Of 377 patients with chronic duodenal ulcer undergoing highly selective vagotomy, 13 were splenectomized peroperatively because of capsular tear. 12 of these patients were alive at the time of the subsequent investigation and each was matched with a non‐splenectomized patient. The postoperative interval was almost 5 years (median).The splenectomized patients were characterized by their longer hospitalization, and increased blood counts of lymphocytes, monocytes and platelets. Analysis of lymphocyte subsets revealed increased numbers of both T and B lymphocytes after splenectomy. Among the T cells, those with receptors for Fc of IgG were significantly more numerous in the splenectomized group. It is a matter of controversy whether or not this population contains T suppressor cells. Hence further functional investigations are needed to clarify whether our observations bear any relation to the low level of plasma IgM found by us and others in splenectomized individuals. Polymorphonucelar leucocytes (PMNL) were incubated with hyaluronic acid (HA), and then characterized with respect to their surface properties. Surface analysis was achieved by partitioning in systems of dextran and poly(ethylenglycol) (PEG), with part of the PEG exchanged for positively charged trimethylamino‐PEG or hydrophobic PEG‐palmitate. The results indicate that HA caused increased exposure of hydrophobic and of negatively charged groups on the PMNL surface. Cultured, marrow‐derived, adherent cells (MDAC) provide a microenvironment which supports the proliferation of haemopoietic stem cells (HSC) for extended periods in vitro. Morphological characterization suggested that MDAC populations consisted of a variety of cell types, including mononuclear phagocytes, fibroblastoid cells, fat cells and vascular endothelial cells. Recently performed functional characterization studies suggest that they consist largely of collagen‐producing, fibroblastic cells. MDAC were not, however, examined systematically for endothelial cell characteristics. Unrecharged cultures of MDAC, shown in parallel studies to support in vitro haemopoiesis, were examined for endothelial cell markers. These included the presence of Weibel‐Palade bodies and synthesis of factor VIII related antigen. They were also examined biochemically for synthesis of basement membrane (type IV) collagen. The results of these investigations were negative in all cultures examined. It is thus concluded that vascular endothelial cells are not present as a significant component of the unrecharged MDAC population and do not, therefore, contribute to the functional haemopoietic microenvironment in vitro or in

Coculture with mononuclear cells from aplastic anaemia (AA) patients and normal bone marrow cells produced different responses among 10 patients studied. In 4 cases, CFU‐E growth was inhibited while in another 2 cases CFU‐C growth was inhibited. To determine the suppressive effects of mononuclear cells from 6 AA patients, mononuclear cells were separated into T cells, B cells, and monocytes. It was found that T cells from 6 AA patients exhibited suppressive effects on normal CFU‐C or CFU‐E growth, whereas T cells from normal subjects enhanced the colony formation of CFU‐C and CFU‐E. On the other hand, monocytes from 6 A A patients and normal subjects strongly suppressed normal CFU‐C and CFU‐E growth, although no significant difference was observed in both groups. The immune system in 10 AA patients was evaluated. 3 AA patients (Cases No. 1, 6, 8) exhibited an increase in Tr cells, impairment of PHA responsiveness, and low level of immunoglobulin. DNA‐repair synthesis was studied in blast cells from patients with acute myeloblastic leukaemia. The DNA‐repair synthesis, measured as3H‐thymidine incorporation, was registered as unscheduled DNA synthesis by autoradiography, and also after suppression of the replicative DNA synthesis by hydroxyurea by liquid scintillation. There was good correlation when results from both methods were compared. A significantly wider variation of unscheduled DNA synthesis was found in leukaemic as compared to normal blast cell populations. There was also a wider variation within individual leukaemic blast cell samples compared to normal samples. A correlation was found between unscheduled DNA synthesis induced by nitrogen mustard and by UV‐irradiation in the leukaemic blast cell populations. The UV‐induced unscheduled DNA synthesis in some of the leukaemic blast cell populations continued to increase at doses of UV‐irradiation at which normal myeloblasts had reached their maximum level of unscheduled DNA synthesis. The mechanisms of red blood cell destruction in 2 patients with penicillin‐induced immune haemolytic anaemia were investigated. Anti‐penicillin antibodies of the IgG subclass were found in the patients' sera and in the eluates of their direct antiglobulin positive red blood cells. Using a rapid51Cr in vitro assay it was shown that fresh peripheral blood mononu‐clear phagocytes and granulocytes but not lymphocytes from both patients lysed and phagocytosed autologous red blood cells previously treated in vivo or in vitro by penicillin and autologous anti‐penicillin antibody. Antibody‐dependent cell‐mediated cytotoxicity (ADCC) as well as antibody‐dependent phagocytosis (ADPh) were proportional to serum concentration and to the number of attacking cells. Anti‐penicillin antibody from 1 patient activated the complement system in vitro but failed to induce lysis of penicillin‐treated red blood cells in the presence of complement.These results suggest that ADCC as well as ADPh participate in the destruction of red blood cells in penicillin‐induced

Colony‐stimulating activity (CSA) was demonstrated in materials taken from a patient suffering from lung cancer associated with excessive leucocytosis. CSA was detected not only in his urine, serum and pleural effusion but also in the supernatant of cell cultures originating from the effusion. The excessive leukocytosis of the patient might be due to a CSA producing tumor. Financial support was obtained from the Herman Järnhardt's Foundation.There are at least 7 variants of Ehlers‐Danlos syndrome (EDS). One family with type 1, one with type 3 and one isolated case with an undefined type of EDS are described. They all had bleeding symptoms, but surgery had been performed without complications. Collagen is believed to be defective in EDS. However, in our cases the platelet aggregating power of the collagen was normal. The bleeding time (Ivy) was slightly prolonged. Extensive examination of the haemostatic process revealed a decreased platelet adhesiveness in only 2 of the patients, a finding believed to be coincidental. The history of former operations on the patients and on affected relatives is considered to be the best guide in determining the risk of an operation, but preoperative estimation of the bleeding time is recommend

摘要:
Fanconi's anaemia is reported in 2 siblings. The simultaneous onset of pancytopenia after possible exposure to common external agents suggest that both a frail genotype and environmental factors may be etiologically involved in the disorder. One of the children died after having developed a monoblastic leukaemia. This patient disclosed very striking cytological abnormalities, including a dyserythropoietic pattern and ropalocytosis. The physiopathology of the latter abnormality, which has hitherto not been noticed in Fanconi's anaemia, is uncertain.

A detailed study is described of a case of hairy cell leukaemia, presenting with a serum paraprotein of an immunoglobulin (Ig) class different from that synthesised by the neoplas‐tic cells. The case was unusual in its association with leukaemic arthropathy but ultrastruc‐turally the hairy cells were typical. By immunofluorenscence and immuno‐electron microscopy the neoplastic cells expressed IgAλ both on the cell surface and intracellularly in the rough endoplasmic reticulum, perinuclear space and Golgi apparatus. No Ig was observed in the ribosomal‐lamellae complexes. These cells also synthesised and secreted Ig of class Aλ in culture. However the serum paraprotein was of class IgA® and could not be attributed to an abnormal population of plasma cells in the bone marrow. There was no other evidence for myeloma and the IgA® paraproteinaemia appeared to be benign, apparently unrelated to the neoplastic proliferation of
