; es zeigte sich auch eine Hemmwirkung auf die reserpinbedingte Hypothermic.Längere Halothaninhalation verursachte bei reserpinbehandelten Tieren eine hohere Sterblichkeit als bei den Kontrollen. SUMMARYThe local tissue‐oxygen tension in the skin was studied, when adrenaline, noradrenaline and Octapressin® Sandoz were used as vasoconstrictors in local anaesthetic agents injected subcutaneously on the forearms of healthy human subjects.During the first hour after injection changes in colour occurred in the adrenaline and noradrenaline zones: the initially pale zones became cyanotic. The Octapressin zones remained pale throughout the entire experimental period.The tissue‐oxygen tension, as measured with a Clark polyethylene membrane electrode attached to a Beckman Physiological Gas Analyzer, was clearly lower after the administration of the catecholamines than after Octapressin.The colour change and the decrease in tissue‐oxygen tension are attributed to the fact that the locally administered adrenaline or noradrenaline gives rise to a local increase in oxygen consumption.Since Octapressin, when used as a vasoconstrictor in local anaesthesia, does not cause any local cyanosis, and the local tissue‐oxygen tension is higher, it may be more harmless to the tissue than the other two agents.ZUSAMMENFASSUNGDie Gewebssauerstoffspannung der Haut am Unterarm gesunder Versuchs‐personen wurde nach subcutaner Injektion einer Lokalanaesthetikums unter Zusatz von Adrenalin, Noradrenalin bzw. Octapressin (Sandoz) als Vasokon‐striktor geprüft.Innerhalb der ersten Stunde nach Injektion treten in den Adrenalin‐ und Noradrenalinzonen Farbveränderungen auf: die anfänglich blasse Zone wird zyanotisch. Nicht so bei Octapressin, wo die blasse Farbe bestehen bleibt.Die Gewebssauerstoffspannung (gemessen mit Clark‐Membranelektrode und Beckman Physiological Gas Analyzer) lag in den Catecholaminzonen tiefer als im Octapressinbereich.Die Farbänderung und der Abfall der Sauerstoffspannung im Gewebe wird auf eine lokale Erhöhung des Sauerstoffverbrauchs unter der Einwirkung von Adrenalin bzw. Noradrenalin zurückgeführt. ‐ Da Octapressin, als Vaso‐konstriktor dem Lokalanaesthetikum zugesetzt, keine örtliche Zyanose verur‐sacht und die Sauerstoffspannung im Gewebe nicht so stark absinken lässt, dürfte es für das Gewebe har

SUMMARYThe temperature of stored blood is 4°C. Massive blood replacement with cold stored blood therefore results in a considerable reduction in the body temperature of the patient. First of all, the heart will be cooled. In the opinion of many investigators, this cooling is the cause of cardiac arrhythmias or, at worst, of cardiac arrest. The element of danger may be eliminated by warming the transfusion blood.We have used heated blood for transfusions in the following cases: (1) In all major operations on children, (2) in all major operations on adults in which massive blood replacement may be expected, and (3) in operations on patients under induced hypothermia.An effective blood heater, which can heat blood during the transfer from the transfusion bottle to the patient, is described. By means of this device, large quantities of blood may be administered with minimal influence on the patient's body temperature.ZUSAMMENFASSUNGKonservenblut hat eine Temperatur von 4°C. Massiver Blutersatz mit Konservenblut führt daher zu beträchtlicher Senkung der Körpertemperatur des Patienten, vor allem das Herz wird stark gekühlt. Nach Ansicht vieler Untersucher verursacht diese Abkühlung Herzarrhythmien oder gar Herz‐stillstand. Dieses Gefahrenmoment kann durch Erwärmung des transfundierten Blutes ausgeschaltet werden.Wir verwendeten erwärmtes Blut für Transfusionen in folgenden Fällen:1Grössere Eingriffe bei Kindern.1Grosse Operationen bei Erwachsenen, wenn massiver Blutersatz zu erwarten ist.1Operationen in Unterkühlung.Ein wirkungsvolles Gerät zur Erwärmung von Blut während der Transfusion wird beschrieben.
