A case of benign idiopathic monoclonal gammapathy with cold sensitivity due to Donath‐Landsteiner antibody is reported. At the age of 42, the patient noticed a tendency to urticarial wheals after exposure to cold. Two years later, he began to have typical attacks of cold haemoglobinuria. At the age of 46, a haemolysin of the Donath‐Landsteiner type and increased amounts of IgG were disclosed. As the eluted M‐protein gave a positive Donath‐Landsteiner reaction, this was probably identical with the haemolysin. That the gammapathy was of the benign type was supported by the fact that no rise was seen in the M‐protein level or plasma‐cell count at a follow‐up examination 6 1/2 years after the onset of the cold sensitivity. In vivo microscopic observations of splenic tissue transplanted into modified Algire back chambers in mice revealed increased vascularity of the transplant following injections of various doses of erythropoietin. Concomitant histologic and peripheral blood studies suggested that this was vasoproliferation that occurred independent of the process of erythropoiesis.

Erythrocyte sequestration was studied in 24 patients with chronic lymphocytic leukaemia (CLL) by infusion of autologous51Cr‐labelled erythrocytes followed by surface measurements at regular intervals over the spleen, liver, and heart.To obtain a comparable expression of the accumulation of activity in spleens of different sizes, the splenic surface activities were corrected. The correction factors were determined using fluid‐filled balloons (simulating spleens) with the same amount of activity.In 19 patients with CLL the splenic erythrocyte pool increased with increasing spleen weight (500–4, 300 g). Moreover, the erythrocyte sequestration in the spleen increased with increasing pool. The maintenance of a large erythrocyte pool makes increased demands upon the erythropoiesis, which is perhaps already insufficient, and an increased splenic erythrocyte sequestration contributes to reducing the number of circulating erythrocytes. Splenectomy is invariably followed by increasing haemoglobin level and an average 30% increase of erythrocyte survival (22–48%).In cases of CLL plus splenomegaly the indications for splenectomy appear to be fairly simple, but may be supported by the demonstration of a large splenic erythrocyte pool and a large splenic erythrocyte destruction.The studies support the likelihood of lymphocytic infiltration in the liver (increased accumulation of activity), a finding which seems of prognostic signi

The iron absorption from ferrous sulphate tablets with and without succinic acid was studied during the first month of treatment of iron deficiency anaemia. The studies were made in volunteers made anaemic and iron depleted by repeated phlebotomies. The two kinds of tablets labelled with different radioiron isotopes were given on alternate days and the absorption was measured by a combined use of a whole body counter and a double radioiron technique. In 4 subjects the absorption was studied during the first 20 days of treatment and the average absorption was 20.9 and 13.5% of the 222 mg of iron administered daily when given with and without succinic acid respectively.In 4 other subjects the absorption was studied during two periods — day 1—10 and day 21–30. In the first period similar results were found as in the study day 1–20. The absorption day 21–30, however, was considerably lower. There was still, however, an absorption promoting effect of succinic acid of about the same magnitude. A calculation showed that the period of treatment needed to achieve the same total amount of iron absorbed can be expected to be much longer with ferrous sulphate alone due to the marked influence of the absorption promotor during the first 3 weeks of treatment when the absorption is markedly increased. The fact that in all periods studied more iron was absorbed than was used for haemoglobin formation thus indicates that iron stores start to be formed early and before the haemoglobin mass is restituted under the condition

RNA was isolated from lymphocytes of the peripheral blood of patients with chronic lymphocytic leukaemia under conditions previously shown (Cline 1966a) to result in undegraded RNA. At the same time, responsiveness of the lymphocytes to PHA was measured by determination of the incorporation of3H‐thymidine in culture. The isolated RNA was characterized by sucrose density gradient centrifugation, gel chromatography on 2% agarose, electrophoresis on mixed polyacrylamide‐agarose gels, and fractionation with 3 M NaCl. RNA isolated from leukaemic lymphocytes unresponsive to phytohaemagglutinin showed a pattern identical to that obtained from normal lymphocytes and liver. RNA preparations from leukaemic lymphocytes responsive to phytohaemagglutinin gave a diverging pattern, characterized by the presence of large amounts of relatively low molecular or polydisperse RNA. Chronic lymphocytic leukaemia lymphocytes not responsive to phytohaemagglutinin gave, in analogy to normal lymphocytes and liver, 28s + 18s rRNA in amounts corresponding to 75 to 85% of total extractable RNA, whereas leukaemic lymphocytes responsive to phytohaemagglutinin yielded only 40–60% of the total extractable RNA in the form of 28s + 18

In a case of hereditary nonspherocytic haemolytic anaemia an atypical erythrocyte pyruvate kinase was found.The pyruvate kinase was characterized as atypical on the basis of the following biochemical parameters which differed from those of the normal enzyme: Michaelis' constant, thermostability, pH dependency, urea inhibition, and electrophoretic mobility.The results indicate that there may be a structural difference between the atypical and normal pyruvate kinase.

Rough endoplasmic reticulum (RER) and free ribosomes were rarely present in blood platelets of normal rabbits or human subjects. After thrombin infusion or bleeding, rabbits became thrombocytopenic and the relative number of platelets containing RER and polyribosomes was considerably increased. The presence of RER in isolated platelets provides a morphological basis for protein synthesis. Guinea‐pigs were investigated 2, 4 and 8 days after intrathymic or intraperitoneal labelling with tritiated thymidine. By the comparison between the content of lymphocytes in afferent and efferent thymic blood, an export of heavily labelled lymphocytes from the thymus was demonstrated in the intrathymically labelled animals after 2 and 4 days but not after 8 days. By use of liquid scintillation tritiated thymidine was localized to the spleen in a significantly higher amount 2 days after intrathymic than after intraperitoneal labelling.By autoradiography heavily labelled lymphocytes derived from the thymus were found in the spleen after 2 days and also to a lesser extent after 4 and 8 days, and in the mesenteric lymph nodes at all times after labelling. In the thoracic duct lymph the frequency of heavily labelled lymphocytes was the same as in the blood after intrathymic labelling, but less than in the blood after intraperitoneal labelling. A method is described for testing the release of ADP in a platelet suspension induced by kaolin, latex, other particles and distilled water. A refined technique was developed to determine ADP quantitatively by its ability to aggregate platelets. Total adenine nucleotide release was also determined spectrophotometrically. Both methods have advantages and disadvantages.In a platelet suspension the release induced by kaolin occurred in the absence of cofactors and even in the presence of EDTA. Maximal release occurred at 37° C. In the absence of other inducers, calcium alone did not produce release, but it potentiated the release by other inducers, which was most conspicious at low temperatures. The release reaction of this type was not mediated by thrombin and was not inhibited by acetylsalicylic acid. Adhesion of platelets to the particles evidently preceded the release.Patients were investigated and a normal or increased release was found in Glanzmann's thrombasthenia and von Willebrand's disease.
