Clients entering therapy with indecision regarding their couple relationship can present a difficult clinical situation for the therapist. These clients vacillate between staying in and leaving the relationship and many resist moves towards a resolution. It is proposed that such indecisive clients fall into two distinct categories, namely, ambivalence and dilemma, with each having a different therapeutic focus. The aim of this paper is to outline a working model for therapists to use with clients experiencing ambivalence in their relationship. A case study based on a composite of actual cases is used to demonstrate the applicability of the model.

In all intimate relationships where difficulties are being experienced, one or both parties can feel ambivalent. Some individuals and couples resolve their ambivalence, discover lost aspects of themselves and new ways of relating, while others either remain together regardless of a satisfactory resolution, or decide to separate.In this paper I will address the nature of ambivalence and its influence and effect on individuals and couples. Understanding and working with ambivalence requires us as therapists to consider both the emotional patterns in relationships and the influence of early childhood experiences. 摘要:
An historical and conceptual overview of psychiatric rehabilitation services is presented as a challenge to the appropriateness of traditional community based psychiatric services which isolate people from the community. The position is taken that the goal of psychiatric rehabilitation services should be to link people to normal community services and support systems. The problems which can arise from treating people in a separate psychiatric system are illustrated by tracing the development of a supported employment program through periods of high employment and high unemployment.

Therapeutic Alliance is a construct which may have value for a family therapy team and influence its day to day practice. This paper discusses whether family therapeutic alliance can be observed, described and measured, and gives an account of the development of the Family Therapeutic Alliance Scale (FTAS) by a brief strategic family therapy team. Our focus is on the use of statistical technique as a means of clarifying the team's construct of therapeutic alliance, informing the team process without being the final arbiter in development of the scale. The second stage outlines the investigation of the usefulness of the construct with a wider group of family therapists. In individual psychotherapies measures of therapeutic alliance have been shown to predict outcome. Our early work suggests this is also true for family therapies and the FTAS may be a predictor for outcome in family work.

摘要:
Children with disabilities experience a significantly increased risk of abuse. This paper explores the cultural context that values body perfection and tolerates violence, so that abuse of children with disabilities, in response to normal frustrations and pressures, becomes a legitimate response. These pressures are analysed as to how they affect parents and siblings. The medical model as it affects hospital organisation and the health care system and rehabilitation is analysed. The objectification of parts of the disabled child is discussed in terms of its effect on lowering the natural modesty boundaries of the child. The child with disabilities as it relates to the family is also a creator of frustration and rage. The structural issues as they apply to the external environment are presented. The paper proposes a new understanding of the child with disabilities within the family, and within the external environment, most particularly the medical system.
