AbstractThe medium‐term effect of cholecystectomy on common bile duct diameters (CBD) was investigated prospectively in 64 patients with gallstone disease examined by ultrasonography immediately before and 27 months to 39 months after the operation. In 32 patients with chronic cholecystitis and patent cystic ducts, CBD diameters increased slightly (median 4.6 mm pre‐ and 5.3 mm postoperatively;p<0.05). A group of 19 patients with acute cholecystitis or cystic duct occlusion showed a significant decrease (median 7.7 mm pre‐ and 5.5 mm postoperatively;p<0.05). In 13 patients with common bile duct stones, the CBD diameters also decreased significantly after surgical intervention (median 7.5 mm pre‐ and 5.0 mm postoperatively;p<0.05). The widest CBD diameter after cholecystectomy observed in this study was 10 mm. We conclude that either increases or decreases of the CBD diameters may occur after cholecystectomy in patients with calculous gallbladder disease, and that the postoperative evolution is governed by the exact nature of the underlying biliary disease at the time of the index op

AbstractShoulder pain is a common orthopedic problem. Clinical examination is often nonspecific and arthrography is normal in those patients with the most frequent cause of shoulder pain—noncalcific tendonitis secondary to impingement.Ultrasonography has recently shown itself to be of value in the diagnosis of rotator cuff tears. In addition to the diagnosis of tears, however, sonography can demonstrate abnormalities within the intact rotator cuff tendon. These abnormalities consist of changes in echogenicity and thickness of the tendon. The patterns of abnormality demonstrated correlate with pathologic changes seen in tendonitis.This preliminary study suggests that the ability to demonstrate an abnormal rotator cuff tendon has potential for becoming a valuable aid to the orthopedist in allowing confident diagnosis of rotator cuff disease in the presence of nonspecific symptomatology and an intact tendo

AbstractThe sonographic features of normal lower ureters, visualized during routine examination of the pelvis in a series of 40 patients, are described. A comparison with the corresponding excretory urogram was obtained in 18 patients. In the authors' experience, visualization of the ureters is a normal finding that should not suggest stasis or dilatation. Recognition of the ureters, although restricted to the lower segment, can be useful in the evaluation of patients examined for recent or current renal colic in order to assess with greater accuracy the existence of ureteral stones and their position. In addition, knowledge of normal features and of physiological changes could presumably be used in the evaluation of pathological conditions, congenital or acquired, of the ureters.

AbstractFetal arrhythmia was investigated in 148 fetuses. Eight of these fetuses displayed signs of redundancy/aneurysm of the foramen ovale, all in combination with various atrial arrhythmias. At presentation 4 (50%) of these fetuses had mild to moderate congestive cardiac failure and a 5th fetus developed mild cardiac failure four weeks after observation of the aneurysm.All fetuses survived, but 2 fetuses required early delivery because of the cardiac failure. Unnecessary morbidity resulting from the possible development of cardiac failure may be prevented by regular monitoring in these cases.

AbstractIn a prospective study of 1000 consecutive patients, abdominal sonography added diagnostic information to that available by history and physical examination in 8.9%. The yield ranged from a high of 27.4% in patients with acute abdominal symptoms to a low of 2.7% in screening sonograms of patients without abdominal symptoms. Sonographically guided biopsy‐proven diagnoses were obtained in 6.6% of the patients and were clinically not suspected in 4.4%.
