There are few data available describing the experience of patients who have undergone photodynamic therapy with porfimer sodium for Barrett’s esophagus. We describe the results of a satisfaction survey reported by 16 of 18 patients (11 men, 5 women; median age 75 years; median response at 27 months after treatment) treated with photodynamic therapy for Barrett’s esophagus with high-grade dysplasia. Treatments were performed on an outpatient basis although two patients required clinic visits for intravenous fluids.Subjects reported their most significant post-treatment problem was odynophagia or dysphagia (75%), which was best treated with a hydrocodone bitartrate and acetaminophen elixir (75%). Cutaneous photosensitivity persisted for a median of six weeks; two patients had phototoxic reactions requiring clinic evaluation and treatment. All but two patients reported swallowing problems lasting a median of four weeks, and weight loss (median 6.8 kg). All patients indicated they would again choose photodynamic therapy if they were faced with a similar choice of endoscopic treatment versus surgery for Barrett’s esophagus with high-grade dysplasia. These results indicate a generally high level of satisfaction in patients who have been treated with porfimer sodium photodynamic therapy for Barrett’s esophagus with high-grade dysplasia.

Flexible endoscopes are complex medical instruments that are easily damaged. In order to maintain the flexible endoscope in optimum working condition, the user must have a thorough understanding of the structure and function of the instrument. This is the fourth in a series of articles presenting an in-depth look at the care and handling of the flexible endoscope. The first three articles discussed the air-water system, the suction channel system, and the mechanical system. This article will focus specifically on the endoscopic retrograde cholangiopancreatography elevator system.

Several studies have documented patients’ intake of parenteral and enteral nutrition following gastrointestinal surgery. There is a lack of knowledge, however, regarding patients’ experiences of intake of food and fluid following gastrointestinal surgery. The aim of this study was to describe patients’ experiences of their appetite, hunger, changes in weight, and intake of food and fluid three months after gastrointestinal surgery due to tumor. Fifteen patients were invited to be interviewed regarding their experiences following gastrointestinal surgery due to tumor. The method used for data analysis was a qualitative content analyses approach. The categorization of data identified three themes: 1) the struggle to eat and drink; 2) bodily estrangement; and 3) nutritional treatment regimens. These three themes consisted of subthemes such as: lack of appetite and hunger; difficulties in eating and drinking; feelings of nausea; and fighting and adaptation. The findings contribute to a deeper understanding of these patients’ nutritional regimen and suggest the individual’s personal situation and living circumstances should be considered when developing a nutritional plan following gastrointestinal surgery for tumor.

The purpose of this article is to present an overview of diabetic gastroparesis, defined as delayed gastric emptying in the absence of mechanical obstruction. Diabetic gastroparesis is a substantial and unrecognized problem. Failure to treat may result in a decreased quality of life and a potential increase for morbidity. The treatment protocol for diabetic gastroparesis combines dietary and pharmacologic measures. This article will discuss normal physiology of gastric emptying along with the pathogenesis of delayed emptying in patients with diabetes. Nursing implications for the care of the patient with diabetic gastroparesis is also presented along with commonly used pharmacologic agents.
