Cholestasis is acommon complication of hepatic hydatid cyst. We describe the various mechanisms of cholestasis in hepatic hydatid cyst as demonstrated by percutaneous transhepatic cholangiography (PTC) and endoscopic retrograde cholangiopancreatography (ERCP) in 16 patients. In our patients, cholestasi was the result of rupture or mechanical pressure of the cyst on the biliary system. Direct visulization of the bile duct, especially by PTC, provided precise definitions of the pathology, and aided the surgeon in his approach. There were no adverse reactions. All pre operative diagnoses were confirmed at operation.

Crohn's disease of the stomach is an uncommon condtion which often progresses to stenosis with associated gastric outlet obstruction. We describe a patient who obtained long-term relief of obstruction following a short course of corticosteroids and total parenteral mutrition, and we discuss the therapeutic options for gastroduodenal Crohn's disease.

We report the preoperative histologic diagnosis of leiomyosarcoma of the duodenum and review the clinical aspects and treatment of this tumor. The endoscopic techniques were critical in obtaining the diagnosis and deciding the treatment plan.

Immunoperoxidase staining allows detection of small amounts of antigens in tissues, and can be applied to formalin-fixed, paraffin-embedded specimens several years old. We utilized this technique to stain for carcinoembryonic antigen (CEA) in 11 specimens of colon from 10 patients with ulceratve colitis. We compared the degree of epithelial abnormality in these mucosal specimens with the extent of immunoperoxidase staining for CEA. CEA staining was also comapared with the duration of colitis, extent of colonie in volvement, the indication for colectory, and the medical therapy before colectomy. We found that neither cytoplasmic nor cell surface CEA staining correlated with the degree of epithelial alteration. Similarly, CEA staining did not correlate with any of the clinical parameters evaluated. We conclude that immunoperoxidase staining for CEA is not useful in assessing the degree of dysplasia in mucosal specimens from patients with ulcerative colitis. There seems to be no clinically relevant role for anti-CEA staining in ul-cerative colitis.

Occasionally, routine computed tomography will fail to demonstrate substantial deposits of metastatc neoplasm in the liver. Repeating the CT scan with selective hepatic arterial infusion of contrast media may demonstrate these lesions. Nuclear magnetic resonance imaging may also provide the same information, avoiding the necessity of an invasity procedure.

The liver plays a major role in modifying the total circulating hormone concentrations of thyroxine and triiodo-thyronme by secretion and degradation of carrier proteins. It also is a major site of peripheral conversion, degradation, and excretion of thyroid hormones. We review the liver's role in thyroid hormone physiology and summarize the changes in circulating thyroid hormone concentrations in various liver diseases.
