DIRECT MEASUREMENT OF PULMONARY VENTILATION IN NEWBORN AND INFANTS DURING ARTIFICIAL VENTILATION WITH THE ENGSTRÖM RESPIRATOR |

SUMMARYAs a step towards the goal of achieving the most reliable regulation of ventilation in infants during artificial respiration with the Engström respirator, two devices for the direct measurement of ventilation have been tested.In both devices, the respiratory volume is separated from the compression volume by valves. The first combination consists of a pair of check valves of the same type as the inlet valve to the respirator patient circuit. The second system employs one such valve together with a pneumatically controlled valve of the same construction as the outlet valve of the respirator.The first system with two check valves is simple to make, but volume measurement is complicated by the lack of a suitable mechanical measuring instrument.The second system—one check valve plus one pneumatically controlled valve—is complicated by the need of accurately synchronising the pneumatially controlled valve with that of the respirator. A gasometer or spirometer can measure the volume with sufficient accuracy. Moreover, the expired air can be collected in a Douglas bag for measurement or analysis.The valve data were determined. They were found to be adequate and together with the synchronisation were checked on a lung model under physical conditions. The tracheal pressure was measured in patients during ventilation at unaltered respirator settings, both with and without valves. All essential details of the pressure recordings were identical.The valve systems, designed to complement the Engström respirator, can be used continuously, both during artificial respiration under anaesthesia, and during treatment of respiratory insufficiency.ZUSAMMENFASSUNGAls einen weiteren Schritt zur Erzielung der verlässlichsten Regulation der Ventilation bei Kleinkindern unter kun̈stlicher Beatmung mit dem Engström‐Respirator wurden 2 Geräte zur direkten Messung der Ventilation getestet.In beiden Geräten wird das Respirationsvolumen vom Kompressionsvolumen durch Ventile getrennt. Das erste besteht aus einem Paar von Kontrollventilen von der gleichen Type wie das Eingangsventil zum Patientenkreislauf des Respirators. Das zweite System besteht aus einem solchen Ventil zusammen mit einem pneumatisch kontrollierten Ventil von der gleichen Konstruktion wie das Ausgangsventil des Respirators.Das erste System mit den 2 Kontrollventilen ist einfach herzustellen, aber die Volumsmessung ist durch den Mangel an geeigneten mechanischen Mess‐instrumenten erschwert.Das zweite System—hat ein Kontrollventil und ein pneumatisches Ventil—wird kompliziert durch die Notwendigkeit einer akkuraten Synchronisation des pneumatisch kontrollierten Ventiles mit dem des Respirators. Eine Gasuhr oder ein Spirometer kann das Volumen mit ausreichender Genauigkeit messen. Darüber hinaus kann die ausgeatmete Luft in einem Douglas‐Beutel zur Messung und Analyse gesammelt werden.Die Daten der Ventile wurden bestimmt. Sie wurden für geeignet befunden und zusammen mit der Synchronisation an einem Lungenmodell unter physikalischen Bedingungen getestet. Der Trachealdruck wurde bei Patienten wahrend der Ventilation bei unveränderter Respiratoreinstellung gemessen und zwar sowohl mit als auch ohne Ventile. Alle wesentlichen Details der Druckaufzeichnungen waren identisch.Die Ventilsysteme, die zur Ergänzung des Engström‐Respirators entwickelt wurden, können kontinuierlich verwendet werden, sowohl während der künstlichen Beatmung bei der Narkose, als auch während der Behandlung eine

SUMMARY1Measurements of the arterial blood pressure, heart rate, and blood flows in the skin and musculature of the forearm and hand were performed in patients before and during the induction of barbiturate anaesthesia, succinylcholine injection and endotracheal manipulations, such as intubation or suction.2The initial barbiturate anaesthesia and the succinylcholine reduced the vascular resistance in the forearm and hand by 30–70 per cent, as compared with the control values.3Endotracheal manipulations caused tachycardia and an increase in arterial blood pressure together with an increase in the blood flows and vascular resistances in the muscle and skin of the forearm and hand. The changes reached a maximum within 1–2 minutes after intubation. If further movements of the tube were subsequently avoided, the values gradually decreased again.The increase in vascular resistance was greater in the skin than in the skeletal musculature.4The conclusions are drawn that the marked rise of the arterial blood pressure during endotracheal manipulations is at least partially due to an increased vascular resistance in the systemic circulation, and that this is caused by reflex activation of sympathetic vasoconstrictor activity.ZUSAMMENFASSUNG1An Patienten wurden der arterielle Blutdruck, die Herzfrequenz und die Durchblutung in Haut und Skeletmuskulatur von Unterarm und Hand gemessen und zwar vor und nach Beginn der einleitenden Barbituratanästhesie, nach Muskelrelaxation durch Succinylcholin, und während endotrachealer Manipulationen wie Intubation oder Absaugung.2Barbituratnarkose und Succinylcholin verminderten den arteriellen Strömungswiderstand in Unterarm und Hand gegenüber den Ausgangswerten um 30–70%.3Endotracheale Manipulationen wie Intubation oder Absaugung bewirkten Anstiege des arteriellen Druckes, der Herzfrequenz, sowie der Durchblutung und des arteriellen Strömungswiderstandes in Skeletmuskulatur und Haut von Unterarm und Hand. Diese Kreislaufreaktionen erreichten 1–2 min. nach Beginn der endotrachealen Intubation ein Maximum und gingen anschliessend ‐ bei ruhig liegendem Tubus ‐ wieder allmählich zurück.Die Anstiege des arteriellen Strömungswiderstandes waren grösser in der Haut als in der Skeletmuskulatur.4Die arterielle Drucksteigerung bei endotrachealen Manipulationen ist zumindest teilweise durch eine Zunahme des arteriellen Strömungswiderstandes im grossen Kreislauf bedingt, und zwar vermutlich als Folge einer reflektorischen Erhöhung des sympathische

