AbstractWe did a statistical study of 294 strains ofStaphylococcus aureus (S. aureus)isolated from skin infections during the period from January of 1989 to December of 1991 in the Department of Dermatology, Kansai Medical University Hospital. We especially examined methicillin‐resistantS. aureus(MRSA) from the point of view of incidence, variety of skin infections with MRSA, coagulase type, phase type, and resistance against antimicrobial agents. The frequency of isolation of MRSA has been increasing. In 1991, the proportion of MRSA isolates among allS. aureusstrains isolated from skin infections was 41.5%. MRSA was isolated most often from infectious decubitus. Coagulase type II and phage group NT (not typable) MRSA were most frequently isolated. The resistance of MRSA to OFLX and IMP/CS had remarkably increased. Notably, the resistance to MINO was low before 199

AbstractThe pathomechanism of furuncle has not been fully elucidated and should be investigated using an appropriate animal model. We observed the invasion ofStaphylococcus aureuscells into hair follicles in mice, using a strain ofS. aureusisolated from human furunculosis. Light microscopical examination revealed thatS. aureuscells attached to corneocytes at 6 h after inoculation, proliferated around the ostium of the hair follicle and invaded the hair follicle at 12 h after inoculation. Electron microscopically,S. aureuscells attached to the horny layer of hair follicle with long, thick, string‐like structures. At 12 h after inoculation,S. aureuscells invaded in a file between the inner root sheath and outer root sheath. We could not induce direct invasion from the follicle ostium. Our findings suggest that there are some regions of the hair follicle through whichS. aureuscells can relatively easily invade deeper into the follicle. The most important question is what confines the invasion and inflammation ofS. aureusto the hair follicle. We suggest that there is some locus minoris for invasion into hair follicles byS. aureus, such as an interface between the two sheath

AbstractMedullasin is one of the serine proteases found in human bone marrow cells. The protease in granulocytes is assumed to play an important role in the induction of chronic inflammation.We measured the medullasin levels as a parameter of neutrophil function in granulocytes from patients with pustulosis palmaris et plantaris. All 17 patients studied underwent tonsillectomies, and the changes in the amount of medullasin were followed after surgery. Seven of the 17 patients were cured during the follow‐up period (average: 21 months). In all the patients in whom PPP improved, the medullasin levels decreased significantly from the period of active disease when pustules were present to the inactive period. The amount of medullasin did not fall significantly in patients whose PPP did not improve. It is concluded that medullasin levels may reflect the activity of PPP and may serve as a criterion for performing a tonsillectom

AbstractThe pharmacokinetics of meropenem were investigated to examine its penetration into the skin of third degree burned rats. The rats were divided into two groups. One group acted as the control, and the other group had third degree burns induced by immersing their backs into 80°C water for 20 seconds. The rats in each group were given 20 mg/kg of body weight of meropenem intravenously by one bolus injection seven days after burn inducement or depilation. In the non‐burned control group, the maximum concentrations of meropenem in the serum and skin of 6.03µg/ml and 0.12µg/g were respectively obtained 15 minutes after the injection and decreased very rapidly thereafter. In the burned rats, the maximum concentrations of meropenem in the serum, skin (eschar), and exudate fluid of 7.07µg/ml, 1.44µg/g and 5.99µg/ml were respectively obtained 15 minutes after the injection and decreased very slowly. The penetration of meropenem into the burned skin was higher than that into the normal skin. These results suggest that meropenem is a very useful antibiotic in the treatment of burn in

AbstractNodular fasciitis (NF) is regarded as a disease of reactive growth of fibroblasts, although it frequently shows clinically rapid growth and sometimes has to be histologically discriminated from fibrosarcoma. In order to confirm the position of NF as a benign tumor, we studied 5 cases of NF by a quantitative analysis of the nuclear DNA content using flow cytometry in paraffin‐embedded specimens. In 3 of the 5 cases, an immunohistochemical study using Ki‐67 monoclonal antibody was also conducted in frozen sections. In the analysis of nuclear DNA content, DNA histograms showed a diploid pattern in all cases, and G1, S and G2+ M periods were calculated as 96.7%, 2.5% and 0.8% (on the average), respectively. Although the labeling index of Ki‐67 antibody (positive cells/tumor cells; %) was as scattered as 1.3–7.8 (5.6 on the average), the mean value was higher than that in already reported dermatofibroma and dermatofibrosarcoma protuberans and lower than that in malignant fibrous histiocytoma. AbstractLevels of epidermal growth factor (EGF) receptor expression were investigated in five basal cell epitheliomas (BCEs) and 10 primary lesions from squamous cell carcinoma (SCC) of the skin, using light microscopic autoradiography with [125I]EGF. All of the BCEs were clinically the pigmented type and histologically the solid type. All of them showed an EGF binding level similar to that of the basal and suprabasal layers of the normal epidermis. The SCCs included one case of Bowen's disease (SCCin situ), five of the well differentiated type, three of the moderately differentiated type and one of the poorly differentiated type. Eight of the 10 SCCs showed an EGF binding level similar to that of the normal epidermal basal and suprabasal cell layers. One of the two remaining SCCs, a moderately differentiated type, showed highly increased EGF binding. The other one, a poorly differentiated type, showed very little EGF binding in a large region consisting of poorly differentiated cells, although a small area composed of more differentiated, nest‐forming cells had an EGF binding level similar to that of the basal and suprabasal cells. Metastasis was found in three of these 10 SCCs after surgery. Two of the three SCCs with metastasis showed increased or decreased EGF binding levels in primary lesions as described above; in contrast, primary lesions of the seven SCCs without metastasis had EGF binding levels similar to those of the normal epidermal basal and suprabasal cells. Abnormally increased or decreased EGF binding level in SCC of the skin may be indicative of a poor prognosis, although it is necessary to examine more SCCs to confirm this assumptio

AbstractLeprosy is a rare entity in Japan, but remains quite common in developing countries. We report two sisters with leprosy from Brazil but currently working in Japan who presented to our clinic. The younger sister was infected with the BB type with HLA‐A2, A24, B51, Bw52, DR2, DRw8, DRw52, DQwl, and DQw3. The elder sister had the TT type with HLA‐A2, A31, B51, Cw3, DRw6, DRw8, DRw52, DQwl, and DQw3.Immunohistochemical findings revealed that CD4+, 4B4+helper/inducer T‐cells were dominant in the granulomas of both cases. AbstractWe report the case of a Japanese female infant with Chédiak‐Higashi syndrome born to consanguineous parents. At birth she had fair skin but, when she was three months old, marked hyperpigmentation of the sun‐exposed skin areas developed. AbstractThe clinical and histopathological findings in a 25‐year‐old Japanese male patient who suffered from chronic graft‐versus‐host‐disease (GVHD) with follicular involvement are described. The patient had been diagnosed as aplastic anemia and underwent an allogeneic bone marrow transplant (BMT). In the eighth month thereafter, pruritic follicular red papules developed over his trunk and extremities. A biopsy specimen revealed histological exocytosis of lymphocytes into the hair follicles and basal‐cell vacuolization of the follicular epithelium. Analysis of T‐lymphocyte subpopulations in the dermis revealed a predominance of CD4 positive cells.
