Peripheral blood from 69 patients with non‐Hodgkin's lymphoma was examined with respect to B and T cell markers. Evidence for monoclonal B cell was found in 29 cases, 8 of ‘high grade’ and 21 of ‘low grade’ malignancy according to the Kiel classification. 17 out of the 29 patients had a normal lymphocyte count. Using conventional staging methods 4 cases of the 29 were in stages II and III, all others in stage IV. The proportion of S‐phase cells in peripheral blood, determined by flow cytometry, was found to be elevated in cases with a monoclonal cell population. 3 patients with myelofibrosis with myeloid metaplasia were splenectomized because of anaemia and disturbing splenomegaly. In the course of the 6 months following splenectomy, a polycythaemia developed. Erythrokinetic studies demonstrated that in all cases a reduction in plasma volume and an increase in red cell volume was obtained. Total erythropoiesis decreased along with normalization of ineffective erythropoiesis and peripheral haemolysis. The reappearance of an erythropoietic activity measured over the sacrum was a constant finding, while in 1 patient, a depression of activity over the liver was observed. The new distribution and organization of erythropoiesis in the splenectomized patients is hypothesized as being due to the removal of the influence of an enlarged spleen on erythropoietic organs.

Esterases of human leucocytes and platelets were studied by cytochemical methods. The aim of the study was to clarify the cellular distribution and possible nature of esterases types differing in their substrate specificity and/or their inhibitor sensitivity. 3 substrates (α‐naphthyl acetate: ANA; naphthol AS‐D chloroacetate: NASDCA; andN‐acetyl DL‐alanine α‐naphthyl ester: NACALA) were used and the effects of 2 inhibitors (sodium fluoride and the serine protease inhibitor phenyl methyl sulfonyl fluoride: PMSF) were evaluated. 4 enzyme types were described: Type I, present in granulocytes, was detected using NASDCA and NACALA and was resistant to fluoride but sensitive to PMSF. Other types were detected using ANA as substrate. Type II, present in monocytes, was inhibited by both fluoride and PMSF. Pure synthetic platelet aggregating factor (PAF) (1‐O‐Hexadecyl‐2‐acetyl‐sn‐glycero‐3‐phosphorylcholine) induces a dose‐dependent platelet aggregation in platelet‐rich plasma (PRP) and in gel‐filtered platelets. Irreversible platelet aggregation was observed at final concentrations of PAF higher than 2 times 10‐7mol/l, while reversible or two‐wave aggregation was obtained with lower final concentrations. The second wave was inhibited by acetylsalicylic acid, indomethacin, dipyridamole, EDTA, EGTA, theophylline, caffeine, PGE1and verapamil. In DNA synthesis deoxythymidine kinase (TK) catalyses the conversion of deoxythymidine to deoxythymidine monophosphate (dTMP) via the ‘salvage pathway’. Serum deoxythymidine kinase (S‐TK) was measured in this study in 75 patients with vitamin B12deficiency by a new, very sensitive method, using125I‐deoxyuridine as substrate. Elevated S‐TK levels were found in those patients who had developed haemolysis and anaemia and the more advanced the disease the higher the S‐TK value. Thus there was a highly significant correlation between S‐TK, haemoglobin level and lactic dehydrogenase activity. Fibrin(ogen) degradation products (FDP) are assayed adequately in defibrinated citrated plasma provided that fibrin(ogeno)lysis is prevented during coagulation. The practical consequence is that all screening tests for haemostatic function may be carried out in specimens obtained from citrated blood.

A 32‐year‐old female with Down's syndrome and common ALL was treated with human leucocyte interferon‐α (IFN‐α) at a dose of 3 times 106units i.m. daily. Side‐effects consisted of slight tenderness and localized haematomas at the sites of injection. During treatment the number of leukaemic cells decreased in the peripheral blood. The proportion of blast cells in the bone marrow decreased only slightly, but a large fraction of them became vacuolized. After the initial response the disease progressed and IFN was withdrawn after treatment for 67 d. Prior to initiation of IFN treatment the natural killer activity of the patient's lymphocytes, as well as the response of these cells to mitogenic stimuli, was low. During IFN therapy these functions increased. Following the withdrawal of IFN therapy the patient received vincristine/prednisone treatment and a complete remission was achieved after 5 weeks. The mechanisms for cell toxicity with adenosine deaminase inhibition by 2′‐deoxycoformycin (dCF) in non replicating lymphoid cells include S‐adenosylhomocysteine (SAH) hydrolase inactivation and reduction of cellular ATP content. These postulates were explored in a patient with T‐CLL receiving dCF with a resultant fall in peripheral blood lymphocytes from 740 times 109/1 to 90 times 109/1 over 15 d. In red cells there was complete inhibition of adenosine deaminase and SAH hydrolase activities, progressive deoxyadenosine triphosphate (dATP) accumulation and ATP depletion but no significant alteration in adenosine monophosphate (AMP) deaminase activity or distribution in purine intermediates from radioactive adenosine. In T‐CLL lymphocytes, there was incomplete lymphoid SAH hydrolase inactivation, reduced AMP deaminase activity and progressive dATP accumulation. The limited decrease in lymphocyte ATP content was related more to dCF administration than dATP accumulation, nor accompanied by significant changes in the distribution of purine intermediates from adenosine. These findings suggest that ATP depletion with dCF therapy does not reflect AMP deaminase activity modulation nor is of critical importance for cell toxicity. 191 combined collections of granulocytes (PMN) and platelets (PLT) obtained by discontinuous cytapheresis were given to 23 patients with acute myeloid leukaemia (AML). The circulation in recipients of the concomitantly transfused cells was compared by measuring the % recovery of PMN at 1 h, and of PLT at 1 and 20 h. The recovery of both cells was greatly reduced in transfusions accompanied by a transfusion reaction. Fever in recipients had no effect on the recovery of either cell, but PMN recovery was significantly higher in patients receiving oral prednisolone. Alloimmunization to PLT, defined as recovery levels less than 15% at 1 h and/or less than 10% at 20 h, was recorded in 42 transfusions, and PMN recovery was significantly reduced in this group. In transfusions where PMN recovery was ‘low’ (less than 5%); the recovery of the coincidentally transfused PLT was significantly reduced only in transfusions accompanied by a reaction.
