AbstractFetal cystic hygromas are fluid filled sacs of lymphatic origin occurring in the posterolateral neck. Based on an experience with 15 of these hygromas diagnosed antenatally, the various sonograhic appearances are illustrated and differential diagnostic features are discussed. A midline septum or hydrops may serve as diagnostic clues. The sonographic and anatomic features of a hygroma are correlated using a gelatin embedding technique.

AbstractThe difficulty in distinguishing an encephalocele from a cystic hygroma has recently been highlighted by five patients referred to our unit with an incorrect diagnosis of cystic hygroma. A review of the literature revealed four other cases of misdiagnosis. Based on review of 46 patients with one or other diagnosis, we have attempted to establish guidelines to aid diagnosis. We have always been able to identify the bony defect in the fetal vault in cases of encephaloceles. The pattern of the septa is usually characteristic in cystic hygroma.

AbstractTwenty‐four hyperechogenic nodules ranging from 0.5 cm to 2.5 cm in diameter were observed within the renal parenchyma in 22 patients. Eighteen patients did not complain of renal symptoms, one had microhematuria, one renal colic, and two pain in the right flank. Because of the sonographic features suggesting fat‐containing masses, these lesions were considered to be small angiomyolipomas. AbstractDuplex scanning with the use of continuous wave Doppler instrumentation was used for the evaluation of carotid disease and compared to angiography. In twenty such patients 100% accuracy for diagnosis of total occlusion by Duplex scanning with continuous wave Doppler, was obtained. The sensitivity for those patients with 50%–99% stenosis was 92% and for those patients with 20%–49% stenosis was 75%. The overall accuracy for this diagnostic test for all groups of stenosis was 85%. This compares favorably with other noninvasive vascular techniques and is felt to be an accurate and useful technique for the detection of carotid stenosis. AbstractCritical review of the current medical literature shows lack of complete evaluation of the parameters of “medical decision making” in the sonographic diagnosis of infantile hypertrophic pyloric stenosis (IHPS). A computer program was specially written to calculate these parameters on three published series of patients with IHPS, using originally advocated and modified sonographic criteria. The sensitivity and specificity of the sonographic tests are influenced by the chosen diagnostic criteria, while predictive values are affected by the prevalence of IHPS in the particular population studied. A change in the ventral‐dorsal diameter criterion from 1.2 cm to 1.5 cm increased the sensitivity (from 0.52 to 0.97,p<0.0001) and negative predictive value (0.60 to 0.95,p<0.01) in the population of Wilson et al (9), changing the prevalence of IHPS from 0.58 to 0.12, (sensitivity, specificity and sonographic criterion kept constant) reduced the positive predictive value from 0.90 to 0.48 (p<0.0001). The term “accuracy” has little scientific meaning and should be discarded in comparative evaluation. AbstractA study was made of the fluid from 11 hydatid cysts (10 hepatic, one renal) and from 50 nonparasitic cysts (25 renal, 10 ovarian, 10 pancreatic and five hepatic). Our analysis indicated that the chloride (range 62–95 mEq/liter), the sodium (range 125–139 mEq/liter) and the potassium content (range 5.1–7.5 mEq/liter) of the former differed significantly (p<0.01) from those of the controls.
