An analysis is presented of the clinical and prognostic features of 22 patients diagnosed with diffuse well differentiated lymphocytic lymphoma at Cleveland Metropolitan General Hospital between 1974 and 1987. At presentation, the disease was usually advanced with 86% of patients having stage IV disease and 73% of patients exhibiting bone marrow involvement. Ten second malignancies were documented in 7 (32%) of our patients. The goal of treatment in our patients was palliation except in the 2 stage I patients. The response rate to initial treatment was 55% (15% complete), although 75% of patients ultimately responded to some form of therapy; and the mean survival was 58 months, a figure not different from historical controls. There were no differences in survival between patients given simple versus complex therapy at diagnosis or at any time during the course of their disease; and no differences in survival between complete, partial and nonresponders.

28 consecutive patients with multiple osteolytic bone metastases due to various types of cancer were treated with 300 mg/day of sodium clodronate given intravenously as a 3-hour infusion. 16 patients were treated for 10 days and 12 for 20 days. Our findings showed that clodronate administration causes an increase in serum osteocalcin (sBGP) in the group of patients treated for 20 days (p < 0.05), a reduction in serum Ca in both groups of patients (p < 0.01), an increases in serum alkaline phosphatase (p < 0.05 and p < 0.01), a reduction in urinary Ca (p < 0.01) and a reduction in uOHP (p < 0.05). The raise in sBGP may be attributed to the reduction of sCa and to the consequent secondary hyperparathyroidism. The increase in sBGP can be considered as the expression of osteoblasts stimulation following an adequate period of therapy with sodium clodronate.

The dispersed neuroendocrine system includes cells with different embryological derivations, sharing a common neuroendocrine (NE) program, as indicated by the expression of NE markers, some of which are shared antigenic determinants. We report here that the small cell lung carcinoma cells NC1-H69 express the two human melanoma-associated antigens (HMAA) NGA/LS62 an LSI09. Incubation of NCI-H69 cells with maturational inducers, such as retinoic acid and bromodeoxyuridine (BrdU), upregulated the expression of both HMAA. Exposure to BrdU for 4 weeks induced the appearance of a different phenotype in subpopulations of NCI-H69 cells, which became epithelioid, substrate-adherent, grew in monolayer and continued to express NE-associated antigens in variable amount. The shift in phenotype was not reversible after BrdU withdrawal and was maintained for at least 6 months in continuous culture. The substrate adhesion of NCI-H69 cells was paralleled by a change in NGA glycosylation pattern, thus suggesting a possible functional role for NGA in cell substrate adhesion/recognition.

The local immunologic phenomena in the thyroid gland of 16 patients with chronic lymphocytic thyroiditis (CLTH) were investigated; 5 of these cases were associated with thyroid non-Hodgkin’s lymphomas (NHL). All patients were admitted because of struma, growing slowly in patients with CLTH alone and rapidly in those with associated thyroid NHL. CLTH was confirmed by histologic findings, including the presence of lymphoid follicles or thyroid autoantibodies in the serum in all 16 patients. Immunologic study revealed that all thyroid NHL were of the B cell type. Surface marker study of suspended cells showed that the percentages of E-rosette-forming cells in patient with CLTH and thyroid NHL (38.0 ± 10.0%) and in patients with CLTH alone (45.0± 11.9%) were between those of B cell NHL of lymph node and B cell hyperplasia reported by others. Immunohistochemical studies confirmed the reactive nature of lymphoid follicles. Subset distribution of T and B lymphocytes in patients with CLTH and thyroid NHL markedly contrasted with that in patients with CLTH alone: an increased ration of CD8+ cells (suppressor/cytotoxic cell) to CD4+ cells (helper/inducer cell). There was a marked increase in the number of immunosuppressive acidic-protein(IAP)-containing macrophages in the thyroid lesion and serum IAP level in patients with thyroid NHL. These findings provide evidence of a difference in the local immunologic conditions in CLTH alone compared to CLTH complicated by thyroid NHL.

A single topical application of 1 jag of 12–O-tetradecanoylphorbol-13-acetate (TPA) to the ears of mice was shown to induce edema, and this TPA-induced inflammation was inhibited by 4-methylsterol and triterpene derivatives. The ED50 of these compounds against TPA-induced inflammation was 0.1–3 μmol. Phytosterols had only slight inhibitory effects. Furthermore, application of 5 μg TPA to mouse skin rapidly caused accumulation of ornithine decarboxylase (ODC). Similarly, sitosterol and lupane-type triterpene derivatives markedly inhibited this TPA-induced ODC accumulation. In addition, 5 umol betulinic acid markedly inhibited the promoting effect of 2.5 μg TPA applied twice weekly on skin tumor formation in mice initiated with 50 ug of 7,12-dimethylbenz[a]anthracene, and 5 μmol of sitosterol caused slight suppression. Papillomas in mouse skin were initiated with 390 nmol of 7,12-dimethylbenz(a)anthracene and 1 week later, were promoted 2 times a week with 4.1 nmol of 12-O-tetradecanoylphorbol-13-acetate (TPA). Five saponin-related compounds used as potential anti-inflammatory agents were applied in the same position with 82 nmol. Saponin-related compounds effectively inhibited tumor formation in the sensitive mouse stock even when these compounds were given 1 h prior to TPA treatment. These results suggested that there was a general correlation between the anti-inflammatory and antitumor-promoting activities of saponins.

Using the same experimental system which had been used to determine the pion relative biological effectiveness (RBE) for skin of mouse foot by Chaplin et al., we have determined the pion RBE for KHT sarcoma and SCCVII tumour transplanted in the foot of C3H mice. The pion RBE obtained by the tumour growth delay time method in comparison with 250 kVp X-rays at a dose rate of 150 cGy per min was determined to be 1.20,1.28 and 1.50 with single, 4 and’10 fractions, respectively, for KHT sarcoma. As for SCCVII tumour, it was determined to be 1.17, 1.45 and 2.05 with single, 4 and 10 fractions respectively. Therefore, it has been concluded that pions have various values of RBE depending on the tumour system involved. KHT sarcoma is a tumour which grows very rapidly, on the contrary, SCCVII tumour shows somewhat slower growth characteristics. These differences of RBE between these two tumour systems possibly stem from differences in the amount of hypoxic cells and/or their rates of reoxygenation. As for therapeutic gain factor, a maximum value of 1.45 was obtained with 10 fractions using the SCCVII tumour.
