Summary:Different hallux valgus deformities require different types of surgical correction. Incongruent metatarsophalangeal joints, greater hallux valgus angles (generally >30°), and metatarsus primus varus (> 12°) are indications for a basilar metatarsal osteotomy with distal soft-tissue realignment. With proper technique and selection of patients, 85–90% good or excellent results can be expected.

Summary:The chevron bunionectomy is a distal metatarsal osteotomy used to correct bunions with a mild-to-moderate hallux valgus deformity. The indications for the procedure and a step-by-step description of the technique are detailed. The procedure has a high degree of patient satisfaction when performed for the proper indications. Care must be taken to perform the technique properly.

Summary:Metatarsocuneiform arthrodesis is one option that should be considered when treating hallux valgus in adolescents and adults with metatarsus primus varus and first-ray hypermobility. With this procedure, correction of deformity with low risk of recurrence is made possible through the use of stable internal fixation and by preserving first-ray length. In this article we describe in detail one preferred surgical technique, aftercare, and rationale for appropriate selection of patients.

Summary:A previously unreported modification to the Cole mid-foot osteotomy is described for correction of rigid equinocavovarus foot deformities. Axial derotation or pronation of the forefoot prior to the second cut of a wedge osteotomy of the anterior tarsus allows for correction of cavus, varus, and associated supination deformity of the foot without creating a lateral rocker, prominent fifth metatarsal base, excessive shortening, or violation of hindfoot articulations. Pronation of the forefoot may exacerbate an existing plantar-flexed first metatarsal bone, which can be corrected by dorsiflexion osteotomy of the first ray, resulting in a mobile plantigrade foot.

Summary:Tarsal tunnel syndrome represents a compression neuropathy of the posterior tibial nerve and its branches within the tarsal tunnel. The anatomy, etiology, diagnosis, and surgical treatment are presented. In properly selected patients,a 90% rate of success with improvement or resolution of symptoms can be expected with surgical release.

Summary:Ankle fusion remains the definitive treatment for advanced arthritis of the ankle joint. Internal fixation with cancellous screws has evolved into the method of choice for achieving arthrodesis in primary uncomplicated cases. The method described leads to rapid fusion with a low incidence of infection and a high success rate using readily available cancellous screws.
