AbstractThe investigation of the vertebrobasilar arterial system by transcranial Doppler (TCD) ultrasonography is difficult due to the numerous anatomical variations in the posterior circulation. To test the validity of TCD, we compared topographic measurements from vertebrobasilar angiograms and corresponding TCD findings in 58 patients with cerebrovascular diseases (N = 37, 21 affecting the vertebrobasilar system), brain tumors (N = 8), and other neurological diseases (N = 13). Normal TCD recordings of the vertebrobasilar system were confirmed by the angiograms in 25 patients, although the noninvasive method failed to separate morphologic variants (N = 15). In patients with pathological conditions of the vertebrobasilar system revealed by either method (N = 17), the combined use of both hemodynamic and radiomorphologic methods improved the diagnostic validity in 24%. We conclude that TCD examination of the posterior fossa circulation provides unambiguous results only if used in combination with cerebral arteriography.

AbstractThe aim of this work is to suggest a new diagnostic approach to the “female varicocele syndrome” which utilizes transvaginal ultrasonography. The presence of circular or linear anechogenic structures with a diameter greater than 5 mm, which were found in transverse and oblique sections of the lateral fornices, was indicative of pelvic varices. The vascular nature of these structures was confirmed with the Valsalva's maneuver and in the upright position. The presence of “pelvic varices” was confirmed by retrograde phlebography of the left ovarian vein in 46% of the cases. AbstractParavalvular leakage is a major complication of prosthetic valve dysfunction. Sixty‐one subjects with valvular heart disease who had received prosthetic mitral valve replacement 5 months to 5 years before (43 received a porcine prosthesis and 18 received Bjork‐Shiley valve prostheses) were evaluated for this complication. Careful auscultation was performed by two experienced cardiologists followed by transthoracic and transeosphageal echocardiography. Physiologic leaks were detected in all Bjork‐Shiley valves, but in only 30% of porcine valves using transesophageal echocardiography. These regurgitant jets were flame‐like, with mean low velocities of 50 ± 12.3 cm/sec and 48 ± 18.2 cm/sec in the two types of valves. Neither transthoracic echocardiography nor auscultation could detect physiological regurgitant jets. Ten cases with paravalvular leak were detected by transesophageal echocardiography and subsequently demonstrated by left ventriculography (7 Porcine, 3 Bjork‐Shiley valves). Pathologic regurgitant jets were seen as high‐velocity, systolic‐retrograde turbulent flow across the prosthesis. However, only 6 cases of prosthetic valve dys‐function were detected by transthoracic echocardiography, 4 cases of mild paravalvular leakage went undetected. Thirteen of the 61 subjects had an apical systolic murmur and suspected prosthetic valve leakage; in 10 of the 13 cases the findings corresponded to those obtained by transesophageal echocardiography. AbstractA study was conducted on a group of 50 obese females with a mean body mass index of 39 ± 16, aged 19 years to 60 years, in order to determine if sonography could be adopted as a direct means for measuring intra‐abdominal fat deposits. Adipose tissue evaluation was performed by computed tomography (CT) and ultrasonography (using 3.5‐MHz and 5‐MHz probes) at the L4 level. CT intra‐abdominal fat area (VAT) and ultrasonography visceral thickness from abdominal muscle to aorta were the values mainly considered. AbstractSonographic examination showed spleen involvement in 43 patients with histological evidence of malignant lymphoma. In 23% of these cases the spleen was of normal size and 77% exhibited variable splenomegaly. Focal lesions could be seen in 27 patients, 16 exhibiting diffuse, small‐nodule transformation of the sonographic parenchymal texture. Hodgkin lymphomas caused both focal (7 of 16) and diffuse (9 of 16), splenic lesions. All non‐Hodgkin lymphomas of high‐grade malignancy exhibited focal lesions, which are larger than 3 cm in diameter in 11 out of 13 patients.
