Multiple gastrointestinal allergies were studied in 98 infants and children. Combined sensitivities were milk and soy in 61, soy and gluten in 34, and milk and gluten in three. Two clinical groups became apparent: those infants with early diarrhea (1–6 months of age) with strong family history of atopy or food allergies and without prior intestinal infection; and those children up to age 3 years who developed chronic diarrhea and gastrointestinal sensitization after an acute viral or bacterial gastroenteritis.Transient or “late turn-on” immunoglobulin disorders were present in nearly half of the milk-allergy infants studied, and in nearly a third of the entire group. Such a high incidence of immunoglobulin abnormalities has not been previously reported in children with allergic gastroenteritis.

We have analyzed the results of anti-inflammatory drug treatment of 100 consecutively treated patients with Crohn's disease. The disease affected the colon in 12 patients, small bowel in 36, and both colon and small intestine in 52. Thirty-two patients had had a prior resection of Crohn's disease. According to an adaptive design, patients were treated with sulfasalazine if able to take oral drugs; with steroids if unable to take, or unresponsive to, sulfasalazine; and with added azathioprine if unresponsive to sulfasalazine and steroids and unsuitable for surgery. Drug regimens included sulfasalazine alone in 39 patients, steroids alone in seven patients, both sulfasalazine and steroids in 43 patients, and added azathioprine in 11 patients. All patients completed at least 1 year, or were regarded as treatment failures after 3 months of drug treatment. We analyzed clinical and ra-diographic responses. Significant improvement occurred in all categories of patients: complete or partial clinical remission was observed in 79%, and complete or partial radiographic regression was observed in 55%; 30% of patients were not reexamined radiographically and the remainder showed no improvement. Our study suggests that properly selected anti-inflammatory drug therapy has definite benefits and is indicated for patients with Crohn's disease.

A 23-year-old white woman complained of sore throat and difficulty in breathing. Examination of her larynx revealed chronic inflammatory changes of the epiglottis and aryepiglottic folds, and a biopsy demonstrated chronic granulo-matous reaction. Six months later she developed chronic diarrhea, rectal pain, and typical anorectal findings of Crohn's disease. Biopsy of the anus revealed the same microscopic features as in the laryngeal structures. Treatment in keeping with the acceptable but limited measures for Crohn's disease brought improvement in both areas of inflammation. We feel this patient has Crohn's disease in the larynx, involvement which preceded that of the gastrointestinal tract.

The oculopharyngeal syndrome (OPS) is a rare variant of muscular dystrophy characterized by progressive ptosis and dysphagia. Previous esophageal motility studies in OPS have yielded conflicting results because of low-fidelity esophageal recording systems. Although cricopharyngeal myotomy improves symptoms, accurate postoperative manometric findings have not been reported. Using a low-compliance, high-fidelity system and a radially oriented esophageal motility catheter in an OPS patient we found a hypertonic upper esophageal sphincter (UES), a hypotonic proximal esophagus, and elevated pressures in the distal two-thirds with prolonged duration of contraction. Clinical improvement followed myotomy. We also found a similar manometric pattern but a normotensive UES in an asymptomatic sibling.

Angiography is one of several methods available for demonstrating active arterial or capillary bleeding sites in the gastrointestinal tract. Bleeding at a rate greater than 0.5 cc/minute is readily identified. Selective infusions of vaso-constricting drugs and embolic therapy have been highly successful in the management of both gastric mucosal and lower gastrointestinal bleeding and represent a reasonable alternative to surgery for these conditions. Bleeding duodenal ulcers are much less responsive to transcatheter therapy and are usually best managed surgically.

In our hope to be as eclectic and still as practical as possible, I have asked Mrs. Angela R. Holder to write a series of discussions about malpractice problems involving treatment of digestive disorders. She has been surprised to find how few claims have been made against gastroenterologists as such, and so she will discuss cases against internists dealing with gastrointestinal disorders. One problem which bothered me was that names of actual physicians are mentioned in the titles of the cases she discusses, but the cases are after all published in the legal literature and for better or worse are in the public domain. If any of you feel this is something we should not do, write to me, but I believe that this series of discussions from a lawyer who is an expert in the malpractice world will be of value to all.H.M.S.
