AbstractEtretinate has proven to be effective in the treatment of psoriasis. Since abnormal proliferation and cytokine secretion are well‐known features of psoriatic epidermis, we studied thein vitroeffects of etretinate on these two processes using human keratinocytes. Etretinate promoted proliferation of normal human keratinocytes (NHKs) grown in keratinocyte growth medium (KGM) but not in growth factor‐deficient keratinocyte basic medium (KBM). Moreover, etretinate partly overcame growth inhibition by PMA. Etretinate was shown to have an effect on either IL‐1α or IL‐8 secretion in unstimulated NHKs. In HSC‐1, a human squamous cell carcinoma cell line cultured in 20% FCS/DMEM, inhibited IL‐1α secretion and enhanced IL‐8 secretion. These results indicate that the effects of etretinate on keratinocyte proliferation and cytokine secretion may depend on cell type and culture conditions. Stimulation of NHKs with PMA significantly enhanced IL‐1α and IL‐8 secretion, and these effects were inhibited by etretinate. However, etretinate failed to inhibit rTNFα‐induced IL‐8 secretion, suggesting that etretinate regulation of NHK cytokine secretion may also depend on the stimulus. As treatment of keratinocytes or epidermis with PMA can induce psoriasis‐like changes, so might the experimental “anti‐PMA” activity of etretinate be related to its therapeutic bene

AbstractThis study was designed with two purposes: first, to elucidate immunologic mechanisms in different cutaneous reactions, particularly in hypersensitivity to mosquito bites, and, second, to develop a more reliable and safer method of identifying the causative species of mosquito in severe cases.The amounts of IgG, IgG4and IgE specific to the mosquito salivary gland extract ofAedes albopictuswere determined in the sera of 116 volunteers with normal reactions, either immediate or delayed, and 4 patients with severe systemic symptoms caused by mosquito bites. Titers of IgG and IgE in the severe cases were considerably higher than in volunteers with normal reactions, but there were no differences in IgG, titers between the two groups. These results indicate that high titers of IgG and IgE may be involved in development of systemic symptoms in severe cases and verify the possibility ofin vitrotests to identify causative species of the mosquito.

AbstractLangerhans cells (LCs) are epidermal antigen‐presenting cells capable of initiating a specific T lymphocyte‐mediated immune response. It is a well known fact that ultraviolet light B (UVB) suppresses LC number and function.In this study, we confirmed that the sunscreens CITY BLOCK, and TOTAL SUN SHIELD 28 (Clinique Laboratories Tokyo, Japan) protected the epidermis against the depletion of LC number. We also investigated whether or not sunscreens could provide LC protection from ultraviolet ray (UVR) damage other than the prevention of the decrease in the total number of cells.Our data showed that the LC population was depressed after irradiation by 100 mJ/cm2or 10 mJ/cm2of UVB, but recovered to within normal levels after 16 days. Both sunscreens provided protection against erythema and LC depression due to UVB irradiation. However, despite the fact that these sunscreens had completely suppressed UVB erythema, shrinkage of LC dendrites was seen. Apparently, sunscreens prevent UVB erythema, but do not protect against functional changes in LC due to UVB.Recently, it has been reported that sunscreens are less effective in protecting against systemic immunosuppression that against inflammation. AbstractAutoantibodies in sera from patients with autoimmune blistering diseases were detected by immunoblotting with chemiluminescence. This method provided the high sensitivity in the detection of autoantibodies against human epidermal extract. It was useful for detecting low titers of autoantibodies, and identifying small amounts of autoantigens in antigen extracts.

AbstractThe records of 10 patients who died with pemphigus have been examined for factors affecting and contributing to death. AbstractSixty‐seven patients with basal cell carcinoma (BCC) were observed at the Institute of Dermatology, Bangkok, Thailand between 1982 and 1989. The male:female ratio was 1:2. The average age of the patients at first examination was 60.6 years. Lesions in sun‐exposed areas were three times more frequent than lesions in covered areas. The principal location was the face, including the cheek, nose, periorbital area, ears, and forehead. The remainder were on the scalp, breast, abdomen, back, thigh, and knee. Ulceration was the most frequent symptom, followed by a mass. Histopathologically, pigmented BCC was the most frequent, followed by solid BCC, superficial BCC, adenoid BCC, morphoeic BCC, cystic BCC, basosquamous BCC, mixed carcinoma, keratotic BCC, and fibroepitheliomatous BCC. In patients younger than 50 years of age, solid or pigmented tumors were frequent. The morphoeic type was present only on the face and scalp. In females, solid BCC or adenoid BCC predominated. Only 4 of our 67 patients had histories of ingestion of herbal medicine with arsenic as one of the ingredients. Sixty‐one patients were treated surgically. There was only one case of tumor recurrence after surgery. The remaining patients were treated by electric cautery, liquid nitrogen, or irradiation. No patients developed distant metas

AbstractA 43‐year‐old woman presented with a well differentiated squamous cell carcinoma of the left earlobe where a small hole for pierced type earrings had been made about 20 years earlier. A year after the removal of the tumor, a metastasis of a moderately differentiated squamous cell carcinoma to the left cervical lymph nodes was detected. A month later, a solid tubular carcinoma of the left breast was found. A modified radical neck dissection and a quadrantectomy with an axillary lymph node dissection were performed. A total of 5 courses of peplomycine‐mitomycine combinated therapy with 26 Gy of radiotherapy was administered for the cervical lesion. A month of postoperative chemotherapy with tegafur‐uracil and medroxyprogesterone acetate and a total of 56 Gy of radiotherapy to the mammary lesion were added. She has been well without recurrence for about a year after the operations. This case is not only a rare double cancer, a combination of a squamous cell carcinoma and a solid tubular carcinoma, but may also be the first case of a squamous cell carcinoma arising at a long pierced site on an earlobe for pierced type e

AbstractBackgroundMultiple, pale, atrophic depressions surrounded by erythema developed on nodular cutaneous mucinosis lesions in a patient with systemic lupus erythematosus.ObservationsA 22‐year‐old man developed multiple nodular erythematous lesions on the upper arms, back, and chest during the course of systemic lupus erythematosus. Histologically, these nodular lesions were diagnosed as lupus mucinosis. Three years later, the patient began to have atrophic depressed lesions on several nodular mucinosis lesions. These lesions did not develop on normal appearing skin. A biopsy specimen from one of these lesions showed occlusion of blood vessels with thickening of their walls and perivascular dense lymphocytic infiltration. No leukocytoclastic vasculitis was found. Hyaline degeneration was present in some parts of the fatty tissue.ConclusionThe atrophic depressed lesions were atrophie blanche‐like and induced by the vascular occlusion.
