The periodic acid‐thiocarbohydrazide‐silver proteinate (PA‐TCH‐SP) technique, which has the same cytochemical significance as PAS staining of light microscopy, was undertaken to reveal the distribution of complex carbohydrates with vicinal glycols in human lymphocytes in several conditions at the ultra‐structural level. The PA‐TCH‐SP method stained the clustered and the scattered cytoplasmic granules, Golgi apparatus and glycogen particles of lymphocytes. In the buffy coat, the lymphocytes with the clustered cytoplasmic granules contained less glycogen particles compared with the lymphocytes without the clustered cytoplasmic granules. The majority of T‐lymphocytes separated from the venous blood possessed PA‐TCH‐SP positive clustered cytoplasmic granules, but glycogen particles were scanty or negligible, while B lymphocytes were rich with glycogen particles but had scanty clustered cytoplasmic granules. A mechanized factor X assay was tested in 2222 plasma samples of patients being treated with oral anticoagulants. The correlation coefficient between this assay and Thrombotest was 0.78. The therapeutic range for factor X amidolytic activity was 150 to 300 units/1. Amidolytic factor X activity and Thrombotest provide similar information about the state of anticoagulation within the same patient, including patients that are not well balanced. The ranges for factors II, VII, IX and X clotting activity in 57 patients on long‐term therapy (Thrombotest within the therapeutic range between 190 and 95 s) were 80–300, 70–600, 40–420 and 50–330 units/1, respectively. A method for the classification and quantification of lymphoma cells in non‐Hodgkin lymphomas is described. Lymphoma‐cell types were defined and used in differential counts of haematoxylin and eosine stained paraffin‐embedded sections.The following cell types were distinguished: small lymphoid cells (small lymphocytes and small plasma cells), medium‐sized lymphoid cells (round, cleaved and cerebriform lymphocytes, and medium‐sized plasma cells), and large lymphoid cells (lymphoblastic, round triple‐nucleolated, and pleomorphic types). The intra‐observer agreement in the classification of cell types was 43–85 %. The agreement in distinguishing small, medium‐sized, and large lymphoid cells was 85–92 %. In differential counts in contrast to conventional histologic classification all cellular profiles were classified regardless of possible tangential cutting. 92 % of the cellular profiles at different sectional planes of the same cells were consistently classified, indicating that tangential cutting of cells only influenced the differential counts to a minor degree. By the quantification of cellular composition in multiple, randomly selected areas, classified as representative at low power magnification, only small intra‐lymphoma variations were observed in contrast to marked inter‐lymphoma variations.The high reproducibility of lymphoma‐cell types in combination with the small intralymphoma variation makes the quantifiable method suitable in the histological evaluation

A 6‐year‐old girl with severe aplastic anaemia improved promptly after treatment with anti‐human lymphocyte globulin (ALG). Blood T lymphocytes were proved to have a suppressive effect on erythroid colony formation. ALG was administered intravenously at a dose of 15 mg/kg/d for 5 d. By the 14th d she showed a prompt increase in the reticulocyte count. Within the next 2 weeks slight increase of the platelet count was observed while the neutrophil count remained unchanged. The suppressive effect of T cells on erythroid colony formation weakened when the reticulocyte count exceeded 100 times 109/1. About 4 months after administration of ALG, a decreased reticulocyte count was observed along with frequent nasal bleeding. Again the T cells exhibited a suppressive effect on erythroid colony formation. The results of serial co‐culture studies revealed a close correlation between the T‐cell suppressive effect on erythroid colony formation and reticulocyte response. Glucose‐6‐phosphate dehydrogenase (G‐6‐PD) deficient erythrocytes are particularly sensitive to oxidant stress. In order to evaluate if these cells are protected against oxidant damage, we assayed the antioxidant enzymes superoxide dismutase (SOD), catalase and glutathione peroxidase (GSH‐Px) in erythrocytes of G‐6‐PD deficient (hemizygous and heterozygous) subjects. Normal levels of antioxidant enzymes were found in all subjects examined both with positive and negative histories of haemolytic crisis after fava bean or drug ingestion. 17 patients (age 15–58 years) with refractory acute leukaemia (14 AML, 3 ALL) were treated with high dose cytosine arabinoside (AraC) at a dose of 3000 mg/m2, twice daily for 6 d (13 patients with AraC alone, 4 patients with AraC and doxorubicin). 9 patients achieved complete remission (53 %) and 2 a partial remission. Although sophisticated isolation and blood banking facilities are required during the pancytopenic period, the toxicity of this treatment was acceptable. Vomiting, headache, somnolence, fever, conjunctivitis and cardiac arrhythmias were found most frequently. The unexpected pulmonary failure in 3 patients was worrisome. The duration of remissions was from 1 to 12 months.Results obtained with high dose AraC are satisfactory and hold promise for the treatment of patients with previously untreated AM

