ABSTRACTHaematological variables were studied in a randomised population sample of 372 38‐year‐old women resident in Gothenburg, Sweden during the period 1968–69. The participation rate was 91.4 %. The study was performed to evaluate the laboratory diagnosis of iron deficiency. The haematological investigation included determination of haemoglobin concentration, erythro‐cyte and reticulocyte counts, serum iron concentration, total iron binding capacity, packed cell volume, sternal bone marrow aspiration with estimation of stainable iron in the reticuloendothelial cells, sideroblast count, iron absorption test with 59Fein a whole‐body counter and determination of menstrual blood loss. Serum ferritin concentrations were later determined on samples which had been frozen for 10 years. Eighty‐nine women with no or only traces of reticuloendothelial iron in the bone marrow completed a double‐blind supplementation trial, in which iron and placebo therapy were compared.Determination of stainable iron in the reticuloendothelial cells of the bone marrow was used as a direct method for the evaluation of iron stores. In 31.5 % of the women there was no reticulendothelial iron and in 14.3 % there were only traces. Of the indirect methods compared with the bone marrow method the serum ferritin concentration and iron absorption gave the highest correlations. A high correlation was also observed between menstrual blood loss and the content of reticuloendothelial iron. An arbitrarily chosen scale for the estimation of the amount of stainable reticuloendothelial iron in the bone marrow was used for these correlation analyses.The differences in mean corpuscular haemoglobin, transferrin saturation, sideroblast count, iron absorption, and menstrual blood loss were all highly statistically significant when the series of women with no or only traces of reticuloendothelial iron in the bone marrow were compared to those with clearly visible or abundant amounts of stainable iron. There were no significant differences concerning these variables between women who had no traces of stainable iron and those who had only traces of stain‐able iron. Increased iron absorption was found in women with a lack of stainable iron in the reticuloendothelial cells compared to women with stainable iron, irrespective of sideroblast count.The results from the iron supplementation study indicates that the haemoglobin concentration values of many of the women, although being within the normal range, were suboptimal for the individual woman. However, we considered it not possible to ascertain which women were responders to iron therapy, as a comparison between the effects of iron and placebo could not be made for the individual subject.According to multivariate analysis, determination of serum ferritin concentration was the most valuable indirect method for the diagnosis of depleted iron stores, and we are of the opinion that serum ferritin determination may replace bone marrow examination in the diagnosis of iron deficiency in women with or without anaemia who are ot

ABSTRACTBlood haemoglobin concentration, packed cell volume, blood cell counts, serum iron concentration, total iron binding capacity, bone marrow iron content, sideroblast count, iron absorption and menstrual blood loss were studied in 372 38‐year‐old women who were representative of the general female population of that age resident in Gothenburg, Sweden. The distributions of serum iron concentration, serum total iron binding capacity, and transferrin saturation were slightly skewed, whereas the distributions of iron absorption, reticulocyte count, sideroblast count, and menstrual blood loss were markedly skewed. The other variables studied were mainly normally distributed. One‐third of the women (32 %) had no reticuloendothelial iron and another 14 % only traces of reticuloendothelial iron in smears from sternal bone marrow. Serum iron concentration and total iron binding capacity were rather high and menstrual blood loss low in women taking oral contraceptives, while factors such as country of origin, socio‐economic group, blood donation, previous oral iron therapy or recent infection seemed to have no or only a slight influence on variables reflecting the iron

ABSTRACTRegression analysis was performed on a number of haematological variables obtained from a population sample of 38‐year‐old women. The correlation coefficients were higher between the blood haemoglobin concentration on the one hand and serum iron concentration, total iron binding capacity, transferrin saturation, sideroblast count, iron absorption and menstrual blood loss on the other than between packed cell volume or erythrocyte count and the same haematological variables mentioned above. The correlation coefficients between transferrin saturation and the above haematological variables were higher than the correlation coefficients between serum iron concentration or total iron binding capacity and the same variables. The highest correlation coefficients with the above mentioned haematological variables were found for reticuloendothelial iron in the bone marrow. The prevalence of women with a lack of reticuloendothelial iron in the bone marrow was high when haemoglobin concentrations and mean corpuscular haemoglobin values were within the lowest two decentiles defined as a haemoglobin concentration below 127 g/l and a mean corpuscular haemoglobin below 31 pg/erythrocyte. ABSTRACTBone marrow smears from 38‐year‐old women participating in a population study were examined with respect to stainable iron in reticuloendothelial cells and in erythroblasts called sideroblasts. Blood haemoglobin concentration, erythrocyte mean corpuscular haemoglobin and transferrin saturation in serum were lower in women with no stainable iron in the reticuloendothelial cells and sideroblast counts below 16 % than in women with stainable iron clearly visible and sideroblast counts equal to or above 16 %. Iron absorption was higher and menstrual blood loss heavier in women with no iron than in women with stainable iron, irrespective of the number of sideroblasts. ABSTRACTSerum ferritin concentration was determined in 38‐year‐old women representative of the general female population of the same age resident in Gothenburg, Sweden. The assays were carried out on serum samples which had been frozen (‐22° C to ‐28° C) for ten years. The values were related to haematological variables which had been examined at the time of the initial study. The distribution of serum ferritin values was markedly positively skewed. Woman with a serum ferritin concentration within the lowest three decentiles had the heaviest menstrual blood loss, no or only traces of reticuloendothelial iron in sternal bone marrow and high iron absorption of a tracer dose of labelled iron. High correlation coefficients were obtained when correlations were calculated between serum ferritin and transferrin saturation, sideroblast count in bone marrow and iron absorption. The correlation coefficients were usually higher between serum ferritin concentration and the other haematological variables than the correlations between the individual haematological variables. There was good agreement between the prevalence of reticuloendothelial iron in bone marrow and serum ferritin concentration. We conclude that bone marrow examination may be replaced by determination of serum ferritin concentration when screening for iron deficiency in otherwise healthy subjects. ABSTRACTThe response to iron therapy in 38‐year‐old women was studied in an iron supplementation trial. Women with no or only traces of reticuloendothelial iron in bone marrow samples received in a double‐blind fashion tablets containing 37 mg ferrous iron or placebo three times daily. The women in the iron supplementation group increased significantly more their haemoglobin concentration and packed cell volume than the women in the placebo group. The average increase in haemoglobin concentration was 7.5 g/l. Approximately 75 % of the women in the iron supplementation group increased their haemoglobin concentration in spite of the fact that only 20 % had pretreat‐ment concentrations below 120 g/l. We found no differences in change of haemoglobin concentration, packed cell volume, serum iron concentration, total iron binding capacity or transferrin saturation between subjects with no reticuloendothelial iron and women with only traces of reticuloendothelial iron.
