SUMMARYArterial oxygen tensions were determined in 16 patients with known cardiovascular and respiratory disease during the administration of 50‐65% nitrous oxide in oxygen supplemented by halothane. The patients were first divided into two main groups according to the site of operation. Each group was further divided into two equal groups according to the ratio of nitrous oxide‐oxygen administered. Ventilation was provided by a volume‐controlled respirator through a non‐rebreathing circuit. Known inspired mixtures of nitrous oxide‐oxygen‐halothane were administered over periods of a few hours' duration.Arterial blood PaO2, pH and PaCO2and standard bicarbonate were determined at the end of each observation period. Significant decrease in arterial oxygen tension occurred in most of the patients studied, more markedly in those who received 65% nitrous oxide. To a great extent, the decrease in arterial oxygen tensions to dangerously low levels was related clinically to a corresponding diminution in the efficiency of the circulatory system. It was demonstrated that in the patient with cardiovascular and respiratory disease a “safe” level of arterial oxygen tension cannot be assured at all times during surgery with these commonly employed mixtures of nitrous oxide and oxygen. It is therefore suggested that in this type of patient the administration of more than 50% nitrous oxide should be avoided unless stability of the cardiovascular system can be assured at all times during anaesthesia, or facilities for frequent monitoring of PaO2are readily available. In the presence of a poor or deteriorating circulatory status, the administration of even 50% N2O should be discontinued and the inspired anaesthetic mixture should contain more than 50% oxygen for the maintenance of adequate level of arterial oxygen tension.ZUSAMMENFASSUNGArterielle SauerstofTdruckmessungen wurden bei 16 Patienten mit bekannten kardiovaskulären und respiratorischen Erkrankungen während der Verabreichung von 50–65% N2O in Sauerstoff verstärkt durch Halothan durchgeführt. Entsprechend der Operationstype wurden die Patienten in 2 Gruppen aufgeteilt. Jede Gruppe wurde ferner in 2 gleiche Gruppen mit jeweils verschie‐denem Verhältnis zwischen Lachgas und Sauerstoff unterteilt. Die Atmung wurde durch einn volumengesteurten Respirator über ein Nichtriickatem‐system aufrecht erhalten. Bekannte Inspirationsmischungen von Lachgas‐Sauerstoff‐Halothan wurden über Perioden von mehreren Stunden verabreicht.PaO2, pH und PaCO2und Standardbicarbonat aus dem arteriellen Blut wurden am Ende jeder Beobachtungsperiode bestimmt. Bei fast alien untersuchten Patienten sank der arterielle Sauerstoffdruck signifikant ab, insbesondere bei jenen, die 65% Lachgas erhalten hatten. Zum grossen Teil war das Absinken der arteriellen Sauerstoffspannung auf gefährlich niedrige Werte klinisch mit einer korrespondierenden Zirkulationsschwäche vergesellschaftet. Es konnte gezeigt werden, dass bei Patienten mit kardiovaskulären und respiratorischen Erkrankungen ein sicherer Spiegel der arteriellen Sauerstoffspannung nicht jederzeit während eines chirurgischen Eingriffes unter Verwendung diese Lachgas‐Sauerstoff‐Mischungen aufrecht erhalten werden kann. Es wird daher vorgeschlagen, dass bei dieser Art von Patienten nicht mehr als 50% Lachgas verabreicht wird, es sei denn, man kann das kardiovaskuläre System während der ganzen Narkosedauer stabil halten oder man hat die Möglichkeit häufige PaO2‐Kontrollen jederzeit durchzuführen. Bei schlectem oder darniederliegendem Kreislauf sollte sogar die Verabreichung von 50% N2O gestoppt werden und die eingeatmete Narkosegasmischung sollte mehr als 50% Sauerstoff enthalten, damit ein adequater Spiegel der arterie

SUMMARY1A thorough study of a surgical case material from one year revealed that in about 6% of the laparotomies the major postoperative problem seemed to be related to a poor nutritional state of the patient.2Inadequate nutrition is fairly common in the pre‐operative period. In many cases, the poor nutritional state will not be obvious until after surgery.3The poor nutritional state could not be compensated for even with intensive parenteral feeding in the postoperative period.4In the postoperative period adequate nutrition is essential until a reasonably good oral intake has been established. If this rule is not followed, the patient will have a severe handicap if surgical complications should occur.5It is essential in all matters concerning nutrition that an account of the actual daily intake (calories, carbohydrates, fat and amino acids) is made both during the pre‐ and the postoperative periods. Enriched appetizing meals in the pre‐operative period, as well as carbohydrates, amino acids and often fat parenterally in high quantities in the postoperative period, are essential means to maintain an acceptable nutritive state in the surgical patients.ZUSAMMENFASSUNG UND SCHLUSSFOLGERUNG1Eine sorgfältige Sichtung des chirurgischen Krankengutes eines Jahres brachte zum Vorschein, dass in ungefähr 6% der Laparotomien das Hauptproblem in der postoperativen Phase im