Three patients are described in whom platelet aggregation and/or degranulation occurred in blood collected into EDTA. These changes resulted in spurious thrombocytopenia and morphological changes similar to those observed in some thrombocytopathies. The abnormalities were dependent on the presence of EDTA and did not occur in citrate, oxalate or heparin anticoagulants. In two patients the abnormality was shown to be due to a plasma factor which was not IgG, IgM, fibrinogen or albumin. The most likely explanation is that these patients have an unidentified abnormal plasma component which, on exposure to EDTA, develops ‘anti‐platelet activity’. Although relatively uncommon a prospective study of the incidence of these phenomena indicates that they are probably more common than either platelet cold agglutinins or platelet satellitism. Biphasic collagen‐induced platelet aggregation, resembling that induced by epinephrine, was noted in platelet‐rich plasma (PRP) of 11 stroke and 1 coronary disease patients. Similar pattern of aggregation was not observed in normal PRP. The occurrence of the biphasic collagen aggregation does not appear to relate to platelet count, smoking habit, medication, or other abnormalities such as hypertension, diabetes, and elevated serum lipid levels. However, platelets of these patients were very sensitive to aggregating agents including epinephrine and adenosine diphosphate. The concentration of collagen that elicited biphasic aggregation in these platelets was too weak to aggregate platelets of normal subjects. We believe that the release threshold of these platelets is reduced to such an extent that minute amounts of collagen, which would be insufficient to induce release from normal platelets, are capable of inducing release from these platelets. The concentrations of unbound amino acids in erythrocytes and in plasma from 7 normal individuals, 11 patients with various types of aregeneratory anaemia, and 4 patients with hereditary haemolytic anaemias were determined on a Technicon Amino Acid Analyzer (Perry et al 1970).Most amino acids were normally found in higher concentrations in plasma than intracellularly. Cystine, methionine and tryptophan were almost exclusively present in plasma. Aspartic acid, however, was mainly found in erythrocytes, and glutathione only in erythrocytes. Glutamic acid and ornithine were more concentrated in the cells, while glycine and asparagine showed approximately the same concentrations in erythrocytes as in plasma.In the patients, plasma amino acids showed little deviations from normal, but in the erythrocytes there were striking changes. Erythrocyte glutamic acid concentrations were moderately to markedly elevated in all patients studied, and glycine concentrations in 13 out of 15 patients. In addition, the following amino acids were increased intracellularly in more than one patient: glutamine (8 patients), serine (7), asparagine (5), threonine (4), taurine (3), alanine (2), valine (2), ornithine (2), lysine (2), citrulline (2). Aspartic acid was decreased in erythrocytes from 4 patients with aregeneratory and 1 with haemolytic anaemia.

Chromosome studies of cells from bone marrow and spleen were performed in a patient with agnogenic myeloid metaplasia (MM) developing blast crisis after a chronic phase lasting for 6.5 years. The proportions of blast cells were roughly the same in bone marrow and spleen. In the bone marrow 43% of the metaphases were abnormal with a marker chromosome whereas all spleen metaphases were normal. The results indicate that chromosomal changes associated with blast transformation in MM may occur in the bone marrow prior to such changes in the spleen and support the concept that bone marrow and spleen may constitute relatively separate pools of haemopoietic tissue in chronic myeloproliferative diseases.

By infusion of large amounts of human fibrinogen into rabbits, the ethanol gelation test turned positive at various plasma concentrations of fibrinogen, depending on the fibrinogen preparation used, and on individual differences among the animals. After incubation of the fibrinogen preparation in the cold the precipitated material gave a positive ethanol gelation test at lower fibrinogen concentration whereas the ethanol gelation test remained negative in the supernatant at a higher fibrinogen level than that of the starting material.It is concluded that the finding of a positive ethanol gelation test is not due to the fibrinogen concentration per se. Most likely the test becomes positive due to the presence of soluble fibrin in the commercial preparations.

Studies were performed to analyse the inhibitory effect of a myeloma globulin (IgG) on fibrin formation.This inhibitory activity was very intense and caused a severe bleeding disorder which proved fatal.The isolated myeloma globulin inhibited all three stages of fibrin formation: the proteolytic action of thrombin on fibrinogen, the aggregation of fibrin monomers and the stabilization of fibrin by cross‐linkages in the γ and α chains. Purified factor XIII, even in excess, previously activated by the addition of thrombin and calcium, did not correct this defect in cross linking. Bone marrow progenitor cells from anaemic W/Wvmice were compared with normal +/+ cells utilizing the spleen colony, the agar colony and the diffusion chamber techniques. Spleen colony formation from W/Wvcells was markedly defective, and more so for erythroid than for granuloid colonies. The progenitor cell concentration was apparently normal as measured by the two other techniques. The concentration of circulating progenitor cells also seemed to be normal. On the other hand, the cell formation per progenitor cell was subnormal in all three assay systems. The initial proliferative response of W/Wvspleen colony‐formers and agar colony‐formers to short‐term diffusion chamber culturing was apparently normal. The incorporation of3H‐thymidine, related to the number of proliferative granulocytes present in the chambers, also seemed to be normal.The results indicate that the W/Wvdefect is not limited to the multipotent stem cells. In vitro studies have been done on haematopoietic cells from a patient with cyclic neutropenia characterized by severe depression of blood neutrophil levels every 21 days. Serial blood counts reveal periodic fluctuations in neutrophils, monocytes and reticulocytes. Agar culture of marrow cells shows normal concentration of colony forming cells. The percentage of colony forming cells in S phase is highly increased during profound neutropenia and normal during the recovery phase relating the granulocyte production to the peripheral neutrophil level. Studies of ingestion rate, bactericidal activity, lactate production and glucose oxidation during phagocytosis in isolated granulocytes show normal results. Also the ingestion rate in isolated monocytes is normal. Serial karyotype analyses of marrow cells during the neutrophil cycle display a normal pattern. Serum myeloperoxidase levels vary inversely with the peripheral neutrophil count indicating increased granulopoietic activity during profound neutropenia, which might be associated with non effective granulopoiesis during profound neutropenia, leading to a lack of granulocyte reserves in the marrow.