In a study of 30 patients with either Plasmodium or falciparum infection, 29 (97%) showed the presence of atypical lymphocytes in the peripheral blood film, 24 (80%) had thrombocytopenia and 17 (57 %) had a mild to moderate anaemia. Bleeding was encountered in 2 patients without confirmatory evidence of disseminated intravascular coagulation.

Examination of the blood of 40 patients with leukaemic reticuloendotheliosis (LRE) revealed a drastic reduction in monocyte numbers (mean 74/μl) when compared to a normal group of 33 (mean 442/μl). Repeated blood studies in 8 LRE patients were made over periods of 2 months to 5 years and monocytopenia was observed to be a persistent phenomenon. Similarly, tissues from 29 patients showed an overall decrease in numbers of monocytes/histiocytes when compared with those of 73 control subjects. Skin window examinations in all 11 patients so studied showed marked reduction or absence of monocyte response to inflammation. 2 exceptional cases to this trend are noted. In one, circulating monocyte levels remained normal over a 14‐month period while examinations of blood and marrow in this case showed few hairy cells. In another case, severe and persistent monocyte deficiency was noted during a 2‐year period of unsuccessful treatment. A radioimmunoassay (RIA) method for erythropoietin (Epo) was developed and validated against the polycythaemic mouse assay. The correlation was good, with a r = 0.94. Several other criteria of specificity were also filled by the RIA, which had a lower detection limit of 5 mU/ml. The mean serum‐Epo level in 6 patients with secondary polycythaemia, 50.2 + 26.2 mU/ml, was significantly higher than in a group of 11 normal subjects, 28.7 + 7.2 mU/ml (P<0.0002). However, the Epo level in 31 polycythaemia vera (PV) patients, M = 21.9 + 6.6 mU/ml, was not significantly different from normal (P = 0.006). Since previous studies with bioassay of heat‐treated and concentrated plasma samples have shown a decreased serum‐Epo level in PV, Epo levels were measured before and after heat treatment and concentration of samples from normals and polycythaemics. It was found that the levels of immunoreactive material increased after heat treatment and 40 times concentration in samples from normals and patients with secondary polycythaemias, but decreased in PV. Undetectable levels of circulating interferon were found in 35 splenectomized patients and in 14 controls. In addition, no differences in virus, purified protein derivative (PPD), and phytohaemagglutinin (PHA) induced interferon production were observed between the patients and the controls. Virus induced lymphocyte transformation tended to be reduced after splenectomy, but only the herpes simplex virus (HSV) induced transformation index was significantly reduced in splenectomized patients without residual splenic tissue (P<0.05). In contrast, the PHA lymphocyte transformation was unchanged in patients and controls. The number of natural viral antibodies in splenectomized patients did not differ from that of controls. Furthermore, there were no major differences in the magnitude of the titers of the sero‐positive cases between the various groups. Thus, splenectomy does not interfere with interferon production in vitro, and as seen from natural viral antibodies, no impairment of humoral immunity was observed.