schlechten Ernährungszustand der Patienten gelegen war.2In der präoperativen Periode ist inadequate Ernährung relativ haufig. In vielen Fällen wird der schlechte Ernährungszustand jedoch erst nach dem chirurgischen Eingriff augenfällig.3Der schlechte Ernährungszustand konnte auch mit intensiver parenteraler Fütterung in der postoperativen Periode nicht ausgeglichen werden.4In der postoperativen Periode ist adequate Ernährung bedeutungsvoll, solange bisausreichend gute orale Nahrungsaufnahme möglich ist. Wenn man diese Regel nicht befolgt, ist der Patient beim Auftreten chirurgischer Komplikationen mit einem schweren Handikap belastet.5In allem mit der Ernährung zusammenhängenden Dingen ist es von besonderer Bedeutung, dass eine genaue tägliche Bilanz geführt wird und zwar über Kalorien, Kohlehydrate, Fett‐ und Aminosäuren, sowohl während der prä‐ als auch postoperativen Periode. Schmackhafte, kalorien‐reiche Mahlzeiten in der präoperativen Phase, ebenso wie in grösseren Mengen parenteral zugeführte Kohlehydrate, Aminosäure und oft auch Fett in der postoperativen Periode sind von grösster Bedeutung für die Erhaltung eines annehmbaren Ernäh

SUMMARY1The effect of intravenous fentanyl on the blood flow and EEG in a circumscribed part of the cerebral cortext was investigated in 13 artificially ventilated cats.2The drug caused a reduction in cortical blood flow and mean EEG frequency in cerveau isolé preparations, and an insignificant reduction in cats in phenobarbitone anaesthesia. Subsequent intravenous nalorphine increased the cortical blood flow and mean EEG frequency to above control values.3Cortical blood flow increased with increasing total dosage of fentanyl (0.005‐0.02 mg/kg) in intact cats without central anaesthesia. A small increase in the mean EEG frequency also occurred. Electrographic seizures followed doses of 0.02‐0.08 mg/kg.4The results suggest that fentanyl has an excitatory action which is mediated through the reticular system of the brain stem. When this system is eliminated or depressed, a depressant action of the drug is seen.ZUSAMMENFASSUNG1Bei 13 kiinstlich beatmeten Katzen wurde die Wirkung von intravenös verabreichtem Fentanyl auf die Durchblutung und das EEG in einem urn‐schriebenen Teil der Gehirnrinde untersucht.2Das Mittel verursachte eine Verminderung der kortikalen Durchblutung und der mittleren EEG‐Frequenz in isolierten Hirnpräparationen, sowie eine nicht signifikante Verminderung bei Katzen in Phenobarbitalanaes‐thesie. Die nachfolgende intravenöse Verabreichung von Nalorphin erhöhte die kortikale Durchblutung und die mittlere EEG‐Frequenz über die Kon‐trollwerte hinaus.3Die kortikale Durchblutung erhöhte sich mit steigenden Gesamtdosen von Fentanyl (0,005‐0,02 mg/kg) bei intakten Katzen ohne zentrale Anaesthesie. Es kam auch zu einem geringen Anstieg der mittleren EEG‐Frequenz. SUMMARYSerum GOT and GPT levels in 145 patients were followed postoperatively for three days. The series was divided into three groups, one consisting of patients subjected to cholecystectomy (Group A), one comprising complicated cases of biliary surgery (Group B), and a control group, including orthopaedic and gynaecological operations as well as gastric resections (Group C). No changes in enzyme activities occurred in die control group. In Group A, GOT and GPT values rose on the first postoperative day, but returned to normal on the second to third postoperative days. In Group B, the rise in GOT and GPT levels was highest on the first postoperative day, but values remained pathologically high even on the second and third postoperative days, to the extent that third‐day values again showed definite rising tendencies (fig. 1). The elevated enzyme activity on the first postoperative day is attributable to liver‐cell lesions caused by operative trauma. The causes of the elevated GOT and GPT values in Group B occurring on the third day are discussed. During the first three days after biliary surgery complicated by acute cardiac infarction, GOT values are unreliable. On the other hand, after operations of the same type as those in Group C, GOT may reliably be used as a diagnostic aid in cardiac infarction.ZUSAMMENFASSUNGBei 145 Patienten (Gruppe A: einfache Cholecystektomie, Gruppe B: Ein‐griffe an den Gallenwegen, Gruppe C: orthopädische und gynäkologische Operationen, Magenresektionen, als Kontrollgruppe) wurden durch 3 Tage nach der Operation GOT und GPT bestimmt. Während die Kontrollgruppe keine Veränderung der Enzymaktivitäten zeigte, fand sich bei Gruppe A ein Anstieg am ersten postoperativen Tage mit folgender Rückkehr zur Norm. In Gruppe B fand sich deutliche Erhöhung am ersten postoperativen Tag, die Werte blieben jedoch weiter hoch oder stiegen sogar an. Die erhöhte Enzym‐aktivität am ersten postoperativen Tage ist durch das operativ gesetzte Lebertrauma bedingt. Die Ursachen der bestehenbliebenden Fermentspiegelerhöhung in Gruppe B werden diskutiert. Bei einem im postoperativen Verlauf auftretenden Herzinfarkt sind nach Galleneingriffen die GOT‐Werte nicht verwertbar, nach anderen Operationen jedoch zur Sicherstellung der Diagnose
