Recent experimental studies have demonstrated erythrophagocytosis in the rat bone marrow after transfusion of heat‐denatured erythrocytes. The present investigation deals with bone marrow erythrophagocytosis following removal of the spleen, which is the main erythroclastic organ in the rat. By light microscopy of the bone marrow 8 weeks after splenectomy erythrophagic activity has been demonstrated in the nursing cells of the erythroblastic islands. As no significantly increased red cell destruction could be found in other parts of the reticuloendothelial system, the bone marrow is regarded as the main erythroclastic site in the splenectomized rat. A minor bone marrow erythrophagic activity can also be demonstrated in the normal rat. Platelet kinetics, volume of megakaryocytes perμl sternal bone marrow, and the volume of the spleen have been quantitated in 20 cases of untreated polycythaemia vera (PV). There was a very good relationship between platelet production rate and megakaryocyte volume/μl bone marrow. The results suggest that megakaryopoiesis in PV represents mainly an effective platelet production. The megakaryocyte volume/μl bone marrow was significantly related to the body platelet mass (p0.05) suggesting that enlargement of the spleen as such influences platelet survival. Hamster platelets separated from plasma by gel filtration (GFP) have been subjected to further studies:(1) The intraplatelet organelles of the GFP as examined by the electron microscope have an appearance similar to those of platelets in plasma and of platelets washed by centrifugation (CP).(2) The frequency of pseudopods exhibited by the GFP and platelets in PRP was significantly less than in the preparations of CP. However, GFP had more platelets with three or more pseudopods than did platelet left undisturbed in citrated plasma.(3) Electron microscopy of the GFP during ADP‐induced aggregation showed these platelets to behave morphologically as PRP.(4) The Ca++and Mg++requirements of GFP in ADP‐induced aggregation were similar to those reported earlier by other workers.(5) Addition of fibrinogen to GFP was not required for ADP‐induced aggregation.(6) Albumin stabilized the GFP so that the platelets remained responsive to ADP for at least 3 h after gel filtr

Erythrocyte pooling and sequestration in the spleen were studied in 73 patients with splenomegaly due to various haematological disorders using51Cr‐labelled autologous erythrocytes and surface counting. Arbitrary values for the accumulation of activity in spleens of different sizes were obtained by correcting measured splenic surface activities (correction factors have been experimentally estimated). Absolute values for the splenic content of51Cr and125I were obtained by comparative surface activity measurements over the enlarged spleen and a model of the spleen charged with a known amount of activity. In this way the splenic erythrocyte and plasma volume may be estimated.In cases of pronounced splenomegaly without complicating immunohaemolysis the splenic erythrocyte pool (splenic erythrocyte volume/total erythrocyte volume) increases with increasing splenic weight, apparently independently of the underlying haematological disorder, the splenic erythrocyte content varying only with body haematocrit. Hyperhaemolysis is a regular finding in enlarged spleens. The splenic erythrocyte sequestration rate increases with increasing splenic erythrocyte pool and is much higher in myeloproliferative than in lymphoproliferative disorders. The total plasma volume is expanded in splenomegalic patients, but the splenic plasma volume constitutes only a minor part of this increment.The anaemia of splenomegalic patients is a consequence of splenic erythrocyte concentration and hypersequestration combined with expansion of the plasma volume. The erythrocyte sequestration in the spleen and liver was investigated in 25 cases of immunohaemolysis by surface counting after infusion of51Cr‐labelled autologous erythrocytes. The erythrocyte sequestration pattern was found to depend upon the treatment with prednisone given at the time of the investigation and furthermore upon the total haemolytic intensity.Increased sequestration of erythrocytes in the liver is always present in cases of severe hyperhaemolysis. After prednisone treatment, hypersequestration of erythrocytes in the spleen predominates. In the present study increased sequestration in the liver was even demonstrated in about 50 % of the cases in which incomplete non‐complement‐fixing antibodies were found.In the majority of cases prednisone therapy gave only short‐lasting remission. Frequently, it is difficult to reduce the dose of prednisone because of increasing haemolytic intensity.Complete, long‐lasting remission seems to follow splenectomy in cases characterized by a splenic sequestration pattern during periods without prednisone therapy, but if hepatic hypersequestration has been demonstrated the remissions are not complete. A patient's clinical course with Phi‐positive chronic myelogenous leukaemia is described. Blastic transformation was associated with a double Ph1chromosome and megaloblastosis. During remission, the megaloblastosis disappeared and the cytogenetic pattern reverted to a single Ph1chromosome. The subcellular localization of fibrinogen in platelets washed in an EDTA‐buffer medium has been studied with special reference to the granule‐bound fibrinogen. After sucrose density gradient centrifugation of platelet homogenates nine fractions of different densities were separated and studied for their relative contents of total protein, fibrinogen, β‐glucuronidase and acid p‐nitrophenyl phosphatase. Fibrinogen was found both in the soluble phase and in the particulate fractions. The distribution of fibrinogen calculated relative to lactic dehydrogenase activity showed that granule‐bound fibrinogen did not represent cytoplasmic contamination. The results further indicate a preferential association of granule‐bound fibrinogen with low‐density granules. The same was true for β‐glucuronidase and acid phosphatase. Two sisters had a congenital hypochromic anaemia in spite of hyperferraemia with a fully saturated serum transferrin, and in spite of heavy iron deposits in the liver. Bone marrow iron was reduced. The anaemia was markedly increased during pregnancy, when each of the sisters needed 8 blood transfusions. Erythrokinetic studies were compatible with hypoplastic anaemia.
