An impaired phagocytic capacity of the neutrophilic leucocytes was found in 4 elderly patients with anaemia, thrombocytopenia and normal white blood counts. Leucocyte alkaline phosphatase activity was subnormal, and in 3 of the patients the morphology of the granulocytes was abnormal with a low degree of nuclear segmentation and an exaggerated clumping of the chromatin. In all patients the bone marrow was hypercellular with an increased myeloid/erythroid ratio. Besides anaemia and an increased tendency to bleeding, a high susceptibility to infections was observed. There were no initial signs of leukaemia, nor was the syndrome compatible with any other conventional haematologic diagnosis. In one patient myeloblastic leukaemia was diagnosed after 8 months of observation and in another signs of chronic myeloid leukaemia developed after 5 months. The syndrome may therefore be preleukaemic. Attempts to improve the condition with steroids, cytostatic drugs, splenectomy, and androgens were unsuccessful. Combination chemotherapy with four different drugs — cyclophosphamide, vinblastine, procarbazine and prednisone — was used in 159 patients with malignant lymphoma. The drugs were given in 2‐week cycles interrupted by a 4‐week rest period. Complete remissions were obtained initially in 59 % of the patients with Hodgkin's disease, in 42 % of the patients with reticulum cell sarcoma, and 61 % of the lymphosarcoma cases. Those who had been treated previously with both radiotherapy and chemotherapy did not respond so well as those previously untreated or those who had received either radiotherapy or chemotherapy alone. The 3‐year survival rates were as follows: Hodgkin's disease 58 %, reticulum cell sarcoma 32%, and lymphosarcoma none. The major toxic effect was leucopenia which was most pronounced in patients previously treated with chemotherapy and/or extensive radiotherapy. Calusterone, a new anabolic, weakly androgenic testosterone derivative, found highly effective in women with advanced breast cancer, was studied for its peroral effect on white blood cell count (WBC). A significant rise in WBC occurred during treatment of 80 women, as compared both with a contol group of 21 untreated breast cancer patients and a similar group of 27 patients receiving Δ1‐estololactone 1, 000 mg daily. Δ1‐estololactone produced no significant change in WBC. The combination of 5‐fluorouracil 500 mg i.v. once a week plus Δ1‐estololactone 1, 000 mg a day by mouth in 28 patients produced significant mild leucopenia. While it has been suggested that orally transmitted erythropoietin can stimulate erythropoiesis, in vitro studies have shown that this glycoprotein is inactivated by incubation with various proteolytic enzymes. This study, utilizing the incorporation of59Fe in the plethoric mouse, suggests that orally administered erythropoietin is ineffective in stimulating erythropoiesis.

On the assumption that, under certain conditions, acid phosphatase may serve as an enzymatic marker of the lipoprotein complex which shows the thromboplastic activity of platelets, the changes in the platelet factor 3 (PF 3) activity after induction by bovine fibrinogen were studied simultaneously with those in acid phosphatases (APh). It has been found that bovine fibrinogen induces not only the aggregation of human platelets and the ADP release, but also the availability of PF 3 and APh. The effect of bovine fibrinogen depends to a certain degree upon its quantity, and the presence of extracellular calcium; it is potentiated by ADP. The inhibitors of the aggregation by bovine fibrinogen inhibit proportionately even the availability of PF 3 and APh. It seems that the mechanisms, by means of which PF 3 and APh become available, depends not only on the release of ADP from the platelets, but above all on the intensity of aggregation.

Several reports show the occurrence of a haemorrhagic diathesis which accompanies haemolytic transfusion reactions (Friesen et al 1952, Binder et al 1959, Seldon 1949, Sack&Nefa 1970). Various haemostatic disturbances have been incriminated as causes of that diathesis, such as thrombocytopenia (Hardaway et al 1954), hypofibrinogenaemia (Conley 1952) and circulating anticoagulants (Muirhead 1951). Lately, disseminated intravascular clotting (DIC) has been implicated as the main cause of bleeding in such patients (McKay 1965, Rock et al 1969, Sack&Nefa 1970). We had the opportunity to study a patient with severe haemolytic transfusion reaction due to major blood group incompatible transfusion, causing life‐threatening bleeding. A macromolecular serum component, platelet stain preventing factor (PSPF), possessing the capacity to prevent staining of platelets, has been observed in the serum of an apparently healthy person. The factor is active only when EDTA is present, and it has reacted with all platelet samples tested until now.Thus far, it has not been possible to demonstrate that the factor resides in any of the known serum proteins.

The effect of a single intravenous flash injection of cytosine‐arabinoside (ARA‐C) (2.0–2.5 mg/kg BW) on the cell cycle of leukaemic myeloblasts in vivo was studied in 7 patients with acute myeloid leukaemia.It was found that the3H‐thymidine labelling index, the mitotic index and the mean grain count of the cells showed a transitory decrease but within the observation period (36 h) returned to pretreatment values. Microspectrophotometric measurements of single cell DNA content suggested an undisturbed flux of cells into DNA synthesis from G1phase. The findings are in accordance with the assumption that ARA‐C arrests cells in DNA synthesis without directly interfering with the progression of cells through G1, G2, or mitosis. The authors describe an unusual case of refractory anaemia in whom plasma membranes of several erythroblasts are connected by junctional surface specializations which resemble septate junctions. Mechanisms and significance of the junctions in this case are unknown.
