AbstractCytologic evaluation of aspirates obtained by ultrasonically guided puncture of circumscribed lesions leads to the diagnosis of a malignant lesion when tumor cells are detected. In the absence of tumor cells, it remains uncertain whether the focus had been biopsied correctly, since the histologic diagnosis of a benign focal lesion can only occasionally be made by means of cytologic smears. Cases are presented showing circumscribed lesions of the liver. With a cut‐biopsy fine needle, small cylinders of tissue were obtained under ultrasonic guidance and examined after histologic preparation. AbstractFor right ventricular volume determination a new method was developed that took into account right ventricular imaging by apical two‐dimensional echocardiography in the four‐ and two‐chamber views. Right ventricular volume was calculated by subtraction of the partial volume of the left ventricle and interventricular septum from total heart volume of the right and left ventricles including the interventricular septum. Accuracy of the subtraction method was evaluated in isolated human hearts by two‐dimensional echocardiography and radiography. Regression analysis indicated that for both the echocardiographic and radiologic procedures, the subtraction method gave the best correlation with the right ventricular filling volume. The correlation coefficients obtained using the echocardiographic procedure were 0.964, 0.848, and 0.851 for the subtraction method, pyramid method, and Simpson's rule method, respectively. For the radiographic procedure, these values were 0.946, 0.860, and 0.872. Percentage difference calculations indicated that the subtraction method systematically underestimated (−7.4%) the filling volume in the echocardiographic procedure, but this was not seen with the radiologic procedures. The random error associated with the subtraction method was only half (10.5%, 13.9%) of that seen with the other two methods in both the echocardiographic and radiological procedures. AbstractThis investigation is concerned with establishing the anatomical location of the site proposed for making fetal thigh circumference (ThC) measurements with ultrasound.1An evaluation of the accuracy of ultrasonic ThC measurements has also been made. The results of these studies indicate that the change in femur profile (polygonal to oval) used to identify the site of measurement (transition plane) occurs at the junction of the upper and middle thirds of the thigh, at the level of the proximal nutrient foramen of the femur. Ultrasonic ThC measurements are systematically larger (mean: 4.3%; range: 1.3–10.3%) than comparable anatomical measurements. Measurements made within 1–2 cm of the transition plane are quite similar, demonstrating that exact positioning of the plane is not necessary. AbstractTo provide standards for evaluating fetal thigh circumference measurements obtained with ultrasound, a study of the growth of this parameter during pregnancy was made. Because of possible sampling problems before 22 weeks (menstrual age), this investigation was limited to the 22–40 week time period. AbstractThe ability of B‐mode ultrasound to follow effectively blood clot echogenicity changes over time under conditions of red cell preservation and lysis was evaluated. Blood clots were produced in cellulose dialysis tubing and then placed into appropriate baths of either saline or water. The echogenicity changes were then followed quantitatively using A‐mode and qualitatively using B‐mode imaging. AbstractThe accuracy of duplex pulsed Doppler echocardiography (d‐PDE) for detecting mitral regurgitation was evaluated in 35 patients undergoing d‐PDE, cardiac auscultation, and left ventriculography. With three transducer positions, the overall d‐PDE sensitivity was 95%, specificity was 100%, positive predictive value was 100%, negative predictive value was 94%, and diagnostic accuracy was 97% compared with ventriculography. This technique was superior to auscultation (sensitivity 74%, specificity 94%, positive predictive value 93%, negative predictive value 75%, diagnostic accuracy 83%). No false‐positive d‐PDE results occurred, but discordant false‐negative results occurred frequently among the three transducer positions. AbstractFifty‐three untreated patients with histologically confirmed Hashimoto's thyroiditis were divided into two groups according to the echogenicity of the thyroid. In 25 cases, the echogenicity of the thyroid was almost the same as or less than that of the adjacent muscles. Such cases were classified as group A. The other 28 cases were classified as group B. Mean T4levels in group A were significantly lower than those in group B. In addition, the cases in group A had abnormally low T4and abnormally high TSH more frequently than those in group B. In all cases in group A, there was severe degeneration and disappearance of thyroid follicles, whereas in most of the cases in group B, follicles of normal size were generally seen.
