摘要:
Investigatlon in the area of colorectal I cancer therapy has led to rapid clinical progress. This article explores both current modalities and newer observations. Staging mechanisms and biologic markers are changing, prevalence appears to be shifting to the right, and these changes will affect detection and treatment. Treatment Is matched to the extent of the disease, avoiding morbidity and mortality associated with extensive surgery and preserving continence for many patients. Although surgery Is still the primary therapy for colon cancer, adjuvant therapy Is now accorded an Increasing role. The value of follow-up Is currently under debate, but follow-up Is recommended for those who would be expected to benefit from therapy In case of recurrence. The exciting developments In the field of colon cancer provide great hope for Improvement In outcome.

Famillal adenomatous polyposis, an Inherited condition, can lead to colon cancer If left untreated. Patients seeking treatment for symptoms of this disorder are often found to have cancer already. Because surgical treatment In symptom free persons with familial adenomatous polyposis greatly reduces their future risk of cancer, presymptomatic Identification of the disorder can save lives. Because the disorder Is Inherited, knowledge of Its familial pattern can help nurses recommend candidates for screening. A case study of one family demonstrates the value of genetic analysis and the role nurses can play.

Necrotizing Enterocolitis, a disease that affects the gastrointestinal tract of some premature neonates, often results In a temporary stoma. The fragile skin of these neonates, the small surface area of their abdomens, and the limited ostomy supplies available In appropriate sizes can cause difficult pouching situations. The use of a flexible skin barrier with the smallest available pouch maintains the principles of ostomy care, protecting the skin and containing the effluent.

摘要:
A gastrostomy, creation of a gastrocutaneous fistulous tract, may be Indicated for the pediatric patient with a functional gastrointestinal tract who Is unwilling or unable to consume sufficient nutrition to meet caloric needs for optimal growth and development. A gastrostomy may also be used for medication administration, decompression, or access. Several pediatric conditions or anomalies may necessitate gastrostomy. Traditionally, a silicone catheter with a mushroom tip was placed through a gastrostomy. Recently, however, alternative gastrostomy devices have become available. The MIC-KEY device (Medical Innovations Corporation, Milpitas, Calif.) Is one such device. This article Introduces the ET nurse to the MIC-KEY device as an option and discusses Its advantages and disadvantages In the pediatric patient.

Dlarrhea is common in the acute care setting, particularly among critically III patients. Factors that cause diarrhea are usually multifactorial; some of the most common Include medications, hyperosmolar or rapidly delivered tube feedings, atrophy of Intestinal epithelium or ischemic bowel, short bowel syndrome, pseudomembranous colitis, Infection (Salmonella and Shigella species), opportunistic Infections In patients with acquired Immunodeficiency syndrome and severe hypoproteinemia. This article reviews different types and mechanisms of diarrhea commonly encountered In acute care. It Includes current concepts of managing diarrhea, such as calculation of stool osmotic gap, Identification of medications that cause diarrhea, modification of enteral therapy, and the use of antisecretory agents. Nursing responsibilities and contributions In the collaborative assessment and clinical management of diarrhea are also explored.
