In a study of 42 healthy, middle‐aged men with high risk of coronary heart disease (CHD), especially on the basis of hyperlipoproteinaemia, a highly significant correlation was found between the euglobulin clot lysis time (ECLT) after venous stasis and serum triglyceride concentration (p<0.01), whereas no such correlation was found between ECLT and serum cholesterol. In accordance with these findings, delayed clot lysis was mostly associated with types IIB and IV hyperlipoproteinaemia. Twenty‐two of the participants had been defined as excellent diet responders on the basis of diet recording and serum lipid lowering, whereas the other 20 were controls. Following dietary changes with normalization of hypertriglyceridaemia, significantly more good diet responders of the intervention group had increased fibrinolytic capacity as compared with the control group (p<0.02). Thus it seems possible to normalize both the lipoprotein pattern and the fibrinolytic capacity to some extent through dietary advice, which again may contribute to less clinical events of

In a study of 42 healthy, middle‐aged men with high risk of coronary heart disease (CHD), we found a highly significant correlation between low ratio of serum testosterone to oestradiol and delayed clot lysis after venous stasis as measured with the euglobulin clot lysis time (ECLT). The upper normal limit of ECLT was set at 60 min. Half of the examined specimen, i.e. from 21 individuals, lysed as normal; 4 specimen lysed between 60 and 90 min, whereas the remaining 17 specimen did not lyse within 2 hours. Sixteen of these 17 individuals with the most defective fibrinolytic capacity belonged to the group of individuals with the lowest ratio of serum testosterone to oestradiol. The association was highly significant (p<0.001). In comparison, the correlation between serum triglyceride concentration and the ratio of serum testosterone to oestradiol was significant at the 2% level, whereas serum cholesterol and this ratio were not associated. Haemostasis parameters alone are often insufficient in distinguishing between disseminated intravascular coagulation (DIC) and liver disease. We have determined the concentration of five additional plasma proteins in 6 patients with DIC and 10 patients with liver disease. All patients had antithrombin III (AT) and normotest (NT) levels at 60 per cent or lower. Prealbumin and albumin were clearly subnormal in all patients with similar mean values in both two groups. It is suggested that this might reflect insufficient protein synthesis even in the patients with DIC. The mean fibronectin concentration was somewhat lower in DIC than in the group with liver disease. C reactive protein (CRP) and the leucocyte derived protein LI were much higher in the DIC patients, probably reflecting the underlying infection.

When symptoms of deep vein thrombosis is confirmed by venography (established DVT), the125I‐fibrinogen uptake test (FUT) will discriminate between thrombi which incorporate the isotope (FUT+) and those who do not (FUT−). These are probably forming and non‐forming thrombi, respectively.The aim of the present studywas to compare the occurrence of fibrinaemia, using the ethanol gelation test (EGT), in patients with FUT+and FUT−thrombi. Fibrin(ogen) degradation products (FDP) were also analysed.The study included 64 medical patients with suspected DVT, confirmed by venography in 43 and precluded in 21 patients. Isotope studies included 12 patients with FUT+and 13 with FUT−thrombi.Significant concordance between hemostatic parameters and venography was found only for FDP. The sensitivity of FDP on admission was 53%, but also patients without DVT had increased values (5%, P<0,001).Among patients with documented DVT, a positive EGT was specific for those with FUT+thrombi which incorporated the isotope in at least 4 recording positions, whereas the sensitivity on admission was 89% (P<0,005). A positive EGT was not associated with other variables, including disorders other than DVT. The heparin precipitable fraction of plasma consists mainly of fibrinogen and fibronectin. In our work with HPF in patients with acute myocardial infarction we observed variations in the amount of precipitate that could not be accounted for by corresponding variations in the plasma content of fibrinogen and fibronectin. To see whether the presence/absence of fibrin monomers in plasma could be of importance for this variation in HPF, we have:1 added fibrin des‐AA monomers to normal and patient plasmas2 incubated normal and patient plasmas with insolubilized reptilase until a strongly positive ethanol gelation test could be demonstrated3 examined a potential correlation between a positive ethanol gelation test and HPF in a patient material.From the results of the above investigations, we could conclude that the presence of fibrin monomers (presence of soluble fibrin) is of no importance for the variations observed in the HPF‐p

