Immature myeloid cells highly enriched for colony‐forming cells, myeloblasts and promyelocytes were isolated in Percoll gradients (lighter than 1.065 g/ml) from patients with chronic myeloid leukaemia (CML) or normal healthy volunteers. The proliferation and differentiation of these cells were followed in suspension cultures in McCoy's medium (15% FCS) with or without the addition of 15% human placenta conditioned medium (HP‐CM) over a culture period of 12–16 d. Ion‐exchange chromatography and gel chromatography indicated that the proliferation‐stimulating activity in HP‐CM was colony‐stimulating activity (CSA). In unstimulated cultures (without HP‐CM), normal cells produced very few neutrophils; macrophage production dominated. CML cells, on the other hand, gave rise to neutrophils even in the absence of stimulatory factor(s). The effect of HP‐CM was dependent on the initial concentration: at a ‘low’ (1 times 105/ml) concentration, the addition of HP‐CM resulted in a great increase in neutrophils, but at a ‘high’ (5–8 times 105/ml) initial cell concentration, HP‐CM gave only minimal increase in neutrophil numbers in both normal and CML cultures. The subcellular localization of acid phosphatase (AcP) in various immunologically‐defined neoplastic lymphoid cells including hairy cells was investigated by electron microscopy. 2 substrates, naphthol‐AS‐BI‐phosphoric acid (naphthol‐AS‐BI‐P) and sodium β‐glycerophosphate, were compared. By incubation in naphthol‐AS‐BI‐P containing medium, the reaction product was found located in granules, vesicles, the Golgi apparatus, the rough ER including the nuclear envelope in the cells of T ALL, T CLL and HCL. A typical pattern of reaction was observed for each of these disorders: enzyme‐positive Golgi membranes and neighbouring granules, clustered in the nuclear notch in T cell‐derived lymphoblasts; enzyme‐positive granules around Gall bodies, aggregated paranuclearly in T CLL lymphocytes and enzyme‐positive scattered cytoplasmic granules and vesicles in hairy cells. Enzyme activity was occasionally seen in singly‐occurring granules in the cells of cALL, B CLL and B PLL, rarely in other substructures. With the Gomori method using β‐glycerophosphate as substrate, the enzyme reaction was limited primarily to lysosomal sites and was seldom observed in other organelles. 20 patients with leukaemia in remission or early relapse received an allogeneic bone marrow graft. Patients were conditioned according to Seattle protocol with high dose cyclophosphamide and total body irradiation to a total dose of 8 Gray to the lungs delivered by a 6 MV linear accelerator at a dose rate of approximately 0.06 Gray/min. As prophylaxis against the graft versus host reaction, methotrexate and/or cyclosporin A were given.Lung function was studied prior to treatment and every 3 months thereafter. Before treatment a marked decrease was seen in the CO diffusion capacity when compared to the predicted value, while the flow volume relationships were within normal limits. The cause of this is unknown, but may be due to previous infections and the cytostatic treatment given. After bone marrow transplantation, a further irreversible decrease was seen in the CO diffusion capacity and vital capacity indicating an additive effect of the lung irradiation.

In this study we have looked for differences in coagulation parameters before and after surgical removal of benign or malignant tumours. A striking increase in thromboplastin activity of the blood monocytes was seen 1 d after surgery. This paralleled a fall in factor VII activity. At the same time, the sensitivity of the blood monocytes to stimulation by endotoxin increased significantly. We propose from this study that monocyte thromboplastin may be a postoperative thrombogenic factor. Increased levels of factor VIII and fibrinogen 2–3 d postoperatively were found, probably caused by inflammation reactions induced by the surgery. No difference in coagulation parameters could be demonstrated between patients with benign, noninvasive lesions and patients with invasive, carcinomatous lesion

16 young women in long‐term remission after first‐line treatment for the early stages of Hodgkin's disease were examined for ovarian function 48 to 125 months after termination of therapy. The patients had received mantle field irradiation, plus either irradiation of infradiaphragmatic lymph nodes or 6 cycles of MOPP. 4 patients showed signs of ovarian failure judged by menopausal symptoms, menstrual pattern and/or hormone values. 12 patients had functioning ovaries; 8 of these had become pregnant after treatment, 2 had had an induced abortion, and 7 had given birth to a total of 9 healthy babies after treatment. The patients with signs of ovarian failure were older than the others, but the difference was not statistically significant. No difference between the patients who had received different treatments was established, nor does the study confirm the proposed protective effect of oral contraceptives. For women under 35 years of age, the long‐term chances of preserving ovarian function after standard treatment for the lower stages of Hodgkin's disease seem to be much better than hitherto as

Computed tomographic (CT) brain scan findings in a patient with severe thrombotic thrombocytopenic purpura (TTP) are described. Multiple scattered lucencies were noted which correlated with thrombotic occlusions found at autopsy. In this patient, the neurologic manifestations of TTP preceded the haemolytic anaemia. In the correct clinical setting, multiple areas of lucency should suggest multiple infarcts secondary to thrombotic vessel occlusions as a diagnostic possibility.

Normal human erythrocytes in isotonic suspension maintain a high gradient of calcium ion concentration across the membrane, with the cytosol [Ca2+] several orders of magnitude below plasma concentration. Despite the high degree of exclusion maintained against the entry of Ca2+, the osmotic resistance of red cells in hypotonic saline is significantly augmented by the presence in the medium of Ca2+in concentrations greater than 0.07 mmol/l. Resistive particle spectroscopy showed this to occur without alteration of the mean critical haemolytic volume; furthermore, in the presence of Ca2+there was a reduction in cell volume at prolytic osmolalities when compared with cells in calcium‐free sodium chloride solutions. This is shown to be due to augmentation of the prolytic loss of K+1in the presence of Ca2+through calcium‐sensitive channels, and indicates that the pathway for passive K+efflux uncovered in the prolytic state is indistinguishable from that of normal passive K+diffusion. It is also demonstrated that the prolytic red cell membrane is permeable to Ca2+and admits the divalent cation to the inner aspect of the plasma membrane. These changes are comparable to those of cells prior to haemolysis in vivo, as in the irreversibly sickled cell, and are therefore not specific to the latter condit
