There has been a tremendous growth in day surgery units to meet the demand for cost-effective healthcare. As a result of increased outpatient use of these units, procedures for administrative and clinical management have been developed. The effectiveness of the use of these protocols, however, has not been tested. This article reports on a study that examined nurses’ compliance with a protocol for postprocedural vital signs measurement in a gastrointestinal day unit. The protocol’s effectiveness in detecting postprocedure complications and the resource implications of the protocol were examined. The rate and type of postprocedure complications detected are also reported.Analysis of the observation data suggests staff are undertaking observations according to the protocol for most patients. Some patients appear, however, to be having their observations done outside the time frame recommended by the protocol. It is possible that staff are exercising their clinical judgement and continuing observations on some patients, though this is speculation and requires further research. Findings from the study raise questions regarding whether postprocedure monitoring is resource efficient.

摘要:
The purpose of this study was to identify priorities for nursing research in the field of gastroenterology nursing using the Delphi technique. A general objective was to provide empirically based information to guide the development of a nursing research program within a specialty nursing organization focused on gastroenterology. Via a two-round Delphi survey, priority research questions were identified from a sample of the membership. Those research questions are presented as well as demographic information of the sample.

摘要:
The effective management of irritable bowel syndrome (IBS) is enhanced by a therapeutic relationship between healthcare providers and the patient. This survey focused on addressing the symptoms and impact of IBS in women, as well as physician perceptions regarding IBS.A national survey was undertaken to 1) describe the attitudes, knowledge, treatment strategies, and compliance of women with IBS; 2) compare women with IBS to those without IBS on general health and activity limitations; 3) compare women with IBS to physicians regarding their relative perceptions about the disorder; and 4) determine whether physician gender influenced patient satisfaction.

摘要:
The treatment for chronic hepatitis C with interferon and ribavirin is associated with a large number of untoward side effects (Cornberg, Wedemeyer &, Manns, 2002). Management of these adverse effects has a great impact on the compliance of affected patients during the course of their treatment. Thus, the influence of the nurse on patient compliance and quality of life improvement during interferon and ribavirin treatment cannot be understated.Anecdotally, from the patient’s point of view, the adverse effects of treatment often overshadow the goal of treatment, viral clearance. Nurses, as a result of their role as patient advocates, have the ability to enhance treatment compliance greatly by recognizing and managing treatment-associated side effects.

Colorectal cancer is the second leading cause of death from cancer in the United States. It is third leading cause of cancer death in men behind lung and prostate respectively, and behind lung and breast in cancer deaths among women. More than 130,000 cases are diagnosed each year with over 56,000 of those patients dying. Six percent of the United States population will develop colorectal cancer in their lifetime.

A bidirectional pathway between the brain and immune system known as psychoneuroimmunology has been the basis of much research and discussion within this interdisciplinary field over the past 25 years. An overly aggressive immune response in inflammatory bowel disease makes the psychoneuroimmunology framework applicable in establishing evidence-based outcomes. Mind-body and mind-gut connections and behavioral interventions can influence outcomes for patients with inflammatory bowel disease. Professional nurses can help develop behavioral interventions to improve quality of life with symptom management for this patient population. Healthcare providers and patients through partnerships should consider the effects of psychosocial factors in manifestations of health and illness.
