Summary:Many peripheral meniscal tears are amenable to repair. An important aspect of these repairs is a safe, reproducible surgical approach for effective meniscal repair, retractor placement, and, most important, minimization of the potential for a neurovascular complication. This article reviews surgical approaches for medial and lateral meniscal repair.

Summary:The surgical techniques required for repair of complex meniscal tears, including tears in the central one-third avascular region, are technically demanding. The technique requires that both superior and inferior sutures be placed in a stacking arrangement to completely approximate and close the meniscus tear site. This technique provides greater inherent stability to the meniscus repair site and also allows immediate knee motion. The use of many individual sutures meticulously placed to completely approximate and close the tear site permits more complex tears to be repaired, instead of excised, thereby retaining meniscus function. This article discusses the technique and principles important to meniscus repair.

Summary:Rehabilitation after meniscal repair should enhance meniscal healing and facilitate an early return to normal knee function. Traditional approaches differ in immobilization and weight bearing but prevent early motion. More recently accelerated protocols begin motion early. There is a need for prospective comparative clinical studies to evaluate these methods as well for basic science research to assess the effects of immobilization, weight bearing, and range of motion on meniscal healing. The authors have adopted a middle-of-the-road approach, which includes components from various protocols.
