摘要:
The purpose of this article is to review nursing literature about international partnerships, describe nursing and nursing education in Brazil, and describe an international partnership about chronic wound prevention and treatment in Brazil. Because countries often share health care concerns, nursing knowledge is greater than any one country's health care trends or clinical issues. Although the literature has numerous articles about pressure ulcers and other chronic wounds, much of this information is lacking for nurses in Brazil because it is primarily written in English. This project provided educational programs in Portuguese to nurses and nursing students in Brazil, involved them in the development of wound care protocols, allowed practice with dressings, and stimulated discussions on improvement of home care to patients and families. WOC nurses have a critical global role in sharing wound care knowledge.

摘要:
Although honey has been used as a traditional remedy for burns and wounds, the potential for its inclusion in mainstream medical care is not well recognized. Many studies have demonstrated that honey has antibacterial activity in vitro, and a small number of clinical case studies have shown that application of honey to severely infected cutaneous wounds is capable of clearing infection from the wound and improving tissue healing. The physicochemical properties (eg, osmotic effects and pH) of honey also aid in its antibacterial actions. Research has also indicated that honey may possess antiinflammatory activity and stimulate immune responses within a wound. The overall effect is to reduce infection and to enhance wound healing in burns, ulcers, and other cutaneous wounds. It is also known that honeys derived from particular floral sources in Australia and New Zealand (Leptospermumspp) have enhanced antibacterial activity, and these honeys have been approved for marketing as therapeutic honeys (Medihoney and Active Manuka honey). This review outlines what is known about the medical properties of honey and indicates the potential for honey to be incorporated into the management of a large number of wound types.

摘要:
Success in pelvic floor muscle training as a therapeutic modality for prevention or treatment of incontinence in women is dependent on appropriate patient selection. This article synthesizes data that supports 4 criteria to be met before beginning a program of pelvic floor muscle training: adequate alignment of pelvic organs, intact structural support, functional muscle activation ability, and absence of existing reversible causes for incontinence. Two components of pelvic floor muscle training are described: strengthening exercises and skill development. For each component, protocols of training and refined patient selection criteria are discussed.

ObjectiveThis study explored clients' perspectives of urinary continence service provision for community-dwelling people from a primary health care perspective.DesignFor this interpretive study, data were collected from 11 clients via in-depth interviews and a questionnaire eliciting demographic details and written comment. A focus group was also held with 7 people belonging to an existing continence self-help group.ResultsClients indicated that appropriate and acceptable continence care was accessible, affordable, and based on accurate knowledge. They valued practitioners who were empathetic, interested, had good networks, and could assist in practical ways. They looked for explanation, information, and discussion, often finding this when they accessed continence specialists. However, they often found it difficult to access this type of care, with many clients finding that urinary incontinence was treated as a secondary issue or a nuisance by generalist health practitioners. Clients believed that many health practitioners lacked knowledge and/or interest in urinary incontinence. In particular, there was very poor transitional care between hospital and community. The cost of consulting a health practitioner was seen as secondary to the cost of continence equipment and aids.ConclusionsThis study found that the principles of primary health care were not being realized in generalist continence care. Client-centered services that address individual needs, relationships, psychological factors, emotional needs, financial circumstances, social contexts, and lifestyle factors, particularly in transition between sectors, are needed.