Urokinase‐induced fibrinogenolysis was monitored with thrombin clotting time, and inhibited by adding Soybean Trypsin Inhibitor (STI). Depending on the concentration of STI (dialysed or non‐dialysed), the thrombin clotting time was alwaysimmediatelyprolonged upon the addition of STI. At a concentration of 0.2–0.5 mg/ml (final conc.) no progressive prolongation of the thrombin clotting time occurred, indicating inhibited fibrinogenolysis.By monitoring a mixture of purified human fibrinogen, thrombin and various amounts of STI, with quantitative N‐terminal analysis, the thrombin clotting time prolonging effect of STI was further examined. N‐terminal glycine increase, reflecting fibrin formation rate by thrombin, was unaffected by STI, whereas visible gelation occurred more slowly in the presence of STI.Thus, STI (or a non‐dialysable contaminant) acts as a fibrin polymerization inhibitor, and not as an anti‐thrombin, even at concentrations regularly used to inhibit fibrinogenoly

Due to more precise readability of the visual gel point, citrated pooled plasma is a better substrate than is purified fibrinogen for thrombin activity assays. Since a lyophilized plasma would be more practical in this context, we have studied whether citrated plasma can be lyophilized and reconstituted without jeopardizing its properties as a thrombin‐assay substrate. While undialysed, lyophilized/reconstituted plasma proved unsuccessful due to a considerable pH shift, the clotting properties of pooled human citrated plasma could be fully preserved if the plasma prior to freeze‐drying/reconstitution was dialysed against a citrate‐NaCl‐diemal buffer pH 7.4 Such plasma could be kept at room temperature for at least 6 weeks or at –20 °C for 18 months and could, subsequent to reconstitution, demonstrate thrombin substrate properties identical to those of freshly prepa

Gel filtration of plasma samples containing I125fibrinogen and either I131des‐AA soluble fibrin or I131des‐AABB soluble fibrin, were performed on Sepharose 6B C1 columns at + 37 °C, equilibrated and developed with prothrombin‐depleted plasma, either recalcified or containing EDTA. With EDTA, des‐AA fibrin eluted in a homogenous, monomeric form, while des‐AABB fibrin had an elution profile indicating high molecular weight derivatives. In the presence of calcium ions, both des‐AA and des‐AABB soluble fibrin eluted mainly as high molecular weight derivatives. It could not, however, be decided whether the high molecular weight derivatives consisted of fibrin molecules alone, or of co‐polymers of fibrin‐fibrinogen. Fibrinogen fibrin complexes exist in “Ca+ +‐plasma” (prothrombin‐depleted, Ca+ +boosted, 2.5 mmol/l) adsorbed with Bentonite and enriched with I‐131‐fibrinogen (1.5 mg/ml) and I‐125‐fibrin monomer Des AA (30μg/ml) at 37 °C. Gel chromatography, in the presence of urea/EDTA, of plasma containing such complexes, which had been enzyme‐linked through the action of 2.5 u/ml plasma XIIIa, demonstrated that fibrin Des AA monomers largely tended to oligomerize (80 per cent of the molecules eluting in front of the 340.000 m.w. species), whereas fibrinogen tended to dimerize and to a smaller extent oligomerize. Plasma (citrated) from normal single donors, plasma treated with plasmin, and plasma from two patients during different stages of disease (one pneumonia, one myocardial infarction) were subjected to isofocusing (1st dimension) in agarose (IsoGel, Marine Colloids) (1 mm, pH 3.5–9.5, Ampholine LKB) using a water‐cooled flat‐bed equipment (Multiphore LKB). Subsequent to isofocusing immuno‐profiles were established by electrophoresing focused species into fibrinogen antibody‐containing agarose (2nd dimension), and developing the plates with Coomassie Brilliant Blue. Normal plasma always demonstrated a broad, partly peaked (2–3 peaks, pI 5.5, 5.3, 5.1) iso‐immuno‐profile, suggesting normal plasma fibrinogen to consist of several charged species. Plasminized normal plasma, and patient plasma (in the absence of significant amounts of serum‐FDP) regularly demonstrated evolving iso‐immuno‐patterns, suggesting the present two‐dimensional iso‐immuno‐profiling of plasma fibrinogen to be a valuable accretion to other methods used to demonstr
