Arthroscopy is the most commonly performed elective orthopaedic procedure in the United States and Canada. White the place of diagnostic arthroscopy has become well established in orthopaedic surgery, it is this author's belief that arthroscopic surgery has yet to find its proper place in the armamentarium of the orthopaedic surgeon. This article presents what the author considers to be the indications and relative contraindications for and advantages and disadvantages of arthroscopy.

The technique of arthroscopic lavage and debridement presented in this article is a low-morbidity procedure that allows early pain relief and early range of motion. The author has obtained satisfactory results using this technique, which affords the advantages of the classical treatment methods for infected knees and avoids some of the disadvantages (such as incomplete removal of debris and increased morbidity and scarring). The author's preferred postoperative regimen is presented.

The severity of anterior knee laxity, as determined by the pivot-shift test, has recently demonstrated markedly different findings when changed from internal to external tibial rotation. The authors and others believe that the external-rotation pivot-shift test is sensitive to translocation of the medial side of the knee and helps determine the degree of anterior translation and anteromedial rotation of the medial tibial plateau. Basic knee anatomy and biomechanics as they relate to the medial side of the knee are discussed to help understand the significance of this test. Descriptions and comparisons of the various pivot-shift tests are also presented. Finally, the authors' preferred surgical procedures for the correction of these complex medial joint injuries are described.

Arthroscopic management of loose bodies in the knee can be either the easiest or the most frustrating of cases. Successful surgery is predicated by appropriate diagnosis and a systematic approach of all knee compartments. Various sites in the knee are known havens for loose bodies and must be inspected. Most loose-body removals require only standard arthroscopic equipment, but a thorough knowledge of portal anatomy and access is necessary.

The treatment of osteochondritis dissecans of the knee is a controversial topic. One common theme is that joint surface restoration is of paramount importance. Arthroscopy has numerous advantages, is strongly advocated in the initial treatment of osteochondritis dissecans, and should be in the forefront of the orthopaedist's armamentarium of diagnostic and therapeutic modalities. The stage of the lesion dictates the course of treatment, which is described in detail.

Meniscal repair is supported by well-founded biomechanic principles. It is hoped that meniscal preservation will prevent osteoarthritis from occurring after a total meniscectomy. Three techniques of meniscal repair are presented: open repair, closed arthroscopic repair, and combined incision and arthroscopic repair. The surgical technique and complications of each repair are explained.
