By microcalorimetry the metabolic activity of malignant cells from 21 patients with non‐Hodgkin lymphoma was monitored. Heat production, a measure of the metabolic activity of cells, showed positive correlation with the clinical‐course of the disease. The heat produced by tumor cells from patients with progressive disease was 5.5 pW/cell, while that from patients responding to treatment was 3.1 pW/cell. The difference between the 2 groups is significant (P<0.05). A higher heat production was obtained in tumor cells from 7 out of 10 patients who were unresponsive to the therapy when compared with cells from 11 patients who had become asymptomatic as a result of the same therapy. Also heat production from blood lymphocytes was found to be higher (P<0.005) in the group of patients with progressive disease as compared to the group of patients who responded to therapy. The presence of immune‐complexes (IC) and antipolymorphonuclear neutrophil (PMN) autoantibodies was investigated in 28 patients with chronic idiopathic neutropenia and normal or hypercellular bone marrow, 19 with a metamyelocyte arrest and 9 with more dysplastic features. The in vivo interaction between IC and PMN membrane receptors was evaluated by means of the PMN immunohistological technique. Circulating IC was evaluated with the C1q and rheumatoid factor agglutination inhibition techniques. An anti‐PM auto‐antibody activity was investigated by challenging Fab obtained from the sera of 22 patients with PMN from normal donors. IC were detected in a high percentage of patients; in no case could an anti‐PM autoantibody activity be seen. Most patients with a metamyelocyte arrest, but only 1 with more dysplastic features, were IC+. During a follow‐up period of 12–52 months, none of the patients with a metamyelocyte arrest (IC+) developed anaemia, thrombocytopenia or leukaemia, while anaemia and thrombocytopenia were almost the rule in the clinical course of dysplastic bone marrow IC‐patients: 2 of them developed acute myeloblas

摘要:
6 cases of chronic myelogenous leukaemia (CML) blast crisis in childhood were treated with vincristine and prednisolone (V‐P). In 3 terminal transferase (TdT)‐positive cases, blast cells were lymphoid in appearance. All 3 TdT‐positive patients entered a complete remission but developed meningeal leukaemia. The mean duration of survival from the onset of blast crisis was 32 months. In 3 TdT‐negative cases, V‐P therapy was ineffective.The results of this study suggest that V‐P therapy is more effective for CML blast crisis in childhood with TdT positive blast cells and that prophylactic central nervous system treatment is necessary to prevent meningea

Total parenteral nutrition for 48 h with Aminoplex solutions and a calorie source led to statistically similar, severe depression of the serum folate concentration (by 63–77%) irrespective of whether the calorie source contained ethanol, sorbitol, lipid or glucose. Folate levels were also depressed following infusion of other ethanol‐free regimens (Freamine and glucose, 59%; Vamin and glucose, 44%). Least depression (31%) followed use of Aminosol and glucose, a regimen based on casein hydrolysate. Aminoacid solutions were given to cholecystectomy patients for a period of 48 h starting 24 h after the operation. Infusion of the aminoacid component of parenteral nutrition regimens, without calorie source, frequently led to an acute fall in the serum folate concentration. The degree of depression varied with different commercial aminoacid solutions. (Aminoplex 14 = c. 60%, Vamin = 30%, Aminosol = c. 0%). The folate depression by Vamin infusion was unaffected by supplementation with L‐methione but was alleviated by the addition of glycine. As a consequence of surgical operation itself, folate values fell by 37% during the 24 h following cholecystectomy. This seems likely to increase the risk of haematological toxicity (i.e. acute leukopenia and thrombocytopenia) in patients given folate‐free parenteral nutrition after a surgical operat

Granulocyte and platelet adhesiveness were measured in 36 patients with haematologic diseases, using a glass bead column assay. Platelet adhesiveness (PA) was reduced in malignant paraproteinaemia (13/20), leukaemia (10/11) and myeloproliferative diseases (3/5). Granulocyte adhesiveness (GA) was reduced in most patients with paraproteinaemia and leukaemia, but results of measurements in whole blood and in suspensions of leucocytes in autologous plasma were poorly correlated due to the influence of abnormal platelet function and/or concentration on the measurement of GA in whole blood.The results of experiments combining leucocytes and plasma from patients and controls indicate that impaired GA in paraproteinaemia is due to a plasma factor, whereas a cellular defect is responsible for the reduced GA in leukaemia.

