摘要:
Urinary incontinence is common, and considerable advances have been made in treatments and therapies for this distressing and socially unacceptable condition. Most people with incontinence use continence products to conceal their loss in the short term or long term. A huge range of products is available, including absorbent products, collection devices, catheters, and accessories. However, little guidance is available to aid consumers and health professionals in product selection. Therefore, a systematic review of research literature from 1982 to 2000 concerning the effectiveness of continence products was conducted. More than 350 articles were retrieved, of which 75 met the selection criteria for this review. It is apparent that there is a lack of high-quality research into the effectiveness of continence products. A recurrent theme in the literature is the importance of individual assessment to find products to suit the particular needs of each consumer. This review indicates that the absence of research information about the effectiveness of continence aids and appliances makes selection difficult.

ObjectiveThis pilot study was undertaken to determine the effects of a 10-degree leg elevation for 30-degree head-up position on body displacement and sacral interface pressure for 2 hours.DesignThe study used a comparative, quasi-experimental design with repeated measures.Settings and SubjectsA convenience sample of 10 healthy Japanese women was used.InstrumentsBody displacement was defined as the difference over time between the top edge of the mattress and subject's acromion and was measured every 10 minutes by tape measure (in centimeters). Sacral interface pressure was measured every 10 minutes using a pneumatic pressure sensor.MethodsThe subject was placed supine on a standard hospital bed. The head of the bed was then elevated to 30 degrees according to 2 protocols: (1) supine for 10 minutes without leg elevation alternating with 10 minutes of side-lying, or (2) supine for 10 minutes with leg elevation at 10 degrees alternating with side-lying every 10 minutes. Body displacement and mean sacral interface pressures in both protocols were compared by using repeated measures analysis of variance.ResultsThe 30-degree head-up position with 10-degree leg elevation significantly reduced the amount of body displacement at the acromion compared with no elevation of the legs. There were no significant differences in mean sacral interface pressure in either position.ConclusionLeg elevation at 10 degrees in the 30-degree head-up position was effective for reducing body displacement at the acromion; it was not effective for reducing sacral interface pressures.

摘要:
Health care in the United States has evolved into a multimillion dollar business. As the health care industry has grown, so too has government regulation and involvement. As both insurers and patients vie to get the most for their health care dollars, federal and state governments attempt to mediate, prevent fraud and abuse, and protect all parties involved. Consumers feel the effects of this “tug of war” in the form of higher copayments, premiums, and out-of-pocket costs, as well as denial of coverage. This denial of coverage sparked a very successful grassroots effort to stop commercial insurers in the state of Connecticut from defining ostomy supplies as cosmetic and thus denying reimbursement. A tremendous amount of collaboration between Connecticut WOC nurses, state legislators, local American Cancer Society advocates, United Ostomy Association chapter members, and health care providers resulted in a powerful mobilization and support for House Bill No. 5120. This bill went beyond defining ostomy supplies as medically necessary but also set a minimum rate for reimbursement. Social policy changed, improving the lives of Connecticut citizens with an ostomy. Although many people fear they do not have the power to make necessary changes in government, this experience proved otherwise. The collaboration that occurred was patient advocacy at its best. This article describes the process that allowed this successful collaboration to take place with the hope that others will be inspired to get involved with patient advocacy through political involvement. It is the intention of this work to capture the essence of dedication of a grassroots campaign involving a small group of well-organized, highly focused participants who were responsible for changing public health care policy in the state of Connecticut.
