Summary:Posterior wire fixation for cervical fusions can be used for any type of cervical instability, including traumatic, degenerative, infectious, or tumors. Two techniques are described: (a) the Bohlman triple wire technique, to be used with an intact posterior neural arch, and (b) the Robinson Southwick technique, used when the lamina and spinous processes have been removed. With these two techniques virtually any type of posterior cervical arthrodesis can be accomplished using only a drill, 18-or 20-gauge wire, and autogenous bone graft. An excellent fusion rate and low complication rate can be expected.

Summary:The technique of lateral mass stabilization using AO reconstruction plates is reviewed. We treated 102 patients with minimum follow-up of 12 months. All patients have undergone successful fusion. Complications include two radiculopathies thought to be related to drilling and screw placement. Lateral mass fixation provides excellent stability but requires meticulous surgical technique. This fixation is indicated in cases of missing or fractured spinous process, multilevel fractures or instabilities, rotational instabilities associated with facet fracture, burst type fractures with posterior ligamentous injury, failed anterior fusions, and neoplastic conditions and when extension is required to the thoracic or occipit.

Summary:The technique of posterior plate fixation for the unstable cervicothoracic spine is safe and efficacious. The surgeon must have a thorough understanding of the relevant anatomy and exercise proper caution in the application of this technique. In patients with insufficient anterior columns or severe deformities, a combined anterior approach and strut grafting should be considered to prevent secondary deformity and loss of reduction.

Summary: Anterior cervical fusion has become established as a standard of care. The addition of plate fixation has increased the initial stability and extended the indications. The current systems that provide a rigid screw-plate interface obviate the need to penetrate the posterior vertebral cortex in most circumstances. The number of patients requiring both an anterior and posterior stabilization has also been decreased.
