The spectrum of complications encountered after injection sclerotherapy span esophageal, extraesophagea, and systemic sequeiae of the sclerosing process. We report a case of pseudodiverticula secondary to sclerotherapy, a hitherto unreported complication of this modality.

Two patients developed hemolytic anemia while taking cimetidine. Neither patient was taking other drugs known to cause hemolytic anemia. In both, the hemolytic anemia resolved after the drug was stopped. In one patient, the direct antiglobulin (Coombs') test was strongly positive when the hemolytic anemia was recognized and became only weakly positive as the hemolysis subsided. However, serologic studies for antidrug antibodies yeilded negative results in both patients; readministration of cimetidine for 55 days in patient 1 and for more than 24 months in patient 2 did not cause recurrence of hemolysis. We conclude that we cannot incriminate cimetidine as the cause of the bemolytic anemia in either of our patients. These findings emphasize that a temporal association of drug administration and hemolytic anemia is not proof of a cause-effect relationship and that reports of drug-related adverse hematologic effects must be interpreted with caution.

Due to conflicting reports on the role of CEA measurement in the gastric juice of patients with gastric malignancy, we have assessed gastric CEA levels in a variety of both non-malignant and malignant Jesions of the stomach. The average gastric CEA levels were higher in patients with gastric ulcer and gastric cancer than in healthy subjects and those with duodenal ulcer. In addition, the frequencies of abnormally high values were significantly (P < 0.001) higher in gastric ulcr or gastric cancer than in healthy or duodenal ulcer groups whereas there was no difference between gastric ulcer and gastric cancer groups. The evaluation of gastric CEA levels in relation to gastric inflammatory changes leads us to suggest a close relationship between gastric CEA values and the degree of gastric inflammation. Gastric lavage cytology did not provide a sensitive tool for detection of gastric disorders. This study suggests little value for gastric CEA determination in gastric cancer detection. However, we beleve the association of rising gastric CEA levels with various types of gastritis justifies measuring gastric CEA to recognize “precancerous” mucosal changes of the stomach.

A double-blind trial using 51 outpatients was aimed at comparing the effectiveness of colloidal bismuth subcitrate (C.B.S.) in table and liquid form in the healing of duodenal ulcer. Criteria of entry included endoscopiclly proven duodenal ulcer, duration of symptoms greater than 4 weeks, and the absence of other major systemic disease. Patients were given either C.B.S. tablets (1 four times daily) or C.B.S. liquid (5ml four times daily) for 4 weeks ulcer symptoms and their relief were recorded by patients, along with data on cigarette, alcohol, and drug intake, Endoscopy was performed after 4 weeks to assess healing. By 4 weeks, 18 of 26 patients taking tablets (69%) and 19 out of 25 patients taking liquid (76%), had healed (p = 0.82). Symptomatic improvement was similar with both tablets and liquid. Smoking and analgesic ingestion did not influence healing rate. We conclude that C.B.S. tablets and liquid are equally effective in healing duodenal ulcer.

Six patients had the clinical and pathological features of immunoproliferative small-intestinal disease (IPSID). All were female with malabsorption or growth retardation in whom testing for alpha-heavy chains was repeatedly negative. They were followed for a mean of 7.5 years, during which time no significant alteration in the jejunal biopsy appearances of five patients has occurred, none have developed tympnomas.

In 23 patients with dermatitis herpetiformic (DH) and five patients with linear-IgA bullous dermatosis (BD) we evaluated the occurrence of histologic jejunal changes and small-bowel function abnormalities. None of the patients showed clinical signs or symptoms of malabsorption. Morphological jejunal changes consistent with gluten-sensitive enteropathy were found in 82% of DH patients and in 60% of DD patients. However, DD patients showed only mild jejunal histologic abnormalities, whereas more severe jejunal lesions were found in most patients with DH. Functional tests showed a rough correlation with the severity of the jejunal lesions, being almost completely normal in BD patients and DH patients with mild intestinal damage, whereas most of DH patients with subtotal or total villous atrophy showed abnormal d-xylose tests and folic acid assays. Lactose tolerance tests (H2breath test and blood glucose after oral lactose load) showed no correlation with the degree of jejunal damage.
