Summary:Because the menisci play an integral role in the complex biomechanics of the knee, increasing efforts are being made to preserve these structures whenever possible. Experimental studies have shown that the menisci are able to generate a healing response if lesions are located within the peripheral vasculature of the tissue. Understanding the basic science of meniscal healing is essential if one is to appreciate the capabilities and limitations of the meniscus with regard to repair.

Summary:Complications occurring with arthroscopic meniscal repair are discussed. Various techniques of repair are presented, with comparisons of the complications of each. Appropriate technical measures are presented to allow the surgeon to perform successful arthroscopic meniscal repair without catastrophic complications. Frequently encountered complications of a lesser nature are also discussed, and appropriate suggestions are made on how to avoid them.

Summary:An inside-out arthroscopic technique using a single or a double lumen cannula system is an effective means of reducing and stabilizing repairable meniscal tears. Limited posterolateral or posteromedial approaches for capsular exposure are recommended to decrease the incidence of neurovascular complications. Concomitant anterior cruciate ligament (ACL) reconstruction is strongly recommended for ACL-deficient patients with repairable meniscal lesions.

Summary:The zone-specific system allows arthroscopically controlled repair of all viable meniscus tears. The zone-specific cannulae are anatomically matched to the normal contours of the femoral condyles and tibial spines allowing easy passage of specially designed, double-armed sutures. This arthroscopic inside-out technique allows visualization, preparation, and repair of tears not approachable by open methods. Suture capture and fixation is accomplished using a 3-cm posteromedial or posterolateral incision exposing capsule and protecting the neurovascular structures. As with other techniques, zone-specific meniscus repair demands proper tear selection and preparation prior to suturing (Fig. 1).

This method delivers suture through the lumen of an 18-gauge spinal needle directed from an anatomic safe zone outside the knee into an intraarticular location visualized with an arthroscope.

This method places sutures across the tear and ties intraarticular knots through an operative cannula, resulting in a vertically oriented suture repair that apposes the meniscus only and excludes the joint capsule.

Summary:Clinical and experimental studies have demonstrated the importance of the meniscus in load transmission, stability, shock absorption, and articular cartilage nutrition in the knee joint. As a result, clinicians have emphasized preservation of the injured meniscus when possible. Meniscal healing is dependent on a blood supply or factors derived from serum. An exogenous fibrin clot placed in a stable lesion in an avascular portion of the meniscus can support a reparative response by providing such serum factors. In this review we describe the basic science aspects of meniscal repair, and the indications, technique, and results of meniscal repair using an exogenous fibrin clot. Directions for future research on meniscal repair and replacement based on our increasing understanding of meniscal biology are briefly discussed.
