Acquired factor VIII deficiency in women postpartum due to a factor VIII inhibitor is rare and the etiology is unknown. In this study a case report and a review of the literature are given. The haemorrhagic diathesis resembles classic haemophilia, with the exception that ecchymoses and tissue bleeding occur more frequently. The potency of the inhibitor may vary from weak to strong and the inactivation of factor VIII coagulant activity (factor VIII‐C) by the inhibitor is of a non‐linear type. Severe bleeding has been fatal in a few cases, but factor VIII concentrate substitution has usually been successful without anamnestic response of inhibitor activity. There is no convincing evidence that immunosuppression is effective, also because the natural history of the disease is characterised by a spontaneous disappearance of the factor VIII‐C inhibitor. Treatment of bleeding symptoms with factor VIII concentrate should therefore not be reserved for life threatening haemorrhages

The present study was designed to discriminate and analyze the presence of direct PMN chemotaxis and leucocyte‐induced PMN chemotaxis in filter assay systems of PMN chemotaxis, namely the Wilkinson chamber and the Boyden chamber, which yield a more quantified information on leucocyte chemotaxis than filming of vital cells.The PMN chemotaxis was reduced by approximately 30–35 μm after incubation with vinblastine, 0.01, 0.10 and 1.00 μg/ml respectively, as measured by the leading front method in the Wilkinson chamber. This figure was thought to represent the contribution of the leucocyte‐induced antitubulin‐sensitive PMN chemotaxis to the casein‐induced PMN chemotaxis under the experimental conditions prevailing. The remaining 60 μm antitubulin‐insensitive PMN migration into the filter probably represented a combination of direct PMN chemotaxis and stimulated PMN random motility.Since the above‐mentioned vinblastine inhibition of PMN chemotaxis was recorded in the absence of serum, complement factors included, the vinblastine‐inhibited PMN chemotaxis was thought to be due to the release of a leucocyte‐derived cytotaxin.The significance of incubation time and chemotactic parameters was further analyzed in the presence of serum in Boyden chambers, in the intercompartmental filters by means of PMN distribution curves and on the bottom filter by cell numbers. The antitubulin inhibition of PMN chemotaxis was evident in the intercompartmental filter during the initial period of incubation and later by cell counts on the bottom filter. DNA synthesis time (Ts) and3H‐thymidine labelling index (TLI) of erythroblasts have been determined in 25 patients with various types of haematologic disorders using in vitro double labelling method. No remarkable differences in both Ts and TLI were noted between haematologically normal subjects and patients with increased effective erythropoiesis (haemolytic anaemias), suggesting that the cell cycle time is not principally altered under the augmented erythropoiesis.In pernicious anaemia, Ts of basophilic erythroblasts was significantly shortened and TLI was elevated above normal. Normalization of erythropoiesis by vitamin B12was associated with a transient increase of TLI in polychromatic erythroblasts, which was interpreted to reflect prevention of intramedullary premature death of basophilic megaloblasts. Erythroleukaemia showed a markedly prolonged Ts and lowered TLI indicating the presence of cells with prolonged cell cycle time. These findings contrasted to that of pernicious anaemia despite certain morphological as well as functional similarities. In idiopathic sideroblastic anaemia, prolongation of Ts was observed to a similar extent as erythroleukaemia, while TLI remained almost normal. Leucocytes which form rosettes with homologous erythrocytes in auto‐immune haemolytic anaemia, infectious mononucleosis and following the stimulation of lymphocyte cultures with PHA have been studied by electron microscopy. The studies showed that the predominant homologous rosette forming cell was lymphocytic and the evidence favoured the conclusion that they were non‐immune in nature. This identification does not classify the lymphocyte as thymus or bone marrow derived. Just under half (44%) of the total lymphocytic RFC had perinuclear microfilaments and limited clearing of the plasma of ribosomes close to the cell membrane. These features are consistent with activated T lymphocytes. The remainder of the lymphocytic RFC had no features which allowed their identification as T or B cells.The form of contact between lymphocytic RFC and erythrocyte was mainly of a pointlike nature, with little deformation or invasion of the attached erythrocytes. The effects of neuraminadase on the electrophoretic mobility and in vitro lysis of PNH, glutathione treated and normal erythrocytes was determined. Neuraminidase strongly affected the in vitro haemolysis of PNH and glutathione treated erythrocytes by substantially elevating the lysis in the Ham acid test while virtually eliminating lysis in the sucrose test. Findings suggest that membrane bound sialic acid or the level of cell surface charge may have a role in the susceptibility of PNH erythrocytes to complement mediated lysis.

Peripheral blood lymphoid cells of 29 patients with acute lymphocytic leukaemia (ALL) at onset were studied for characterization of B and T membrane markers and phytohaemagglutinin responsiveness. 24 cases (83%) were classified as ‘null’ cell ALL and 5 (17%) as T‐cell ALL. Isozymes of amino acid naphthylamidase were investigated in 11 patients with chronic myeloid leukaemia (CML) and 1 patient with erythroleukaemia. Two isozymes were found in all cases (isozymes A and C) which also occur in normal granulocytes. An isozyme with intermediate mobility appeared in all the patients in two main forms (isozymes BFand BS), which both differed from the isozyme B found in normal granulocytes. This may therefore support the diagnosis. In 3 patients a shift occurred between isozymes BFand BS, the possible significance of which is discussed.

The proportions of myeloblasts, promyelocytes, myelocytes and promonocytes within the granulopoietic precursor pool of the bone marrow and mitotic indices (MI) of the precursor cells were determined in 277 patients with various forms of myeloid leukaemias. The MI of the leukaemic patients were significantly lower than those of 20 normals. Thus the median MI in 186 cases of acute myeloid leukaemias was 0.68%, compared with 1.0% in 85 cases of chronic leukaemias and 1.2% in the normals. Separate MI of the different leukaemic precursor cells were counted in 21 cases of acute myeloid leukaemias and these indices were lower than those of the corresponding normal cells. There were significant differences between the different groups and subtypes of leukaemias and it is suggested that mitotic countings might be of diagnostic and prognostic significance.

This brief case report describes a sustained haematological remission obtained in a patient with primary myelofibrosis treated by bone marrow curettage after previous therapy with androgens was unsuccessful. The patient remains in haematological remission 5 years after the procedure. It is suggested that this procedure be attempted in more cases of myelofibrosis.

The rapid development of chemotherapy and radiotherapy during the last decade makes an increasing demand for a reliable classification of malignant non‐Hodgkin's lymphomas. This is especially important in children since in this age group the lymphomas show a much worse prognosis than in adults. An attempt at a modern classification is offered by the so‐called Kiel classification. 38 children with non‐Hodgkin's lymphomas previously classified according to Rappaport were re‐evaluated according to the Kiel system. Reclassification was technically feasible in 26 patients. There was good agreement on typing between the two independently working cytopathologists. 3 patients proved to be cases of histiocytic medullary reticulosis. Among the remaining patients, a much larger variety of histological subgroups was seen than in the one published paediatric series of Lennert. 17 patients had high‐grade malignant lymphomas with lymphoblastic lymphomas predominating. Some clinical correlations not detectable with the Rappaport classification were found using the Kiel system. 6 patients were judged as having low‐grade malignant lymphomas but in this group the survival was poorer than expected and 1 patient showed leukaemic transformation. The Kiel classification makes high demands for adequate surgical techniques and preparatory routines. It seems to represent a step forward in the classification of non‐Hodgkin's lymphomas but much more experience is needed, especially in children, in order to evaluate its role as a guide to different