SUMMARY1Eighteen dogs were cooled in ice water of 5–7°C., and the frequency of spontaneous ventricular fibrillation was observed. Five dogs served as controls, nine dogs were treated with 0.5 mg persantin/min., and four dogs with 0.25 mg persantin/min.2In the control group, 80% fibrillated at 19.0–20.4°C. In the group with 0.5 mg Persantin/min., 33.3% fibrillated at 16.0–17.4°C.; in the 0.25 mg persantin/min. group, 75% fibrillated at 16.2–18.1°C.3The pulse rate in the groups treated with persantin showed a sudden extreme bradycardia of 5–9 beats/min. at 20.0–21.8° C., whereas this phenomenon did not occur in the control animals.4The possible mode of action is briefly discussed.ZUSAMMENFASSUNG118 Hunde wurden in Eiswasser von 5 bis 7°C unterkühlt und dabei die Häufigkeit des Auftretens von spontanem Kammerflimmern beobachtet. 5 Hunde dienten als Kontrolle, 9 Hunde wurden mit 0,5 mg Persantin pro Minute und 4 Hunde mit 0,25 mg Persantin pro Minute behandelt.280% der Kontrolltiere zeigten Kammerflimmern, bei 19,0 bis 20,4°C. Aus der Gruppe, die 0,5 mg Persantin pro Minute erhalten hatte, flimmerten 33,3% bei 16,0 bis 17,4°C, und bei der 0,25 mg Persantin‐Gruppe flimmerten 75% bei Körpertemperaturen von 16,2 bis 18,1°C.3Die Pulsfrequenz in der mit Persantin behandelten Gruppe sank bei 20,0 bis 21,8°C plötzlich ab und zeigte extreme Bradycardie von 5 bis 9 Schlägen in der Minute. SUMMARYElectrocardiograms of 32 patients were studied during neuroleptanalgesia. This method of anaesthesia did not seem to protect from all harmful reflexes. Intubation and stretching of the eye muscles seemed to give the same cardiac reflexes as during other forms of anaesthesia.ZUSAMMENFASSUNGBei 32 Patienten, die unter Neuroleptanalgesie standen, wurden die Elektrokardiogramme untersucht. SUMMARYThe respiratory function was studied clinically and by analyses of arterial or capillary blood in 44 unselected unconscious neurosurgical patients immediately on admission to hospital. The actual pH, PCO2and oxygen saturation were determined by the micro‐methods of SiggaardAndersenet al. (1960a, b, 1962)1,2,3.Abnormal blood‐gas values were disclosed in 21 patients, while normal values were obtained in 23, including four who were given oxygen therapy and/or assisted respiration.Of the 21 patients with abnormal values, 13 showed clinical signs of impaired respiratory function; seven of these were intubated. In five non‐intubated patients, intubation before the transport would presumably have prevented respiratory complications. Among the seven intubated patients, deficient function of the endotracheal tube was considered to be the principal cause of the respiratory abnormalities.In one case, no pulmonary or cerebral disorders could be demonstrated, but severe haemorrhagic shock was responsible for the abnormal blood‐gas values.Among the 19 patients with normal values without oxygen therapy, accumulation of secretions occurred in two who were not intubated.ZUSAMMENFASSUNGBei 44 nicht ausgewählten, bewusstlosen, neurochirurgischen Patienten wurde unmittelbar nach der Krankenhausaufnahme die Atemfunktion sowohl klinisch als auch durch arterielle und Kapillarblut‐analysen untersucht. Das aktuelle pH, PCO2und die Sauerstoffsättigung wurden nach den Mikromethoden von Sigaard Andersen und Mitarbeiter (1960a, b, 1962)1,2,3bestimmt.Bei 21 Patienten wurden abnorme Blutgaswerte aufgedeckt, während 23 normale Werte aufwiesen. In der letzteren Gruppe befanden sich 4 Patienten, die Sauerstofftherapie und/oder assistierte Beatmung erhalten hatten.Von den 21 Patienten mit abnormen Werten zeigten 13 klinische Zeichen einer gestörten Atemfunktion; 7 von ihnen waren intubiert. Bei 5 nicht intu‐bierten Patienten hatte wahrscheinlich eine Intubation vor dem Transport ins Krankenhaus respiratorische Komplikationen verhütet. Unter den 7 intubierten Patienten musste die unrichtige Funktion des endotrachealen Tubus als Hauptursache für die Atmungsstörungen angesehen werden.In einem Fall bestanden weder pulmonale noch cerebrale Störungen, doch dürfte der schwere haemorrhagische Schock für die abnormen Blutgaswerte verantwortlich gewesen sein.Unter den 19 Patienten mit normalen Werten ohne Sauerstofftherapie kam es bei 2 nicht intubierten zur Anh
