Bone marrow haemosiderin iron was correlated with various laboratory parameters of iron status in order to define a reliable laboratory index of marrow iron stores. Marrow iron, serum iron, plasma transferrin, transferrin saturation, iron absorption, red cell iron incorporation and serum ferritin were measured in 21 non‐dialysed, 20 peritoneal dialysed and 34 haemodialysed patients with chronic uraemia. Diagnostic accuracy of the laboratory tests in terms of diagnosing reduced marrow iron stores was expressed as the predictive value of a positive test and a negative test, and efficiency. Diagnostic efficiency of serum iron, plasma transferrin and transferrin saturation was too low to be of value in the individual patient. Iron absorption and red cell incorporation had a higher diagnostic efficiency, but are unpractical for routine use. Serum ferritin displayed a high diagnostic efficiency and appears to be a reliable index of marrow iron stores, suitable for sequential monitoring of iron status in uraemic subject

The 2 types of erythrocytes from a person with persistent mixed‐field polyagglutinability (Tn abnormality) were separated from each other by preparative cell electrophoresis. Surface labelling using the galactose oxidase/NaB3H4technique followed by polyacrylamide gel electrophoresis showed a strong labelling in the glycophorin A region of Tn positive erythrocytes indicating exposed galactosyl N‐acetyl/galactosaminyl residues. Tn positive cell membranes were labelled by the galactose oxidase/NaB3H4technique and solubilized in non‐ionic detergent. After chromatography on Helix pomatia lectin‐linked Sepharose, glycophorin A was immunoprecipitated from the sugar eluate using specific antiserum. Glycophorin A from Tn negative cells and normal red blood cells did not bind to Helix pomatia lectin but to Lens culinaris lectin‐Sepharose. Glycophorin A and band 3 were isolated by preparative gel electrophoresis from normal cells and the two red cell populations of the Tn individual. Pronase treatment of labelled glycophorin A followed by gel filtration revealed a more efficient proteolysis in molecules isolated from Tn positive cells. Mild alkaline treatment of galactose oxidase/NaB3H4or periodate/NaB3H4labelled glycophorin A liberated 3 different oligosaccharides from Tn positive cells. Heparin levels were followed by an amidolytic method during open heart surgery in 10 adult patients and correlated to the activated clotting time (ACT) (Haemochron®). There was a good correlation between the 2 parameters when the ACT was below 600 s. There are 3 varieties of idiopathic thrombocytopenic purpura (ITP) in children: the acute, the chronic and the recurrent forms. We have tested the platelets from 80 children with these forms of ITP by an immunofluorescence test. Platelet‐bound immunoglobulins were detected in 19/22 acute, 41/48 chronic and 5/10 recurrent cases of ITP, i.e. in 65 of a total of 80 cases. A significance differences between acute and chronic ITP was found in the observation that only IgM was bound to the platelets of 7/16 patients with acute ITP.In all cases of chronic ITP with a positive direct immunofluorescence test, IgG was present on the platelet membrane. It is assumed because some patients with chronic ITP do not have elavated platelet‐bound immunoglobulins, and show refractoriness to immunosuppressive therapy, that in these patients the disorder is not of a humoral autoimmune nat

38 children with acute lymphocytic leukaemia (ALL) in haematologic relapse were retreated with vincristine, daunomycin and prednisone (VPD) together with intrathecal methotrexate and prednisone, followed by asparaginase in those patients not in complete remission after 4 weeks. The overall complete remission (CR) rate was 79%; asparaginase was needed to achieve CR in 7 of the 30 responding patients. The median duration of second remission was only 36 weeks, but 6 out of 15 children receiving the COAP‐POMP‐CART consolidation regimen remain in continuous second remission after 37–260 weeks; 3 of them are currently off all therapy. Scanning immuno‐electron microscopic labelling techniques were successfully used to identify a variety of leukaemia‐lymphoma cells. Anti‐T, ‐B and ‐common type acute lymphoblastic leukaemia (ALL) antisera were covalently conjugated to polystyrene latex spheres by means of glutaraldehyde. Peripheral blood mononuclear cells obtained from healthy individuals and from patients with T‐type ALL, B‐type chronic lymphocytic leukaemia (CLL), acute and chronic non‐lymphoblastic leukaemias (ANLL) and cultured leukaemia‐lymphoma cells were incubated with the above antisera‐latex conjugates. Positively labelled cells were easily identified by the presence of latex spheres on their surfaces. Cross reactivity of the antisera used and non‐specific attachment of the markers were infrequently encountered. Between 1970 and 1975, 73 patients with surgically staged IA and IIA Hodgkin's disease, localized above the diaphragm, were treated with mantle field irradiation only. 24 patients relapsed during the first 5 years after the primary treatment, but 14 of them obtained long‐lasting complete remission after retreatment. 23 patients with lymphocyte‐depleted histology, more than 3 sites involved or a mediastinal mass of more than 6 cm had a high relapse rate (60%); these patients need treatment additional to the mantle field irradiation. The other 50 patients had an 80% relapse‐free survival, 96% complete remission after treatment of the first relapse, and 98 % survival after 8 years. 136 patients suffering from ALL were subdivided into 5 subtypes (C‐ALL, C/T‐ALL, pre‐T‐ALL, B‐ALL) according to rosetting tests and using specific antisera directed against membrane antigens. In addition, leukaemic blasts of all patients were investigated according to morphological and cytochemical criteria. In APh and ANAE, indices and the percentages of cases showing a granular staining pattern were high in pre‐T‐ and in T‐ALL, but low in C/T‐ and in C‐ALL. PAS‐staining, conversely, was more pronounced in C/T‐ and C‐ALL. APh proved to be more discriminative for recognition of the T‐ and pre‐T‐ALL subgroups than ANAE, but ANAE‐cytochemistry may be useful to detect contaminating normal T‐lymphocytes in ALL. Receptors for C3were more frequent in C‐ and in T‐ALL than in C/T‐ and in pre‐T‐ALL, receptors for Fcwere distributed equally among all subtypes. Positivity of C3‐ and Fc‐receptors was not correlated with cytochemical results. Prognostic factors were studied in a series of 80 patients with aplastic anaemia. The most important and easily obtainable individual variables, isolated in a previous univariate statistical analysis, were placed in a linear logistic regression model. A prognostic formula was derived containing the following 4 variables, in decreasing order of significance: (1) reticulocytes, (2) interval from onset of symptoms to first visit, (3) mean red cell volume, and (4) platelets. The prognostic formula permits an estimate of the probability of death within 3 months from the first visit. Its usefulness in several clinical situations, especially those related to bone marrow transplantation, is illustrated.
