AbstractIn order to clarify the effects of temperature on vibration‐induced white finger, two groups of chain saw operators in the forestry industry served as the subjects of this study. One group worked at a cold high altitude; the other group worked at a warm low altitude. Except for the difference in temperature, these two groups had similar work environments. The high altitude group had a high prevalence of white finger (75%), but none of the low altitude group suffered from this circulatory disturbance. AbstractSixty‐two and 38 psoriatic patients were treated with topical PUVA and combined etretinate and topical PUVA (Re‐PUVA), respectively. In both groups, 50% of the patients showed initial recovery after 6 weeks and over 90% after 14 weeks. Re‐PUVA was more effective than PUVA alone in obtaining complete clearance (p<0.05). To clear psoriasis in 50% of the patients, PUVA and Re‐PUVA required 63 and 26 weeks, respectively. Furthermore, the integrated clearance rates after 70 weeks were 50% in PUVA and 63% in Re‐PUVA. Each therapy showed a similar remission period; psoriasis recurred in 50% of the patients after 4 months. In addition, 17 patients were treated with oral etretinate, and Re‐PUVA was found to be more effective than etretinate monotherapy. Another aim was to determine whether etretinate would inhibit the development of PUVA side effects. Adding etretinate failed to inhibit the production of PUVA lentigines but clearly suppressed antinuclear antibody (ANA) expression. AbstractOral administration of cyclosporin A (CsA) 14 mg/kg/day has been reported to improve psoriasis in a double‐blind study. The purpose of this study was to evaluate the efficacy of a lower dose of CsA in severe psoriasis and to monitor lymphocyte subpopulations in peripheral blood in order to detect its mechanism of action. Eleven patients with severe, active psoriasis were treated only with oral administration of 5 mg/kg/day CsA twice a day for 3 months, during which they were closely examined, including single‐ and two‐color analysis of lymphocyte surface markers by flow cytometry using monoclonal antibodies. Seven patients showed improvement within a week, and the others within 2‐3 weeks. Five patients had total remission, 2 showed marked improvement, and 4 showed moderate improvement; no clinically important side effects, except hypertrichosis in 2 females were seen. In the peripheral blood of patients treated with CsA there was a decrease in the percentages of helper inducer (CD4+4B4+)T(Thi) cells, while no significant decrease was found in those of suppressor inducer (CD4+2H4+)T(Tsi), suppressor effector (CD8+CD11+)T(Tse), or cytotoxic (CD8+CD11‐)T(Tc) cells, resulting in a decrease in the ratio of Thi to Tsi, Tse and Tc. The activated helper (CD4+HLA‐DR+)—suppressor (CD8+HLA‐DR+)T cell ratio was also decreased. AbstractErythrocyte selenium dependent glutathione peroxidase (GSH‐Px) activity and plasma vitamin E levels were measured in 20 psoriatics. Psoriatics had significantly lower GSH‐Px and plasma vitamin E levels than did controls. AbstractImmunopathological studies on a patient with clinical evidence of epidermolysis bullosa acquisita (EBA) revealed an increase in type IV collagen in the skin of the chronic scarred lesion, but not in the clinically normal skin. AbstractThe culture medium for normal human monocytes pretreated with interleukin (IL)‐1, either α and β, induced normal human monocytes to spread and to increase Fc (IgG) receptor sites, angiotensin converting enzyme production and glucose consumption of normal human monocytes, suggesting that this medium contains a monocyte activating factor(s). On TSK G3000SW gel filtration, the major monocyte activating factor in the culture medium and that in sarcoidosis serum were eluted at the same position (with a molecular weight of about 70,000). Thus, IL‐1 may be able to induce normal human monocytes to produce a monocyte activating factor quite similar to that found in sarcoidosis
