Arthroscopy is gaining increasing popularity in treating disorders about the shoulder. A working knowledge of the surgical anatomy of both the shoulder joint and the subacromial bursa is essential when performing these types of procedures. This article discusses the anatomy of the shoulder, both on the surface and as it would appear when visualized through an arthroscope.

摘要:
Arthroscopic surgery of the shoulder is technically reproducible if proper attention is given to equipment details and operating room personnel. A facility committed to arthroscopic surgery will provide the finances to maintain and update equipment. The surgeon should be familiar with the proper operation of arthroscopic equipment.The patient is anesthetized and placed in the lateral decubitus position. The arm is suspended from a two pulley system attached to the table at 45° to 60° of abduction and 15° of forward flexion, allowing the torso to roll 30° posteriorly. The surgeon and assistant stand posterior to the patient; the equipment and the scrub nurse are on the opposite side of the table. The patient will benefit from having surgery performed under optimum conditions.

Decubitus orientation is described and advocated for glenohumeral arthroscopy and subacromial bursoscopy. Methods of maintaining decubitus orientation are discussed. The anatomic landmarks are described from this perspective for both the glenohumeral joint and subacromial space. Decubitus orientation facilitates triangulation in both the glenohumeral joint and the subacromial space.

This article outlines the complications of shoulder arthroscopy and discusses methods to minimize the risks of this procedure. It encompasses complications inherent in arthroscopy in general, including damage to the articular surface, infection, and burns, as well as complications specific to the shoulder, including direct injury to neurovascular structures, portal selection, nerve strain, and saline extravasation. Special consideration is given to arthroscopy of the subacromial bursa and also to complications related to capsular stapling.

Arthroscopic techniques are applicable in the treatment of stage 3 and intractable stage 2 impingement syndrome of the shoulder. While adhering to Neer's guidelines for decompression of the subacromial space, arthroscopy further allows for outpatient surgery, less disturbance of the deltoid function, less scarring, fewer complications, and a thorough diagnostic evaluation of the glenohumeral joint and rotator cuff. A surgical technique utilizing the lateral acromial portal is described. Three and one-half years of clinical experience is evaluated and guidelines are suggested.

Debridement of the acromioclavicular (AC) joint and distal clavicle resection can be performed using arthroscopic techniques. Standard arthroscopic equipment is used, with direct visualization of the AC joint via the subacromial bursa. The techniques require careful dissection of the capsular and soft-tissue structures from the undersurface, followed by resection and removal of the bone in cartilaginous debris from the joint from above. Postoperative rehabilitation is usually quite benign and results seem to be similar to open AC joint resection with the benefit of a more cosmetically acceptable incision.

Rotator cuff lesions are frequently associated with varying degrees of disability. Recently, arthroscopic techniques have been established that provide visualization of the glenohumeral joint and subacromial bursa and allow a thorough evaluation of rotator cuff lesions and associated pathology.Arthroscopic procedures for evaluating and treating rotator cuff lesions are described. A system for classifying rotator cuff lesions, based on arthroscopic findings, is presented. The classification of these lesions provides an accurate description of the involved pathology and may aid in predicting the response to various forms of treatment.Shoulder arthroscopic techniques are gaining popularity and early clinical results are encouraging. Although open surgical procedures about the shoulder have withstood the test of time, often providing satisfactory results, we believe that shoulder arthroscopy plays a major role in the evaluation and treatment of rotator cuff lesions.

Two techniques for arthroscopic reconstruction for anterior shoulder instability are described. The first is an arthroscopic modification of the Bankart repair, and the second is a modification of the Gallie reconstruction utilizing freeze-dried fascia lata as a graft substitute for the anteroinferior glenohumeral ligament. The indications for each procedure and techniques for performing the procedures are described, as is postoperative rehabilitation.

This article presents a technique of arthroscopic anterior shoulder capsulorrhaphy using a specialized cannula system and a cannulated screw and a ligament washer for fixation. The procedure provides an arthroscopic surgical approach for repair and reconstruction of lesions of the glenohumeral ligament labral complex (GLLC). This system was developed to provide solid fixation of an arthroscopic anterior capsulorrhaphy, the key element of which is GLLC advancement or capsular shift. The technique is discussed in detail, and initial results of the first 23 patients are presented.
