Long-term deficits in visuospatial function and memory and disturbances in mood have been clinically identified and followed in four patients occupationally exposed to perchloroethylene (PCE). A frontal/ limbic hypothesis is offered as the site of pathology. A separate study among 65 dry cleaners was conducted to provide similar evidence of impairment, suggesting a continuum between clinical and preclinical effects. Three exposure zones were identified for the counter clerks, pressers, and operators corresponding to air levels of 11.2, 23.2, and 40.8 ppm. Decrements were found for visual reproductions (14.4%), the number correct (6.7%) and the latency (10%) for pattern memory, and the number correct (3.9%) for pattern recognition. Chronic, life-long deficits appear below 50 ppm and require at least 3 years of exposure. A reexamination of the OSHA standard is recommended and shows that behavioral testing can be used as an early indicator of more serious clinical effects.

This article describes the development of a worksite survey to evaluate a continuing medical education (CME) program to improve preventive pulmonary practices and worksite environments of occupational physicians and nurses, and baseline findings from the survey. The two-part instrument assessed individual counseling practices for preventing lung disease and worksite environments. Eighty-one occupational physicians and nurses completed the survey before the CME program. Reliability coefficients (Cronbach's alpha) were calculated for five test subsections: smoking counseling (.72), workplace hazards (.70), environmental programs (.78), workplace environment (.94), and smoking policies (.91). Baseline findings indicated that 80% of respondents reported that their employees/patients were exposed to various workplace hazards. This instrument can be used to evaluate occupational health professionals' performance and worksite environments and may be useful for evaluating change over time or following a training program for occupational health professionals.

An 8-week educational intervention focusing on low-fat eating pattern messages was conducted among employees of the city of Phoenix, Arizona. One hundred nineteen employees with serum cholesterols of ≥5.2 mmol/liter who participated in at least one of eight weekly sessions were compared with 112 nonparticipants. Multiple regression analysis indicated significant intervention effects. For participants, total blood cholesterols decreased an average of 0.22 mmol/liter, low-density lipoproteins (LDLs) decreased an average of 0.30 mmol/liter, and triglycerides decreased an average of 1.91 mmol/liter. High-density lipoproteins (HDLs) increased an average of 0.68 mmol/liter. Significant effects remained after considering initial lipid status, and variation in age, sex, occupation, ethnicity, alcohol intake, fat intake, and BMI. These results constitute a 3.8% decrease in serum cholesterol and a 7.8% decrease in LDLs.

In 1992, the North Carolina Department of Environment, Health, and Natural Resources received 18 case reports of brucellosis from a county health department. All patients had potential exposure to the kill floor of one pork processing plant. A subsequent National Institute for Occupational Safety and Health health hazard evaluation surveyed 154 (99%) of 156 kill floor workers of this plant and found that 30 (19%) had evidence of recent (or persistent) brucellosis. These data show that significant exposure to Brucella is occurring among packing plant workers in North Carolina and suggest that some of the approximately 38,000 production workers in pork processing plants in the United States are at risk of contracting swine brucellosis. Additional measures may need to be taken to prevent occupational exposure to Brucella.
