AbstractThe early detection af the macrosomic fetus is important to the obstetric management of pregnancies in which this growth abnormality occurs. Ultrasound has been used for this purpose but for this approach to be effective, a clear definition of macrosomia, selection of the appropriate parameters for measurement and specification of the optimal time for carrying out the ultrasound studies are required. In this review we examine the definition of macrosomia and introduce two new terms, mechanical macrosomia and metabolic macrosomia. We also evaluate the effectiveness of the limited number of parameters studied to date and conclude that they are marginally effective, primarily because of their low sensitivity and the limitations of current ultrasound methods for estimating fetal weight. The use of the growth profile, a multiparameter approach to the evaluation of fetal growth, is suggested as a possible means for improving the effectiveness of ultrasound in detecting the macrosomic fetus.

AbstractUltrasound has become more and more widespread in the diagnosis of breast disease. However, opinions still diverge concerning its exact place in the diagnosis of breast cancer. An understanding and awareness of the principal pitfalls and limitations of this modality is essential for its optimum and responsible usage. An analysis of four interesting cases illustrates the benefits and limitations of this technique.

AbstractPlacentas of 84 antenatal patients have been examined throughout pregnancy (14–40 weeks of gestation). The coefficient of variation (the standard deviation divided by the mean) of echo‐peak amplitude distributions obtained from placental images was used to characterize the placentas in different groups. Placentas of smokers yielded higher coefficient of variation values throughout pregnancy (P<0.05). This placental texture index was significantly lower for nonsmoking mothers of low‐birthweight babies after 30 weeks of gestation (P<0.001). AbstractReal‐time sector ultrasonography is the only imaging procedure that can simultaneously evaluate the paradiaphragmatic spaces, the hemidiaphragms themselves, and their motion. Our experience with the use of real‐time Ultrasonography in the evaluation of 14 cases of suspected abnormalities of diaphragmatic motion and position is presented. AbstractAn analysis of the sonograms of 60 patients with a surgically confirmed diagnosis of ectopic pregnancy was performed. In 36 examinations, both real‐time and static images were compared, and real‐time was judged superior in 22 based on a correlation between the original report and surgical pathology. The appearance of the endometrial canal and the adnexa was characterized in a total of 65 scans. The endometrial canal appearance was variable, with the most common finding being a linear echo (39%). The adnexal changes were less diverse, with an adnexal ring suggestive of an extrauterine gestational sac identified in 60%. In 10 cases, the adnexal ring contained a distinct fetus with a measurable crown–rump length, and cardiac activity was evident in six. Our results indicate that real‐time ultrasound coupled with serum β‐hCG RIA should be the procedure of choice when ectopic pregnancy is

AbstractUltrasonic measurements of biparietal diameter, head circumference, abdominal circumference, and femur length were made in 50 preterm fetuses within 48 h of delivery. Measurements of femur length in addition to fetal head and abdomen significantly improved fetal weight prediction. AbstractSeven female subjects in whom thyroid hemiagenesis was suspected on the basis of clinical findings and scintiscanning before and after TSH administration were studied with ultrasound. Ultrasonic scanning confirmed true hemiagenesis in only three cases. In the other four it revealed a nonfunctional hypoplasic lobe contralateral to the assumed single lobe.