Experimental deficiencies of cyanocobalamin and folacin separately and in combination were induced in an air‐breathing teleost fish Channa punctatus with the help of a complete vitamin test diet. Cyanocobalamin deficiency produced normocytic hypochromic anaemia while the folacin deficiency as well as the combined deficiency of both vitamins produced macrocytic hypochromic anaemia. Leucocytosis was observed in the individual and combined deficiencies of these vitamins with significant increase in thrombocytes and decrease in neutrophil population. The relative population of different developing stages in erythropoiesis showed significant change. Thus small lymphoid haemoblast decreased in number while young and mature reticulocyte populations increased. Recovery to normal condition could be obtained by restoring deficient groups of fishes to complete vitamin test diet fortified with an initial i.m. administration of 0.01 mg/g and 0.02 mg/g body wt. of cyanocobalamin and folacin respectively. A comparison of deficiency effects on C. punctatus with Labeo rohita shows that the former, a carnivorous species with higher Hb content in peripheral blood is more susceptible to deficiency than the latter, a herbivorous species of fish with lower Hb value

Bone marrow from each of 8 untreated patients with myeloproliferative disorders was grown in diffusion chambers in 760 rad total body irradiated rats. Rats were then exposed to 11.5, 57.5, or 108.5 rad daily for 14–21 d and cell growth compared to that detected in unirradiated chambers. Cells from acute myelogenous leukaemia patients exposed to 11.5 rad per d grew for 11–21 d and there was no consistent stimulation of differentiation of immature granulocytic cells to mature granulocytes that was attributable to irradiation. Cells from a chronic myeloid leukaemia patient in chronic phase or blast crisis, and a polycythaemia vera patient with myeloid metaplasia showed significant morphologic differentiation from immature to mature granulocytes in control chambers with no additional effect of daily irradiation. Marrow specimens from 2 AML patients exposed to each of 3 daily dose fractions over 14 d revealed a dose‐dependent decrease in immature granulocytes with no persistent increase in mature granulocytes. In both irradiated and control chambers, macrophages increased over 21 d. The red cells from 5 related patients with hereditary stomatocytosis were investigated. Maximal rate constant of Na+passive permeability was increased while that of K+passive permeability was nearly normal. Ouabain‐sensitive Na+efflux was elevated. The Na+component of furosemide‐sensitive Na+, K+cotransport was also increased. However, its K+component, determined in 2 patients, remained within normal limits, thus departing from the strict 1:1 stoichiometry of the Na+, K+cotransport system. Yet, intracellular Na+and K+concentrations displayed limited and inconstant changes. A variety of abnormally‐shaped cells, including stomatocytes, were observed in scanning electron micrographs. Upon differential centrifugation, reticulocytes usually concentrated in the most dense region of the gradient. Red cell deformability, as studied by ektacytometry, was reduced. Functional hyposplenism, a clinical entity which has only recently been recognised, is usually assessed quantitatively by the rate of clearance of radiolabelled heat damaged erythrocytes (HDE) from the circulation. Based on recent observations on the kinetics of HDE, we have developed a 3 compartmental model of HDE distribution between the spleen and blood, and calculated, in a large series of clearance curves, splenic blood flow, HDE intrasplenic transit time and splenic HDE extraction ratio. Transit time (about 15 min) was of the same order as platelet transit time and extraction ratio (about 35%) was similar to that recorded in animals. The relationships between blood flow, transit time and extraction ratio provided evidence in support of 2 separate functional compartments, of which only one was engaged in phagocytosis, present within the spleen.
