摘要:
A system of specialty pediatric care management requires an understanding of how medical management and care coordination integrate. This article describes an assessment process undertaken in a specialty pediatric medical center to determine the scope and cost of care management for specific diagnostic groups. The results of this assessment demonstrated the cost of care coordination and the influence of medical severity and psychosocial factors on cost. Replicating the case type assessment approach with clinical care teams expands the understanding of care management functions and helps teams embrace the concepts of severity of illness and complexity of care coordination.

The financial pressures of today's healthcare environment require healthcare professionals to be able to keep all quadrants of quality in check. Tools such as the data card will assist the users of quality data in providing the organization with a catalyst for change. The reproducibility of both the process and the tool has proven to assist an individual organization in creating a clear concise message in the integration of performance measures.This article discusses a methodology and tool for organizations to integrate and evaluate disparate sources of information that can be used to direct priorities for Performance Improvement. The data card is a tool adapted from the Dartmouth Clinical Value Compass used to assist Performance Improvement teams in both the tracking and evaluation of measures related to patient satisfaction, functional status, clinical outcomes, and financial outcomes. The visual enhancement of adding color to the arrows of the compass allows the end users to quickly identify performance improvement opportunities.Examples of the data card are included to assist the readers in identifying specific indicators that can be measured with a specific clinical population as well as adaptation to other uses such as organization-wide indicator measurement. Benchmarking is key in defining best practices and has been integrated into the data card.

摘要:
Noncompliance with a designated healthcare plan is a common barrier to achieving healthcare goals. It can exacerbate health problems for the patient and add significantly to healthcare costs throughout the healthcare system. Researchers have identified certain characteristics that increase the risk of noncompliance. Preliminary screening for these characteristics by the case manager would identify individuals at risk and could provide information that could be incorporated in the care plan for the patient. A screening tool based on identified risk indicators was developed to aid in the recognition of and the planning for individuals at risk for noncompliance.

The healthcare industry is only beginning to understand e-health. E-health can be defined as the use of technology to directly improve healthcare delivery-affording patients the opportunity to participate in their own healthcare management, provider, and institution. The market is changing rapidly, and innovations, partnerships, and mergers are taking place daily. For healthcare institutions, setting a long-term, yet adaptable e-health strategy is of vital importance for the continued success of the organization. For clinicians, an understanding of and familiarity with technologies can significantly improve workflow, organization, and patient interaction. For the patient, technology can be leveraged as a means to take initiative and responsibility for his/her own health. This article defines e-health and explains the implications and benefits of e-health to nurses and their patients. The article also identifies unique opportunities e-health/e-commerce can provide case managers in promoting patient connectivity, care management, and economy in cost of care.
