Peptic ulceration is multifactorial in both causes and treatments. Agents that have been shown to be beneficial in the treatment of ulcers include antacids, histamine receptor antagonists, proton pump inhibitors, cytoprotective agents and antimuscarinic agents. Knowledge concerning characteristics of these medications will help nurses to provide more effective health care.

Malnutrition is a large problem among patients admitted to U.S. hospitals and is an area of increasing concern for GI health care professionals. We conducted a prospective assessment of the admission nutritional status of 500 consecutive patients admitted to the Huntington Veterans Affairs Medical Center. Admission nutritional status was compared to the length of hospital stay (LOS). Nutritional status was calculated based on three equally weighted and easily obtained parameters — serum albumin, total peripheral blood lymphocyte counts and unintentional weight loss over time. A significant difference (p< 0.01) was noted for the LOS between patients with normal, mildly abnormal and moderately abnormal nutritional status as compared with the patients with severe malnutrition. The length of hospital stay progressively increased with deterioration of nutritional status. The GI practitioner is in a key position to assess nutritional status of hospitalized patients. If malnutrition can be documented on hospital admission, attempts can be made to reverse the malnutrition and hopefully diminish LOS.

Although the governing of medical conduct has always existed, medical ethics has become an issue to be addressed in the 1990s. The purpose of this article is to address the dilemma of reinserting a feeding tube in a patient whose wishes are unclear and whose family and physicians are in conflict. By presenting a case study, the author encourages the reader to resolve this dilemma on a personal level.

Cardiac transplants have emerged as life-extending procedures for patients with diseased hearts. Their frequency has increased over the past decade. Evaluation of the clinical course of these patients has revealed trends toward development of gastrointestinal complications which involve the upper and lower gastrointestinal tracts as well as the accessory organs. This article will discuss the complications that have been seen in the post-cardiac transplant patient as reported at Saint Joseph's Hospital of Atlanta. A more detailed discussion of opportunistic infections caused by immunosuppression therapy will be presented since these are the major complications seen in this institution. This article will share this institution's practice in regard to caring for the cardiac transplant patient. Nursing implications relating to the GI endoscopy specialty will be included. Our experience has shown that although these complications are common, they may be managed effectively through early evaluation with attention to the special needs of the immunocompromised endoscopy candidate.

Most nurses are inadequately prepared for critically analyzing research. Gastrointestinal ‘nursing’ encompasses a diverse group which includes registered nurses, licensed practical and vocational nurses and trained technicians. The parameters of practice sometimes overlap within this group regardless of formal educational background. It is imperative to the safety of ourselves and our patients and to the advancement of gastrointestinal nursing practice that we are capable of making intelligent, informed decisions regarding the research and reports we are offered. This paper discusses the key elements of research methodology to enhance the interpretive skills of the reader.

Biliary obstruction can prohibit therapeutic endoscopic stent placement. For patients with this condition, percutaneous transhepatic biliary catheters can provide an alternative method to drain the obstructed system. The biliary catheter presents nurses with several challenges: assessment of the patient for potential complications, home care teaching and supportive care.
