AbstractUltrasonographic features of tumor nodules in 64 patients with hepatocellular carcinoma and 87 with metastatic liver cancer were analyzed for differential diagnosis. Sonographic characteristics of hepatocellular carcinoma were the mosaic, posterior echo enhancement, and lateral shadow patterns, which were rarely observed in metastatic liver cancers. Small hepatocellular carcinomas, less than 3 cm in diameter, showed a hypoechoic pattern. Metastatic liver cancers originating from the gastrointestinal tract were frequently hyperechoic irrespective of their size and the bull's‐eye‐like pattern was specific for metastases from the lung. AbstractThe improvement of sonographic examination of the uterus during artificial uterine cavity distention was studied in 30 consecutive patients in whom a hysteroscopy was indicated. In 10 of 21 patients with a normal sonographic appearence of the uterus, after filling the uterine cavity with Hyskon, various pathologic conditions became visible. In 5 of 9 patients with abnormal sonographic appearance of the uterus, the procedure enabled a more precise diagnosis concerning localization of abnormal echoes in the uterus. The advantage of this method was clearly demonstrated in cases of central echogenic structures, congenital uterine anomalies, myoma's, synechiae, and corpora aliena.

AbstractUnequivocal narrowing of the anterior cranium, with an apparent constriction in the approximate region of the coronal sutures, in the absence of oligohydramnios is presented as a warning sign of spina bifida in the second trimester fetus. It makes detailed assessment of the spine, cerebellum, and other ultrasonographic features imperative and may justify amniocentesis.

AbstractIntestinal gas is the most common cause of technically unsatisfactory abdominal ultrasound scans. In the attempt to increase the visualization of abdominal organs, many investigators use antifoaming agents or laxatives. In the present, randomized, placebo‐controlled, double‐blind trial, a liquid dimethicone preparation and an extract of senna, alone and in combination, were tested as a means to improve scan quality. Response variables were defined on the basis of pancreas, aorta, and kidney visibility. There was no difference in the response variables of the three groups compared to placebo (P>0.3–0.55). AbstractReal‐time B‐mode ultrasound imaging was performed in 24 intubated patients in order to confirm the correct placement of endotracheal tubes. The large acoustic impedance mismatch between the air within the endotracheal tube cuff and the tracheal wall could be bypassed by (1) use of a foam‐cuffed Bivona endotracheal tube, or by (2) cuff inflation with saline instead of air. Optimal repositioning of the endotracheal tube could be done under direct visualization. Imaging of the foam‐filled and saline‐filled cuffs was easier in the longitudinal (sagittal) than in the transverse view, was enhanced by a slight longitudinal to‐and‐fro motion of the tube, and was often improved with the use of a stand‐off pad. Cases of esophageal intubation were not considered. Use of a noninvasive imaging modality such as ultrasound will spare selected patients from the radiation exposure associated with a chest x‐ray. This is of value in pregnant patients and in those requiring frequent chest radiographs for the sole purpose of confirming correct endotracheal tube placement. Limitations of the techniq
