Knowledge of oxygen saturation and the timing of nursing interventions were studied for their relationship to positive patient outcomes in a group of patients receiving intravenous conscious sedation during endoscopy. The convenience sample consisted of 38 patients receiving midazolam or a combination of midazolam/fentanyl during esophagogastroduodenoscopy or colonoscopy in a hospital-based endoscopy unit. Data collection included monitoring oxygen saturation levels, physiological parameters, and skin color. Baseline arterial oxygen saturation was obtained and monitored continuously throughout the procedure and during the recovery period. During the procedures, the nurse was aware of the oxygen saturation levels for half of the subjects. For the other subjects, the nurse was informed only when the oxygen saturation dropped below 85%. Data revealed a clinically significant relationship between knowledge of oxygen saturation and the timing of nursing interventions in effecting positive patient outcomes during endoscopic procedures. The results of this pilot study contributed to the development of standards of care for monitoring oxygen saturation in an endoscopy unit. The timeliness of nursing interventions was enhanced by the use of pulse oximetry, thereby producing positive patient outcomes and improving the quality of care for the endoscopic patient receiving conscious sedation.

Nursing process is referred to as the basic framework for the practice of nursing, which allows the nurse to provide care in a systematic, organized fashion. Because nursing is an evolving profession, this framework is constantly being modified and adapted to current practice. Periodic analysis and review of the current trends in nursing process allow the practicing nurse to improve patient care. This article serves as an overview of observations on nursing process including historical data, a description of each of the five steps of nursing process, and application to daily practice.

Recent advances in the field of flexible fiberoptic technology have facilitated the diagnosis and treatment of a wide variety of gastrointestinal diseases in children. As a result, pediatric patients are becoming more common in the endoscopy suite. Although many technical aspects of colonoscopy are the same whether applied to adult or pediatric patients, subtle differences do exist. Recognition of these differences will lead to a safer and more successful procedure for the pediatric patient. This article addresses the application of colonoscopy to pediatric populations based on review of current literature and personal experience of the author.

Textbooks and articles that address the issues and responsibilities of a nurse manager generally focus on the role of the nurse manager in a hospital setting. While many of the principles set forth in these publications may be appropriately used by the nurse manager working in an endoscopy setting, certain characteristics of GI nursing are unique in terms of managing the delivery of patient care, managing staff, and managing self. This article describes these critical “3Ms” of nursing management and offers practical guidelines to the GI nurse manager.
